A rack of scaffold equipment was being hoisted with a forklift. The rack bumped into the scaffold structure and started to fall off the forks. The worker attempted to push the rack back onto the forks when the rack tipped forward. The worker pinched the tip of middle finger between the rack and a hand rail. Worker received one suture an antibiotic and pain meds.

Employee walking across the yard when EE's right knee gave way.

Employee was walking between equipment and slipped on the ice.

Performing maintenance on vibrascrew. Got lime dust in eyes despite wearing safety glasses.

EE was going down to close the lids on a railcar. As EE turned to reach down to close the lid, EE felt a pop in right knee.

While exiting truck on scale, lost balance and fell against safety rail. Twisted knee when falling against rail.

Slipped when exiting truck falling to the ground.

A dust bowl of lime blew into face penetrating safety goggles. Diagnosis - Acute Toxic Conjunctivitis of the right eye.

While opening the lids on trailer, grabbed latch and pulled up. It was stiff and didn't open so EE pulled harder. It snapped open and twisted EE's wrist. EE didn't think much about it until EE was home and it started hurting pretty badly.

While loading, employee looked up and a couple of handfuls of kiln dust fell from silo into eye.

Worker was repairing a belly dump trailer. EE used a jack to jack up the trailer. When the EE reached through the gates to move a railroad tie. EE either hit the jack or the jack moved in the dirt causing the trailer to drop, closing the gates on the EE's arm. Barney trucking reported verbally to MSHA on 10/20/2020.

Contractor's employee smashed fingers under a brick while unkeying brick ring for removal from kiln.

Employee was cutting holes in the bottom of a plastic tool bucket with EE's Leatherman knife to drain water out. The knife slipped and EE cut EE's hand.

Employee was cleaning product from the ground in a congested area with a skid steer loader. The loader bucket caught the cement pad jerking the equipment. EE's right index finger was pinched between the controls and a cross beam.

Contractor's employee was changing a tire on a trailer. The rim was stuck on the hub. When they pried it off, the rim fell on the air hose severing it. The air hose then whipped around through the air striking the employee in the right eye.

Employee twisted knee while unplugging a feeder tube with pry bar.

Employee was lifting wear liners for the coal mill repetitively. At the time ee did not feel this was anything more than a sore back from a long day of work. Two days after the heavy lifting ee notified supervisor that back was hurting and not getting better.

Descending a ladder the employee missed the second to last rung slipped and jarred his back. The employee reported the incident then. On 2/1/16 he reported to his supervisor that his back was giving him problems. He was taken to the emergency room on 2/2/16 for evaluation. He then followed up with his own physician which prescribed an anti-inflammatory prescription on 2/3/16.

The employee was exiting the 660 Drill to go to lunch. He had spent the morning drilling on the Big Sage Road. As he went to close the door to the drill he lost his footing, he had his hand on the bottom of the door, which caused him to pinch his right index finger between the door and the frame. It was noted that he fractured the top of his finger required several stiches.

Employee received a foreign body in his right eye while working in windy conditions while wearing safety glasses.

Employee was walking across ground - fell on loose gravel lacerating left lower arm.

Employee caught fingers in crusher when changing the grid.

Employee was raking ash. He lost his grip on the rake handle. He made a grab for it and pinched his thumb between the handle and a handrail.

Employee pinched r little finger between rock and wall of crusher.

Employee was opening truck lids when the wind blew it shut causing the bar the employee was holding to slip and strike him in the face.

Employee was raking ash. Felt pain in arm.

Employee moving an electrical motor from #4 screen house to door. He felt a slight burning pain in left groin area.

Employee walked by fan that blew a piece of lime into his left eye. He was wearing his safety glasses.

Hyper-extended knee while walking down stairs.

Employee was repairing a ram. Lifting ram with bar. The bar shifted allowing the ram to pinch finger causing a laceration on finger.

Secretary was working in the office late & when she went to lockup, as she opened the office door, the wind shipped the door open catching her purse on the door handle which then jerked her out the door. As she went to pull her purse loose she lost her balance & fell down the stairs striking the left side of her face on a stair or handrail. This resulted in a broken tooth.

Employee was emptying the baghouse hopper. The dust from it went up pant leg.

EE was using a 7" grinder, grinding a piece of steel that had to be cut with a torch. He was wearing oily gloves from the before drilling and the grinder slipped in his hand and the disc cut into his forearm.

EE was carrying scaffolding planks in his arms when he dropped the plank on the ground a wood splinter went into his right arm. He removed most of the sliver himself but a piece remained.

EE came off the ladder with his tool bucket, did not see opening. He fell approx 3'. His chest hit his tool bucket, which caused the bruising.

Employee was pulling on hand brake on railcar - dislocated shoulder

Employee was tightening coal roll bolts with a wrench and felt pain in left shoulder. This was not immediately reportable but became reportable when it did not heal. He is scheduled for surgery 1/16/09. Employee has not missed any work as of today with this injury.

FELL ON SHOULDER WHILE DRILLING. BOON MOVED SURPRISING EMPLOYEE CAUSING HIM TO LOSE HIS FOOTING ON THE ROCK.

Employee was assisting in clean up activities during refractory removal inside kiln #4. While removing debris from a rubble pile, a brick fell hitting his left hand resulting in lacerations to his thumb and fractures to the thumb and first finger.

EE twisted knee. Addendum: Accident happened on 9-7-07 but was not reportable at that time. On 9-20-07 had to go to doctor, doctor drained knee, wants to do MRI.

Employee cut finger on fan.

Employee fell while wearing a dust mask with plastic exhalation valve and lacerated his right nostril requiring stitches. Most likely cause of the cut was from the edge of the plastic valve on the dust mask.

