An employee was hooking up a drum basket to a forklift, EE bent down to lift and slide the right-side fork to align with the basket, when EE lifted the fork, EE felt like an impact to EE's lower back. EE was taken to the Tooele Utah hospital where EE was diagnosed with lower back muscle sprain. EE received 2 injections for pain and inflammation.

Employee was pulling out a pump, when pulling EE caught EE's right-hand pinky finger between the pump and the pipe flange.

Employee was loading shots for the quarry at the south ridge area, while walking on a down slope slipped on loose dirt and rocks and fell on EE's right side, felt a pop on EE's right shoulder.

Employee was unplugging core product from the core chute, when product dislodged lime went into EE's left eye.

Employee was flagging a shot at around 2:00pm, suddenly the ground gave way and the employee fell in a hole, the right leg outside the hole, the left leg inside the hole, the right knee was at about EE's chest, then EE felt a pop on EE's right knee, EE then called a co-worker to help EE get up.

EE exited EE's truck on the lower scale and started climbing the steps to the scale house. EE tripped on the top step and hit the bridge of EE's nose on a structural beam. EE exclaimed that EE was in a hurry and that is what led to EE tripping and falling. EE's nose was broken as well as a minor spinal cord injury from the impact of the fall.

Emp was trying to open a dump chute using the slide hammer. As EE was sliding the hammer, the handle broke and EE's thumb slipped between 2 pieces of steel, pinching it. EE was taken for treatment and received sutures. EE was released full duty and returned to work.

Employee was lifting a coal mill liner, it slipped and as they tried to catch it, felt a pain in R shoulder. Went to clinic and was given restrictions on lifting and use of right arm.

On 6-26-2020, emp was backing up a loader and struck a parked haul truck. Was jarred and had soreness in neck. EE was taken to the ER to be evaluated and was told to rest for the next 2 days, missing both shifts. EE returned to work on 6-29-2020 at full duty.

Employee was working to open a stuck door on a hydrator. When door came open it broke out of the hinges and fell to the floor, striking employee in the left leg/ankle. Employee was taken for treatment and later diagnosed with a contusion and was given restricted duty.

Emp was swinging a sledgehammer and felt a pain in their back. Emp was taken in for treatment and was given restrictions. Emp worked 1 day with restrictions.

Employee was working inside kiln on brick rig platform. EE's foot slipped and fell off of the platform. Employee landed on EE's right foot, fracturing the ankle.

Emp was reinstalling a guard. Emp caught R index finger between the guard and the frame, causing a laceration. Emp was taken in for treatment and received 4 sutures

Employee was helping with cleanup when a small bit of dirt flew up and went under the safety glasses, hitting the eye. Employee sustained a small abrasion and was given prescription eye drops and returned to work.

Emp was helping to remove a drill head. The drill head swung free and contacted the frame, pinching EE's left ring finger. EE received sutures and returned to work.

Emp was being fitted for custom earplugs. While removing plugs, vendor caused pain and injury to both ears. Emp was seen by Dr and returned to work. EE was later told to have surgery due to continued pain. This became a Reportable and Lost Time on 10-17-2017.

Employee was working on a ladder removing a pipe. The pipe came free and swung into the ladder causing EE to fall and sprain ankle.

WORKING ON CABLES FOR A TRANSFORMER, UNDER A CRAWL SPACE. EE EXPERIENCED BACK PAIN AND MUSCLE SPASM.

Emp. was changing a belt and caught a finger between the sheave and belt. Emp was treated onsite and returned to work that shift. On 5-1-17 Emp complained of pain and was taken to clinic where a small fracture of the tip of the finger was found. Incident became reportable at that time.

Driver closed lid on top of trailer, lining up cam-lock on lid. Didn't realize finger was in latch. Closed cam-lock pinching left index finger, splitting end and taking off back portion of fingernail.

Our employee had his right hand injured in a refractory mixer while in the process of cleaning it. He had injuries to his wrist and fingers. The mixer was unexpectedly energized while our employee had his hand inside the mixer.

employee was cutting skirt board with a razor knife. The knife slipped and caused a small laceration on his upper left thigh, requiring 3 sutures.

employee was exiting the kiln and lost his footing on a ladder. he slipped from the ladder and landed about 4-5' below on the grating. employee was taken to a clinic as a precaution and found to have a minor fracture in his back.

Driver stepped into hydrate, which got into the left boot and caused a minor burn.

While removing bolts with an impact wrench the socket came loose and contacted the employee's mouth area, damaging his tooth.

The miner was dumping lime dust when the latch on the rear gate of the dump truck wouldn't shut. The miner exited the vehicle to unstick the latch traveling thru the dust. The lime dust entered the miner's boots and was not removed before causing burns to the miner's feet. The burns were caused by the reaction of the dust and perspiration.

Employee was trying to remove build up of material from a product gate, when pry bar struck the employee below the eye.

