A crusher hand with approximately 3 months of experience broke two fingers while performing routine maintenance on crusher apron. EE was able to return to work the following day
Supervisor of crusher crew suffered a laceration to his left hand while unloading a conveyor belt from the back of a pick up truck. The laceration required 30 stitches to his hand.
A piece of electrical switch gear broke off and arced between B & C phases inside switch gear. This caused an arc flash which resulted in a first degree burn to the operators face and neck on the right side.
A rock was stuck between a conveyor pulley and the frame of the conveyor, as the employee removed the rock, the rock slipped and smashed left fifth finger.
IMPROPER USE OF COME-A-LONG. EMPLOYEE WAS TRYING TO PULL A BAR DOWN ON A GRIZZLEY. THE HOOK ON THE COME-A-LONG WAS NOT PROPERLY SECURED AND THE HOOK SLIPPED OFF THE BAR STRIKING THE EMPLOYEE 'S HAND.
