A fire occurred on the front end loader. The operator (victim) was dumping a bucket of refuse rock on a stockpile. A nearby truck driver noticed an oil leak on the loader and radioed the victim to shut down. The victim dumped the loaded bucket, backed up and drove the loader forward. The loader burst into flames. Fire was located between operator's cab and engine compartment.

Coal lab technician lifted a sample bucket that was too heavy thereby injuring the lumbar region

While cleaning up after a welding repair in sizing screen, the Miner transferred their weight onto their left arm and rotated to swing their legs over side of sizing screen to exit. This caused the Miner to strain their left shoulder.

While changing work shirt at the end of the shift, foreign matter that was on the Miner's shirt got into EE's eye which was removed by an eye care specialist the following day. The Miner missed the following day due to discomfort in their eye.
Employee was working on the Skid Steer Loader. Upon entering the cab of the loader, employee slipped on the wet smooth surface of the loader bucket and injured their left leg and ankle. Employee reported they did manage to maintain three points of contact with their two hands on the grab bars and one foot on the inside of the bucket.

Moving 55 gal barrel, felt burn in lower back into left leg. Injury occurred on 1/11/11, sought medical treatment on 1/27/11. Doctor has him off work till further notice.

Moving 55 gal barrel. Injury occurred 12/21/10. Rechecked on 3/16/11 - off work since 3/16/11.
Standing on ladder, EE slipped and fell to the floor. Landed on buttock. Acc. occured on 10/21/10, did not seek medical attention until 11/12/10. EE has been off work since 11/12/10.
Cutting pipe w/torch overhead, dropped torch and burning tip entered glove, causing burn.
EE experienced chest pain while shoveling material. Pain seems to be heart related as no injuries were reported. Further testing as outpatient is scheduled for heart condition. Tests showed no blockage but irregularities to the vessels were noted. Also drinking power drinks may have brought on symptoms.
Replacing screen on sieve. Weld broke on rail, fell to floor, injuring back.

Grinding on metal with 4" angle grinder - foreign material entered left eye. Accident occurred on 9/2/05 - did not seek medical attention until 9-6-05.
