Ondo Extension Mine Coal

Injured states he was carrying a bag of B-Bond and a jug of water, he slipped in the mud and strained his groin area. The injury was diagnosed as a hernia on 10-10-14 that will require surgery to repair. The employee is currently working his regular job. The accident will become reportable as a NFDL (LTA) once he has the surgery.

Contract employee making pre-shift on section when he began having chest pains. A Mine EMT administered O2 and transported to surface. Although he felt better on the surface, mine management insisted transport to ER by ambulance. He received a cardiac catheterization consisting of Angioplasty and stent implantation for 85% coronary artery blockage. NOT A MINE ACCIDENT OR ILLNESS!

Employee developed soreness in his right knee from repetitive crawling over an extended period of time. Diagnosed by MRI as degenerative changes consistent with cartilage loss as well as degenerative meniscus tearing. Employee continued working his regular job until he had surgical repair of the knee on 10/31/2014.

Employee stated that while assisting co-workers to belt up the section, he lifted a piece of belt structure straining his left shoulder. He continued to work his regular job until he had surgery to repair a torn rotator cuff on 12/2/2014.

Two employees building cribs in 4-East area of mine, as one employee tightened his crib, the sledge hammer handle hit the crib, bounced out of the employee's hand causing the sledge to fly, striking an employee on the left side of his face causing a 3/4-inch laceration on the left side of his face between his lips and nose requiring sutures to close the wound.

Employee lacing belt through belt tail piece when he felt a pop in his right shoulder. Possible shoulder dislocation (He has history of having dislocated this shoulder at least two other times).

Employee tightening down take-up on the E-14 Belt Drive. As the take-up was reengaged, the dolly came back and pinched his hand left hand ring finger between the guide roller on the dolly and the track frame causing multiple lacerations requiring sutures.

Continuous Miner struck - breached an unmapped, active, low pressure / volume gas well in the R2 Entry of E-13 Section. Methane monitor on the miner immediately shut down the continuous miner. Power to the section was immediately shut off, the crew and the rest of the mine evacuated.

Injured riding in Damascus Mac 12 Mantrip traveling out of the mine at quitting time. The mantrip pulled over to the side to allow a Johnson Mantrip to pass by. When the Mac 12 inadvertently brushed a coal rib an Ocenco SCSR Storage Box came up out of the 2" channel brackets holding it in place. The SCSR Box rolled over striking the injured's right lower leg causing a laceration.

Injured was removing side cover off miner when he pinched his left small finger between the cover and a bit block causing a laceration requiring sutures.

Injured and co-worker removing RFM Tail cover/guard in order to re-thread the belt through the tail when a bolt that secures the guard broke allowing the cover to slide down on his right hand and index finger causing crushing injuries and an open wound requiring sutures.

Employee stated that he reached across the RFM with his left arm to pull himself over to the MBC to put oil in the machine when he felt a pain in his left shoulder. The employee continued to work his regular job until 10/01/2012 when he had surgery to repair his rotator cuff.

While using a razor knife held in his left hand to cut zip ties off of hydraulic hoses in scoop tractor center section, the knife skipped across a zip tie and cut into his right wrist positioned approximately 18" away.

Employee was removing a 4-foot drill steel from the roof, when the starter steel fell out of the roof striking his right pointer finger causing a laceration requiring sutures.

Roof fall occurred in the 4-East section one cross-cut inby the RHS tail piece. The fall measured 24' long x 18' wide x approximately 12' high. The area was bolted with 6-foot FG resin bolts on a 3' x 4' pattern with large bearing plates and 8-ft to 12-ft cable bolts. A coal rider was detected at 12-feet.

Employee starting to bolt an open 20 foot cut in which roof rock had been dislodged and fallen out. After setting the ATRS and while swinging out his safety post canopy, the roof began chipping and the injured was struck by a piece of falling roof rock estimated to be 3.5' long x 2' wide x 10" thick. The rock caused him to fall striking his right shoulder and ribs on the bottom.

Employee stated that while shoveling coal along the beltline, he felt a "pop" in his left shoulder.