Employee was cleaning up an area. He picked up a piece of metal that had a burr at the end. It cut through his leather gloves.

Employee injured lower back while shoveling and sweeping material.

While hauling limestone fines, tractor trailer tipped over injuring driver's shoulder and rib.

EE was cutting rubber sheet with razor knife and cut left hand.

Employee was changing drive belts on motor, caught finger in belt injuring finger, knee and head. (per phone conversation with company: EE slipped & fell, hitting his head on a cement pier and his knee on a motor base after freeing his finger from the pulley)

Working on level ground, he cut his finger with a razor knife fixing lights on a trailer.

Slick road pre-wet to be ready for dust control. Tag axle in front of driver was down making it hard to steer. 70+ mph winds and too much speed. Driver required three stitches to his head.

WHILE REPLACING THE RUBBER SKIRTING ON A CONVEYOR, THE EE SLIPPED AND INJURED HIS RIGHT CALF MUSCLE.

HE WAS PUTTING ON GLOVES AND WAS BITTEN BY A BUG IN THE GLOVE (UNDER INVESTIGATION).

WORKER WAS PERSPIRING HEAVILY AND RUBBED CLOTH GLOVE AROUND HIS RIGHT EYE SOCKET. QUICKLIME DUST ON THE WORKERS GLOVE REACTED TO THE WATER AND CAUSED BURN.

ATTACHING FROG TO RAIL & FELLOW WORKER LOST HIS GRIP RESULTING IN EE'S FINGER BEING SMASHED BETWEEN FROG & RAIL.

WORKER WAS USING A PICK AXE TO CHIP AWAY A BUILDUP OF FINES AGAINST A HOPPER WALL. HE SAID HE FELT A POP IN HIS RIGHT INDEX FINGER BUT CONTINUED WORKING. AT THE TIME, CLAIM WAS NOT REPORTABLE.

WAS REMOVING IMPELLER FROM A BLOWER MOTOR FINGERGOT CAUGHT BETWEEN HOUSING & IMPELLER & SPLIT MIDDLE FINGER ON LEFT HAND

CUTTING HOSE AND CUT FINGER.

SMALL PIECE OF METAL IN LEFT EYE. THE WIND WAS BLOWING DUST ALL DAY LONG, HAD DUST IN BOTH EYES ALL DAY LONG. WAS ALSO DOING CUTTING, GRINDING AND WELDING WITH STEEL FOR 4 HOURS. BUT I HAVE NO IDEA WHEN SMALL PIECE OF METAL ENETERED EYE.

WINDY DUSTY CONDITIONS LIMEDUST BLOWN INTO WORKERS EYE. THROUGH SIDE OF GLASSES. WHILE WALKING TO WORK STATION.

PUTTING RAILCAR BACK ON TRACKS. USING RAIL FROGES AND PUTTING THEM IN PLACE. SMASHED FINGER WITH RAIL FROG.

EE WAS TRYING TO PUSH A RAIL BACK INTO PLACE AND THE KICK PLATE CAUGHT HIS FINGER, TAKING OFF A PIECE OF IT. EE WAS WORKING FOR SOS TEMPORARY SERVICES,THEY DID A WORKMAN'S COMP. FORM. WE ASSUM ED THEY WOULD ALSO DO A MSHA REPORT. SOS DID NOTFILE MSHA FORM. I FOUND OUT 11-6-00 THAT MSHA REPORT WAS OUR RESPONSIBILITY. I AM NOW REPORTING CLAIM ON 11-7-00. THANKS

CUTTING UP TREES & PILING THEM UP WHEN PICKINGUP A TREE LIMB, HIS SHOULDER POPPED.

DRIVER OF TRUCK WAS RETURNING TO THE QUARY AT CRICKET MOUNTAIN TO PICK UP ANOTHER LOAD OF LIME ROCK. JUST PRIOR TO REACHING THE QUARRY HE FELL ASLEEP AND RAN OFF THE ROAD. THE TRUCK WAS NOT DA MAGED BUT DRIVER SUSTAINED TWO LACERATIONS ON HIS FACE AND EYE.

WHEN CLEANING RAILCAR, A GATE WAS JAMMED & WHILETRYING TO CLOSE WITH A PRY-BAR PUSHING WITH BODYWEIGHT, IT BROKE LOOSE--CAUSING EMPLOYEE TO FALL1-1/2' AND RE-INJURED BACK.

FEEDER PAN POKE HOLE CAP WAS NOT ON. GOT A LADDER AND CLIMBED UP TO PUT CAP BACK ON CAP WOULD NOT FIT SO WENT TO STEP OFF LADDER AND FELL TO GROUND ABOUT 3 FEET LITTLE FINGER ON LEFT HAND CAUG HT HAND RAIL ON THE WAY DOWN AND BROKE.

WAS CLEANING UNDER CONVEYER BELT, BENT OVER AND REACHED TOO FAR WHEN TRIED TO LIFT SHOVEL. IT WAS TOO HEAVY AND STARTED TO FEEL PAIN AND CONTINUED TO WORK THE REST OF THE DAY. AFTER A FEW DAYS WAS UNABLE TO GET RID OF STIFFNESS IN BACK.

SLOPING GROUND COVERED WITH SNOW AND LOOSE ROCKS CAUSED EE TO TO SLIP AND FALL STRIKING HIS ELBOW ON THE GROUND. EE HAD SORENESS IN HIS ELBOW UNTIL MARCH 24, 2000 WHEN HE VISITED A DR. INCIDEN T WAS NOT REPORTABLE ON 2/2, NOW IS REPORTABLE.
