EAST LOOP 375 SAND PLT Metal/Non-Metal

The employee was working inside of a hopper when a loader operator brought a load of material and dumped it on top of the employee and immediately his coworker began digging him out,uncovered within 15 minutes, he was taken to the hospital for testing and released ok. The next day he had a swollen ankle on his right foot and is being treated with modified duty release.

Employee was getting down the water truck's cabin and when first stepping on the ground he twisted his left ankle. The ground was level; employee did not follow three points of contact procedure. Last training on three points of contact before the injury was during 2010. The employee was retrained the same day of the incident.

The lead person stopped a worker because he was using a rubber cord instead of a safety pin to secure the jack of a service trailer. When they tried to replace the rubber cord it got loose and hit the supervisor on the eyelid causing a cut. Medical attention was provided; he got two small sutures on the eyelid and was release for full duty at the moment.

Employee was working at the washer plant & he went to clean mud filters by opening 2 different valves. When attempting to clean the 2nd one he missed the first step of the stairway & cut his forehead at the edge of one of the steps. At the clinic, no concussion or fissure was detected. He got 11 stitches (1.5" cut at the forehead)& was released for regular duty.

As the employee exited the water truck he slipped on a step causing him to fall on his back.