After scaling loose roof rock in previously bolted area belt - R1, a roof bolt that was previously tight to roof was now protruding away from the roof. As Injured pushed exposed roof bolt tight to the mine roof, his right hand, that he had resting on the drill head, got pinched between the drill head and the safety post canopy causing a laceration requiring stitches.

While removing a front rub rail off the side of the Continuous Miner, the back rub rail fell off striking the employee on the top of his left foot causing a fracture.

Employee was assisting on a belt move, when a jack set to hold the RFM came loose and allowed a crib to fall from on top the jack and smashed his left thumb between the crib and RFM, causing an open fracture.

Injured entering the mine to begin shift when a he was hit by small piece of laminated shale roof rock - causing a 1/2" laceration to his forehead requiring stitches.

Employee & co-workers replacing belt drive motor. The motor was blocked and lifted by come-a-long onto the bed plate. Employee using a rock bar to assist in positioning the motor onto the bed plate. During positioning motor, supporting eye-bolt stripped out of top motor. Motor fell 10" striking employees lower left leg causing a simple fracture.

Employee was drilling a hole when the drill steel stuck in the hole. He raised the head to reengage the steel and caught his right hand between the drill head and the roof causing injury to his thumb and index finger

Employee claims that while operating his scoop tractor on rough bottom, he started to experience pain in his neck and left shoulder. Employee continued to work his regular position when on 01/29/2010 he informed mine management that his neck and shoulder pain was increasing and he was going to go to a doctor. Employee did not report to work on 02/01/2010.

Employee states that as he was placing a 5 gallon can of oil on his jeep he accidently pinched the tip of his left index finger between the can of oil and the frame of the jeep causing a simple fracture to the tip of the finger. While the accident occurred on 01/13/2010, the employee did not go to the doctor until 01/22/2010, and did not inform mine management until 01/25/2010.

Employee was working with another employee installing water line using a crib to assist. His hand slipped and his left thumb was caught between the crib and the water line.

Employee and co-worker were carrying a drill head for a Fletcher Twin-boom Roof Bolter. The co-worker slipped losing control of the drill head, which caused the injured employee to pinch his left middle finger between the drill head and the coal rib, causing a fracture and laceration.

Employee states that he was moving rocks in preparation to bolt a cut when he felt a pain in his right groin. He worked his regular job until the scheduled miner's vacation (June 29 to July 5). Employee was disabled by Dr. Bucci effective 7/6/09 for a right inguinal hernia.

Employee and co-workers were attempting to raise the belt drive spill-board into place. The piece of heavy metal was too heavy to hold and started to slip from the worker's grasp. The injured worker had his hand caught between the spill-board and the belt rope causing a laceration and a fracture to the finger tip of his right hand.

Roof fall found by examiner in worked out area of Main A 4 two and one-half cross-cuts outby spad 2769. Fall measurements undeterminable. Roof supported with 4' fully grouted resin bolts (4 bolts across) with 6' x 6' plates. No known defects. Bottom heave contributing factor.

Injured was drilling hole when a small piece of shale roof rock struck him on his forehead causing a laceration which required sutures.

Employee was removing rockdust from scoop bucket when the scoop moved forward knocking him down. Scoop bucket ran up on him causing injury to his left foot, leg, thigh area.

A roof fall occurred in intersection in the beltline adjacent to spad #4430 in the 2 Left North Section. The cave measured approx. 25' x 18' x 12-15' high. Roof in area supported by 6' fully grouted resin bolts (4'x4' centers) plus 2 - 12' cable bolts. Shale and slicken-sided roof.

Employee was standing in intersection preparing to cross over the continuous haulage unit, when a piece of soap stone rock approximately 36" x 24" x 1" on the ends to 10" in the center, broke loose from a roof bolt and fell striking him on his right foot.

Employee was in the process of hanging belt pull cord, when a small piece of roof rock fell striking him on his right shoulder and cheek, causing a bruise to both.

Walking toward telephone in Surface Shop tripped over ground clamp cable and struck right knee on concrete floor.

