Employee was trimming excess aluminum honey comb off of panels using 10lb Sawzall. Process involves a lot of bending, twisting which caused back strain. Injury was not reported until 8/21/25 when they were treated for back strain.
Employee's annual audiometric test determined EE has a Standard Threshold Shift (STS) in EE's left ear.

Employee was hurrying to get out of the rain as heavy rain was coming down during severe thunderstorm. In haste, they got their left middle finger caught between door and door frame of forklift as door slammed shut, causing laceration requiring sutures. No lost time, no lost wages.
Employee was setting up saw at quarry, walked around the end of saw tracks which were close to edge of granite block, EE tripped over the saw tracks and fell, hitting EE's head on sharp edge of stone. Has a laceration above left eye which required sutures, some facial scrapes and a fractured nose.

Employee was trying to level granite block for sawing, using hydraulic jack. Thinner roughback that wasn't braced tipped over, struck employee, knocking EE down onto the saw frame, and then to the concrete, injuring EE's back, left shoulder and arm. Lumbar fracture, lost time injury.
Employee's annual audiometric test determined EE has an STS in EE's right ear.
Employee operates loader over quarry terrain that is rough due to rains & heavy traffic, causing potholes & washouts. Employee has been jostled around in loader seat continuously for weeks, resulting in low back pain, left leg pain. Originally felt it was aggravation of old injury, but filed new report on 5-28-2020.
Employee was setting drill bar of trimmer 200 on granite block when the inside feet on the bar slipped & twisted to the side. The outside right leg on bar set on employee's foot, just behind steel toe. Dislocating right second toe, requiring pin placement. Lost time injury.
Employee was working in Texas on temporary project. EE was pulling a stuck wedge out of a broken block. When it came loose, the upward motion pulled towards another wedge and left thumb got pinched in between the two wedges, fracturing left thumb. EE is working light duty. No lost time.
Employee was operating hydraulic wedger, wedger hung up in stone, section of stone fell over with wedges still stuck in stone, causing tool bar to swing into EE's right shoulder, knocking EE over to same level onto EE's left shoulder. EE sought medical treatment however didn't receive restrictions until 2nd visit which was 2/11/2020. Is now working light duty. No lost time.

Employee was building wood A-frames for transporting granite slabs. While working with the wood, they got a wood splinter embedded in right ring finger. EE thought it was all removed, however continues to fester, feels it may be infected. Saw doctor, received prescription for antibiotic. Feels it may still need to be removed somehow. No lost time. Foreign body right ring finger.

Injured was moving set of "L-frames" with overhead crane. The frames came together and hand got caught between.
Right thumb was caught between table and piece of iron while trimming to size.
Improper lifting procedure to recover fallen handle held stone breaker. Pulled muscle in hip. 2-day lifting restrictions of 15 pounds.

Accident was first reported to Safety Manager, on Jan. 4, 2010, at 8:45 am. One of the employee's job duties is Loader Operator which requires him to pull the wheel chocks used to secure the loader while it is parked. While performing this duty, he felt a pain in his lower back but continued to work. EE is on restricted light duty, with no lost time.

Employee was loading slabs on L frame when his glove slipped and the tip of his finger slid between slabs. Employee is on restricted light duty, no lost time.
Employee was unhooking a cable from a bundle of slabs when the 5/8" cable grabbed his glove and twisted his shoulder / back causing a mid-back muscle sprain.

Employee was lifting objects weighing 30 to 40 lbs. while bracing a saw when he felt some discomfort in his lower back. He chose not to go to the doctor until 9-11-2006 where the doctor diagnosed him with sciatica/back problems. He gave him some medications and put him on light duty restrictions.

Lifting a stone-breaker while breaking stone; acute lower back strain.
Employee had climbed an eight foot ladder to unhook a chain from an air compressor. About 15 to 20 minutes later he felt a pain in his lower back. It got better and he chose not to go to a doctor until July 1, 2005 when it began hurting again. (Lower back strain)

EE WAS LOWERING A STONE-BREAKER TO ANOTHER LEVEL USING AN AIR HOSE TO DO SO. HE LEFT THE AIR ON WHILE LOWERING THE BREAKER. THE HOSE CAME LOOSE FROM THE BREAKER, STRIKING HIM IN THE FACE THUS CAUSING THE LACERATION TO THE UPPER LIP AREA.

EMPLOYEE WAS BREAKING A GRANITE BLOCK WITH WEDGES AND WHEN THE BLOCK BROKE IT TILTED SLIGHTLY, CAUSING EMPLOYEE TO LOSE HIS BALNCE THUS LACERATING HIS RT FOREARM ON THE CORNER OF THE GRANITE BLOCK.