Contractor was removing refractory when brick slipped and landed on right thumb. Causing a fracture.

Employee was removing material from the crusher, when a large rock broke free and crushed the right ring finger.

going to #2 cooler pit to obtain sample lime got in eyes causing irration

Employee was sitting in break room waiting for shift to begin. Employee started experiencing chest pains and not feeling well. Other employees coming into break room also noticed that he did not look well. Office was contacted blood pressure taken and first responders came up. Paramedics were called and employee was transported for further observation.

Employee is unaware of any incident that may have caused this just reported he started feeling pain in his back at 10:00 am and continually worsened.

EE was removing fittings from old section of waster pipe when pocket knife he was using slipped and came down across back of left hand and first knuckle of thumb.

Employee shoveling coal when he turned & felt sharp pain in lower back.

Working with bags from bag house inhaled lime dust.

Employee was rodding #3 Kiln preheater & felt pain in lower back & leg.

Employee was rodding plugged coal hopper with rod plug gave way and rod was pushed to metal side cutting finger.

Employee putting new cable on chute and forgot to remove vice grip from motor shaft they were using to line hole for new cable. They were tightening cable against limit switch and vice grip came around and struck employee in the face.

Rodding out chute with metal rod, chunk of material fell and hit rod causing cut on chin.

Employee fractured finger while checking the tension of a belt on a screen- pan feeder pulley.

Employee was cut on the face by a piece of metal.

Moving shaft in warehouse, it rolled and smashed finger.

Employee slipped while descending a ladder.

EE works for tri-phase electric. he was installing fiber optic cable on the cable tray at pilot peak. in the area he was working in, there was lime ont he floor that he had to work in. while working, lime made it way up his left paint leg, down into his sock and boot, burning his ankle.

EMPLOYEE WAS UNDER SCALE TRYING TO FREE ICE & CAME IN CONTACT WITH WATER & LIME CAUSING BURNS TO HIS LEGS.

Ee was changing a bearing at the kiln. The wind was blowing quite hard & something got in his eye.

EE WAS SITTING HAUL TRUCK USING LEATHERMAN. WENT TO CLOSE BLADE & SLIPPED SLICING RIGHT THUMB.

EMPLOYEE WAS LOADING RAILCARS & HURT LOWER BACK WHEN PRYBAR SLIPPED FROM HOPPER DOOR.

WHILE REMOVING THE CONVEYOR GUARD, IT SLIPPED & SMASHED HIS LEFT INDEX FINGER AGAINST THE WALL.

STRIPPING LUGS- UNWIRING A MOTOR.

LOADING PARTS ON PALLET ONE ROLLED OFF AND STRUCK LEG.

MOVING A 2X10 BOARD AND GOT A 2 1/2" SLIVER IN LEFT LEG.

SLIPPED ON ICY SLOPE AND FELL FRACTURING HIS HIP.

ON A JAMMED ELEVATOR BUCKET WHILE HAMMERING A LARGE SPROCKET WITH A SLEDGE HAMMER A PIECE OF METAL CAME OFF. SMALL PUNCTURE WOUND TO LEFT LEG.

PUTTING BELTS ON MICROPULVERIZER, HE USED PIPE WRENCH TO TURN PULVERIZER. PIPE WRENCH CAUGHT LEFT INDEX FINGER BETWEEN WRENCH AND PLATE CAUSING LACERATION.

PRYING WITH BAR HEARD POP IN SHOULDER. DIDN'T FEEL THIS WAS ANYTHING. COMPLETED SHIFT. AWOKE NEXT MORNING ON DAY OFF. SHOULDER WAS RED AND SORE. NOTIFIED SUPERVISOR.

WHILE CLEANING #1 PREHEATER LOST BALANCE AND FELL THREE FEET AND LANDED ON WALKWAY BELOW ON BACKAND NECK. WAS STANDING ON PUSH RAM.

WHILE BREAKING LOSE A BOLT, THE WRENCH SLIPPED, CAUSING HADN TO SWING INTO A BROKEN WELD. RESULT WAS EIGHT STITCHES.

WORKING ON #2 DOUBLE BELLS TRYING TO PUT RAM BACK IN PLACE WITH HAMMER, MISS RAM AND HIT RING FINGER.

EE SLIPPED WHLE STEPPING OFF LADDER.LANDED WRONGON FEET CAUSING INJURY TO THE LEFT FOOT MAINLY

WHEN EE WAS CLIMBING DOWN THE LADDER, IT SLID OVER TO THE SIDE ABOUT AN INCH. THE EE THOUGHT THE LADDER WAS GOING TO FALL ALL THE WAY OVER SO HE JUMPED DOWN TO THE COOLER GRATES THREE FEET BEL OW AND LANDED WRONG ON HIS ANKLE.

REMOVING BRICK RING.