After inserting the drill steel into the drill chuck, he pulled forcefully on the drill steel to align the drill head. The drill steel suddenly pulled out of the drill chuck pinching his left thumb between the end of the drill steel and the safety post. The employee was operating the operator side of the Fletcher bolter. The employee was wearing metacarpal gloves.

Employee was lifting a belt rail out of the stand when he felt a pain in his lower back. The Employee was in a kneeling position with both arms under the belt rail. The belt rail was stuck so he gave a sudden upward motion straining his lower back.

Federal Mine Inspector was examining worked out area (2 H Rooms) when he found a cave 1 cross-cut inby spad 1339. Cave extended from R1 across belt to L1. Cave was 50' long by 18' to 50' wide by 5' to 6' high. Area supported by 4' resin bolts on 4' x 4' pattern, cribs, & steel posts. Ventilation not affected. Cave will not be cleaned.

Employee was pulling the belt pin (spindle) on a splice using a claw hammer when he was struck in the eye with a sliver of metal which required eye surgery.

Employee was participating in a practice alternate escapeway drill while crossing over a belt cross-over he lost his balance and fell off the cross-over striking his right side. Accident was not reported until 12/4/06 at that time he was sent to the doctor.

Employee was removing a 5 gallon oil can from the top of a pallet of oil at shoulder level. As he moved the can, it pulled sideways causing his shoulder to twist. Employee continued to work his regular job until 11/16/06.

Injured was in the process of pulling a belt splice pin (spindle) by striking a claw hammer with a mason hammer when a piece of steel spalled off the mason hammer striking and becoming lodged in the lobe of his right ear.

Employee was attempting to break off a roof bolt which had bent during installation. He had used a hacksaw to partially saw through the bolt and was rocking the bolt back and forth to break it off. He gave one extremely hard push, the bolt broke off, he lost his balance falling onto the roof bolt plate causing a laceration on his right forearm.

Roof Fall discovered by Mine Superintendent during his weekly examination of a worked-out area, 2 Left Rooms off 2 Right South, Spad 3426, The Cave was approximately 50' long x 18' wide x 5' high. Area was supported with 4' resin bolts on 4' x 4' pattern

Employee was loading roof bolts into the scoop bucket when the metal retention band on the bolts came into contact with and cut his left index finger

Employee was shoveling coal along the rib when a piece of roof rock, 5" to 2" tapered by 28" wide by 52" long fell from between permanently supported roof bolts and struck the employee on his left shoulder and lower left leg

Employee was assisting co-workers in loading a belt take-up into a scoop bucket by retracting the dozer blade which was hooked to the take-up using a chain. Injured employee had his right hand on the scoop bucket frame and used his left hand to push on the chain. His right hand got caught between the dozer blade and the frame of the scoop bucket.

Employee was attempting to get slack in the miner cable by stepping down on the cable. As he pushed his leg downward he felt a pop in his knees (meniscus tear - both knees).

Employee was removing a belt clamp from the 42" top belt. As he removed the clamp collar, the belt clamp moved suddenly upward, striking the employee on his forehead and causing a laceration that required stiitches.

Employee operating Shuttle Car when he hit a bump in the roadway causing a sudden jarring motion which caused a whiplash to his neck.

Employee was installing metal equipment doors. He lost his grip on the door allowing it to fall and strike his left hand causing a laceration requiring 4 stitches.

Employee cut metal retention band from bundle of roof bolts. Band sprung up and struck his left arm, causing a laceration requiring 4 stitches.

EMPLOYEE WAS SETTING UP THE TRANSIT IN THE 2 RT SOUTH SECTION OF THE MINE WHEN HE FELT SOMETHING GIVE WAY IN HIS BACK. EMPLOYEE WORKED REMAINDER OF SHIFT. EMPLOYEE HAD DR'S APPT 11/15/05 AND WAS PRESCRIBED MEDICATION. HE WORKED REMAINDER OF DAY FROM HOME OFFICE AND RETURNED TO NORMAL DUTIES ON 11/16/05.

Employee was using a belt knife to whittle a splinter from his hammer handle when the knife slipped and lacerated his arm. Employee received 6 stitches.

Operating Johnson Mantrip when steering knob struck back of hand

Employee was dragging 2 pieces of 4" x 20' water line when he experienced a burning sensation in his center groin area.

