Roof fall occurred right at the pit mouth at end of canopy where you start underground. Best contributing factors to my knowledge would be years of weathering on roof and lots of rain that week. Fall occurred on intake entry. Still had plenty of air going over fall to ventilate intake travelway.

Injured states he was carrying a bag of B-Bond and a jug of water, he slipped in the mud and strained his groin area. The injury was diagnosed as a hernia on 10-10-14 that will require surgery to repair. The employee is currently working his regular job. The accident will become reportable as a NFDL (LTA) once he has the surgery.

Contract employee making pre-shift on section when he began having chest pains. A Mine EMT administered O2 and transported to surface. Although he felt better on the surface, mine management insisted transport to ER by ambulance. He received a cardiac catheterization consisting of Angioplasty and stent implantation for 85% coronary artery blockage. NOT A MINE ACCIDENT OR ILLNESS!

Employee developed soreness in his right knee from repetitive crawling over an extended period of time. Diagnosed by MRI as degenerative changes consistent with cartilage loss as well as degenerative meniscus tearing. Employee continued working his regular job until he had surgical repair of the knee on 10/31/2014.

Two employees building cribs in 4-East area of mine, as one employee tightened his crib, the sledge hammer handle hit the crib, bounced out of the employee's hand causing the sledge to fly, striking an employee on the left side of his face causing a 3/4-inch laceration on the left side of his face between his lips and nose requiring sutures to close the wound.

Continuous Miner struck - breached an unmapped, active, low pressure / volume gas well in the R2 Entry of E-13 Section. Methane monitor on the miner immediately shut down the continuous miner. Power to the section was immediately shut off, the crew and the rest of the mine evacuated.

Injured riding in Damascus Mac 12 Mantrip traveling out of the mine at quitting time. The mantrip pulled over to the side to allow a Johnson Mantrip to pass by. When the Mac 12 inadvertently brushed a coal rib an Ocenco SCSR Storage Box came up out of the 2" channel brackets holding it in place. The SCSR Box rolled over striking the injured's right lower leg causing a laceration.

Roof fall occurred in the 4-East section one cross-cut inby the RHS tail piece. The fall measured 24' long x 18' wide x approximately 12' high. The area was bolted with 6-foot FG resin bolts on a 3' x 4' pattern with large bearing plates and 8-ft to 12-ft cable bolts. A coal rider was detected at 12-feet.

Employee starting to bolt an open 20 foot cut in which roof rock had been dislodged and fallen out. After setting the ATRS and while swinging out his safety post canopy, the roof began chipping and the injured was struck by a piece of falling roof rock estimated to be 3.5' long x 2' wide x 10" thick. The rock caused him to fall striking his right shoulder and ribs on the bottom.

As Injured pushed exposed roof bolt tight to the mine roof, his right hand, that he had resting on the drill head, got pinched between the drill head and the safety post canopy causing a laceration requiring stitches.

During positioning motor, supporting eye-bolt stripped out of top motor. Motor fell 10" striking employees lower left leg causing a simple fracture.

Employee continued to work his regular position when on 01/29/2010 he informed mine management that his neck and shoulder pain was increasing and he was going to go to a doctor. Employee did not report to work on 02/01/2010.

Employee states that as he was placing a 5 gallon can of oil on his jeep he accidently pinched the tip of his left index finger between the can of oil and the frame of the jeep causing a simple fracture to the tip of the finger. While the accident occurred on 01/13/2010, the employee did not go to the doctor until 01/22/2010, and did not inform mine management until 01/25/2010.

Employee and co-worker were carrying a drill head for a Fletcher Twin-boom Roof Bolter. The co-worker slipped losing control of the drill head, which caused the injured employee to pinch his left middle finger between the drill head and the coal rib, causing a fracture and laceration.

Employee and co-workers were attempting to raise the belt drive spill-board into place. The piece of heavy metal was too heavy to hold and started to slip from the worker's grasp. The injured worker had his hand caught between the spill-board and the belt rope causing a laceration and a fracture to the finger tip of his right hand.

The employee was operating the operator side of the Fletcher bolter. The employee was wearing metacarpal gloves.

Federal Mine Inspector was examining worked out area (2 H Rooms) when he found a cave 1 cross-cut inby spad 1339. Cave extended from R1 across belt to L1. Cave was 50' long by 18' to 50' wide by 5' to 6' high. Area supported by 4' resin bolts on 4' x 4' pattern, cribs, & steel posts. Ventilation not affected. Cave will not be cleaned.

Injured was in the process of pulling a belt splice pin (spindle) by striking a claw hammer with a mason hammer when a piece of steel spalled off the mason hammer striking and becoming lodged in the lobe of his right ear.

Employee was attempting to break off a roof bolt which had bent during installation. He had used a hacksaw to partially saw through the bolt and was rocking the bolt back and forth to break it off. He gave one extremely hard push, the bolt broke off, he lost his balance falling onto the roof bolt plate causing a laceration on his right forearm.

Injured employee had his right hand on the scoop bucket frame and used his left hand to push on the chain. His right hand got caught between the dozer blade and the frame of the scoop bucket.