EE WAS LOADING LIME TRUCKS AT THE #2 SCALE (T-342). HE STEPPED DOWN OFF A LADDER AND TWISTED HIS LEFT KNEE. (HE SPRAINED HIS LEFT KNEE).

EMP WAS CLEANING OUT THE INSIDE OF S2 SCREEN WITH AN AIR LANCE. MATERIAL MOVED AND PINNED HIS LEFT FOOT AGAINST AIR LANCE.

EMP WAS REPAIRING A FUSE HOLDER IN AN ELECTRICALDISCONNECT. EMP TURNED OFF POWER AND WAS REPLACING A FUSE HOLDER. HAD TO DRILL A HOLE IN THE MOUNTING PANEL. DRILL BIT WENT THROUGH AND HIT A HO T WIRE BEHIND THE PANEL CAUSING A SHORT BETWEEN THE BIT AND THE BACKING PANEL, CAUSING A FLASH AND FLAME, BURNING EMPS HAND AND FLASH BURN TO EYES.

USING AIR LANCE TO REMOVE ACCUMULATED DIRT OFF SIDE OF S-1. TWISTED HEAD/SHOULDERS WRONG AND HEARD A SNAP AND FELT PAIN.

LIME BLEW IN BOTH EYES WHILE WELDING ON KILN #1. WAS WEARING SAFETY GLASSES AND WELDING HOOD. EE BELIEVES THERE WERE FANS ON TO COOL DOWN THE KILN.

WHILE SHOVELING, EXPERIENCED LOWER BACK PAIN DIAGNOSIS PER M.D. ACUTE LUMBAR STRAIN.

EE WAS DIGGING OUT A SCREW CONVEYOR WHICH CAUSED A THICK DUST CLOUD. THE EE INHALED THE LIME DUST WHILE BREATHING WHICH CAUSED DAMAGE TO THE LUNGS.

EMPLOYEE SLIPPED ON ICE AND TWISTED ANKLE AND KNEE.

GOT HYDRATE IN BOOT AND REACTION OCCURED BEFORE IT COULD BE CLEANED.

WHILE REMOVING ASH FROM OUR COOLER GRATE, INDIVIDUAL SUFFERED HEAT PROSTRATION DUE TO MINIMUM FLUID INTAKE AND EXPOSURE TO EXTREME TEMPERATURES.

THE PERSON INVOLVED WAS PROPERLY TIED OFF BUT DISCONNECTED THE FORM HE WAS TIED TO.

EE WAS MOVING ACROSS THE FORMS AND LOST HIS GRIP, CAUSING HIM TO FALL TO THE GROUND HIS FEET TAKING THE IMPACT OF THE FALL.

EMPLOYEE WAS ATTEMPTING TO THROW VEHICLE SNOWCHAINS ON THE BED OF HIS TRUCK WHILE PERFORMING THIS TASK HE STRUCK HIS (R) WRIST ON THE VEHICLE SIDE CAUSING HIM TO STRAIN HIS WRIST

WHILE INSPECTING A BAGHOUSE DAMPER SHAFT FOR LEAKS, EMPLOYEE FELL OVER THE EDGE. FELL APROXIMATELY 4.5 FEET LANDING ON HIS FACE.

EE WAS CLEANIG UP SPILLED LIME DUST BY SHOVELING IT INTO A BUCKE,T AND THE EMPTING THE BUCKET ONTO A CONVEYOR BELT. THE LIMME DUST SPLASHED UP OFF THE BELT INTO HSI FACE, BURNIGN HIS LEFT EYE .

DRILLER HELPER WAS CLEANING OUT SAMPLE RETURN HOSE WHEN PLUGGED MATERIAL EJECTED AND AIR PRESSURE CAUSED HOSE TO BUCK, STRIKING HELPER ON THE LEFT SIDE OF UPPER TORSO.

A PART WAS BEING DRIVEN OUT OF A DRILL BODY. A SLIVER OF METAL BROKE OFF OF THE HAND HELD HAMMER HE WAS USING AND CUT THE LEFT FOREARM. THE SLIVER MADE A 2 INCH CUT, 1/2" DEEP AND CUT A VEIN I N THE FOREARM.

ON 01-25-91 I NAME REPORTED TO ME THAT ON 01-24-91 WHILE WORKING AT THE CRUSHER HE STEPPED OFF A LADDER & IN DOING SO HE STEPPED INTO A HOLE & TWISTED HIS KNEE

INJURED WAS WALKING ACROSS THE YARD WHEN HE COLLAPSED FROM AN APPARENT HEART ATTACK.HE WAS NOT PERFORMING ANY WORK AT THE TIME AND HAD STARTED HIS SHIFT JUST 30 MINUTES BEFORE.AN EMT WAS WITH HIM WITHIN MINUTES.THE AMBULANCE WAS ON THE SITE AT APPROX 9:05AM AND HE WAS PRONOUNCED DEAD AT 9:41AM.