Employee was operating mantrip when steering knob struck his hand.

Injured was operating scoop tractor when it articulated suddenly causing his left hand to slide between the frame of the machine and the rubber bumper block. The skin and nail of his left middle finger tip was removed exposing the inner flesh.

THE ROOF FELL IN R1 OF 4TH RIGHT IN THE RETURN. IT FELL FOR 35'X 5'HIGH.

THE ACCIDENT HAPPENED IN MAIN A#S. THE BOLTER WAS BOLTING THE BELTLINE, THE OPERATOR OF THE ROOF BOLTER WAS OBSERVING THE ROOF WHEN THE RIB BROKE FREE FROM THE CORNER HITTING THE OPERATOR IN THE FOOT. THE OPERATOR INJURED HIS LEFT FOOT.

WHILE DRIVING A JOHNSON 4 WHEEL PERSONNAL CARRIER VEHICLE HIT A ROCK IN ROADWAY & THE STEERING WHEEL WHIPPED OUT OF HIS HAND AND THE KNOB STRUCK EE ON THE RIGHT THUMB CAUSING PAIN.

WHILE RETRIEVING HIGH VOLTAGE CABLE - A LENGTH OF CABLE FELL FROM ITS HANGER AND STRUCK EE ON THE LEFT BACK OF WRIST. THE CABLE WAS DE-ENERGIZED PRIOR TO REMOVAL.

While lifting a piece of belt material using a rock bar (belt dolly), employee had a pain in his back which moved to his left leg.

WHILE TRAMMING BATTERY POWERED SCOOP-EE RAN OVER OBSTRUCTION ON MINE FLOOR CAUSING SCOOP TO BOTTOM OUT-JARRING EE'S BODY CAUSING HIS BACK TO HURT-EE WORKED WITH PAIN TILL HE COULD NO LONGER GO - LAST DAY OF WORK WAS 3-27-04.

WHILE LOADING CABLE IN THE BUCKET OF SCOOP, EE'S FOOT GOT STUCK IN THE MUD AND HE WAS UNABLE TO GET CLEAR FROM THE MOVING SCOOP. THE BUCKET HIT THE TOP OF HIS FOOT BRUISING IT.

EE ASSISTED WITH AN UNDERGROUND SURVEY AND AFTER THE WORK WAS FINISHED EE NOTICED MILE IN THE SHOWER WHAT SEEMED LICKE A HERNIA CAUSE IS UNKNOPWN BUT EE SUSPECTED THAT CARRYING THE SURVEYING G EAR IN A CROUCHED POSITION MAY HAVE CAUSED THE HERNIA.

WHILE LIFTING A PIECE OF BELT STRUCTURE EE HAD A PAIN IN HIS LOWER BACK.

WHILE LIFTING A PIECE OF BELT STRUCTURE EE HAD A PAIN WITH BURNING IN HIS LEFT THIGH.

WHILE BOLTING THE FACE OF L1 THE FOOT JACK BROKE ALLOWING THE CANOPY TO DROP SUDDENLY A COUPLE INCHES STRIKING EE ON THE HARD HAT.

BUMPED LT. KNEE WHILE ENTERING OPERATORS COMPARTMENT OF SCOOP.

WHILE UNLOADING 8" YELLOW PUC PIPE EE CAUGHT HIS RIGHT THUMB BETWEEN PIPE AND FLOOR, FRACTURED TIP BY RT THUMB, 2 STITCHES.

WHILE CLEANING UNDER BATTERY TRAY OF SCOOP, EE HIT HIS RIGHT HAND ON THE METAL FRAME OF THE SCOOP AND A PIECE OF METAL ENTERED HIS MIDDLE FINGER OF HIS RT. HAND. HE LATER REMOVED THE SMAL SLIV ER OF METAL HIMSELF.

WHILE TRYING TO PULL ONE SHUTTLE CAR OUT WITH ANOTHER SHUTTLE CAR, THE CHAIN BROKE STRIKING VICTIM IN BACK OFHEAD AND RIGHT SHOULDER.
