A miner reported that EE felt discomfort in EE's left eye when arriving to work on Monday 6/25/2018. The Saturday 6/23/2018 prior the miner was assisting in the replacement of a pump. The miner was sent to an eye doctor who brushed out a small metal chip. A prescription eye drop was prescribed on 6/26/2018. The miner does not recall exactly when the eye discomfort began.

As employee was making adjustments to shroud bolts to try and eliminate noise, the fan/shroud assembly broke apart which allowed the employee to fall against the rotating fan assembly of the 600 HP, 480 VAC, TEFC (totally enclosed fan-cooled) electric motor which was rotating at approximately 1787 RPM.

EMPLOYEE WAS TALKING WITH ANOTHER EMPLOYEE IN THE CONTROL ROOM OF THE MILL, WHEN HIS HEAD WENT BACK AND HIS EYES ROLLED INTO HIS HEAD. THE OTHEREMPLOYEE TRIED TO SHAKE HIM AND GOT NO RESPONSE FROM HIM. THE EMPLOYEE HAD NO PULSE AND WAS NOT BREATHING. THE EMPLOYEE HAD SOMEONE CALL 911 AND BEGIN CPR.
