FOLLOWING A REPAIR TO THE ROLL SCREEN, THE INJURED TURNED THE MACINE BACK ON TO TEST THE OPRATION. THEN EE ATTEMPTED TO REINSTALL THE GUARD WHILE THE MACHINE WAS RUNNING. EE'S GLOVE WAS CAUGHT BY A SPROCKET, PULLING EE'S FINGERS INTO THE SPROCKET AND CHAIN.

Working on the CAT 349 excavator. They opened the expansion tank on the coolant system, and hot liquid and steam sprayed out. Victim was burned (1st & 2nd degree) on their left hand, wrist, and fingers.

Miner was using a portable hydraulic ram to remove a bushing. A sudden movement of the ram caused the victim's arm to be twisted backwards. Miner worked remainder of the shift & rested over the weekend. Upon return to work Monday, the miner was hit in the shoulder/arm with an entrance door at the processing plant. The 2nd incident caused the miner to seek medical attention.

The miner was standing on the #4 shaking table (which was not operating at the time of the accident), and was reaching for a slurry pipe. The miner lost EE's balance and fell to the concrete floor. The miner sustained a serious head injury.

Employee was found lying on ground, still conscious, but complaining of chest pain and numbness in face and hands. Rescue squad was called and they evaluated miner. No signs or symptoms of heart attack were observed, however miner was transported to ER for observation. EE was released from ER a few hours later and told by doctors that the episode was likely stress related.

An over-the-road truck driver was on site to make customer pickup. Driver climbed up the ladder on the trailer to unroll the tarp, which was stuck in the partially unrolled position. Driver pulled on the tarp and it unrolled all at once, causing driver to lose balance and fall off the ladder. Driver fell to the ground and injured right foot/ankle upon impact with the ground.
Employee was walking up the stairs when employee's left leg gave out. Employee was unable to get back to employee's feet without assistance. Pain soon followed.

Returning from load. Hit rut in road or rock and threw him to left side. Felt a twinge in lower back. Started getting worse as the day went on.
Pump had shut down due to over-temp. fault. Injured read the temp gauge on the radiator & assumed it was safe to open the radiator cap. The coolant had not been circulating and therefore the temp. gauge was giving a false low reading. When the injured opened the radiator cap, hot coolant sprayed out and burned his arms, hands, chest, and abdomen, face & neck.

Miner was using bottle jack to align steel plate on conveyor. A pipe was being used as an extension to the jack. As the miner began to raise the jack, the jack shifted and the extension pipe kicked out and the end of the pipe glanced his upper lip and struck the end of his nose.

Men were moving lengths of black poly pipe. They were using a forklift to pick up pipe off of ground to place on lowboy. A few pieces fell off lowboy. They went to retreive the pipe and almost had it back on lowboy when the weight of the pipe shifted and knocked him backwards and into a below ground level (6') containment area. He landed on his back.
Working to repair soap hose. When he grabbed it where there was a hole the soap sprayed out in his face.

Air conditioner unit on cab of truck was dripping water on employee's head. He stuck a rag up into fan area and tip of his middle finger contacted fan blade, cutting the meat of his finger away.

Miner was holding wrench on bolt head. Wrench slipped off bolt head and the end of the wrench flew back and hit miner on left brow.
EE was climbing out of excavator & slipped & lost his footing. His foot slipped off the step below the top of the track. His head hit the track.

They were cutting long piece of steel - cut piece of metal 100" - fell on foot. He was holding with another employee; a third employee was cutting.
Getting down in bed of truck - slipped - fell down & hit back of head.
Pick up pipe/rubber hose-wire at end stricking out-cut on right hand, between thumb index finger.

EE WAS WELDING AND WAS WEARING PROTECTIVE GEAR, BUT APPARENTLY HE GOT FLASH BURNS FROM OTHER WELDERS IN THE AREA.

WALKING DOWN A FLIGHT OF STAIRS, FOOT SLIPPED ON STEP, FELL TO THE BOTTOM. GOT UP AND SAT ON A PALLET TO CATCH A BREATH. NEXT THING HE REMEMBERSIS WAKING UP FACE FIRST ON THE GROUND, BACK WAS SCRATCHED, ELBOW CUT AND TEETH OUT OF MOUTH.
TRYING TO UNPLUG PUMP - APPARENTLY HEAT & PRESSURE HAD BUILT UP - WHEN DISCONNECT FOR UNPLUGGING, IT BLEW.
