CONTINUATION OF ACCIDENT THAT OCCURRED ON 05-06-92 WHEN EMPLOYEE STEPPED OUT OF MOTOR AND EXPERIENCED PAIN IN HIS RIGHT LEG AND HIP.EMPLOYEE SCHEDULED ELECTIVE SURGERY ON 01-04-93 BECAUSE HE A ALLEGEDLY HAD PAIN IN HIS KNEE SINCE ACCIDENT ON05-06-92.
EMPLOYEE WAS USING AN ADJUSTABLE WRENCH TO REMOVE FLOATING AXLE ON SHUTTLE CAR WHEN THE WRENCH ALLEGEDLY SLIPPED CAUSING EMPLOYEE TO HIT THE FRAME OF THE SHUTTLE CAR THUS CAUSING LACERATION OF LOWER LIP WHICH REQUIRED STITCHES
ROOF FALL OCCURRED IN 002 SECTION #8 ENTRY 100' INBY SS #3102. MOUNTAIN BREAKS ARE PRESENT IN SEVERAL ENTRIES, AND SECTION IS ADVANCING TOWARD LOW COVER. FALL WAS DUE TO MOUNTAIN BREAK SLIPP ING. CRIBS HAD BEEN SET, DANGERING OFF AREA BEFORE FALL OCCURRED. FALL WAS APPROX 18'X80'X4'. NO PERSONAL INJURY OR EQUIPMENT DAMAGE. VENTILATION WAS NOT AFFECTED AND FALL WILL NOT BE CLEA
EMPLOYEE WAS PREPARING SUPPLIES TO TAKE UNDERGROUND BEFORE HIS SHIFT BEGAN. A DIFFERENT TYPE OF ROOF DRILL BIT WAS REQUIRED AND THE PLASTIC PAILS PROVIDED FOR THE BITS WERE FULL, THUS EMPLOYEE DROPPED THE EXTRA BITS IN A LUNCH BOX. THE BITS STRUCK A GLASS SODA BOTTLE WHICH EXPLODED, AND THE FLYING GLASS STRUCK THE EMPLOYEE ON THE BRIDGE OF HIS NOSE REQUIRING STITCHES.

EMPLOYEE WAS WALKING TO END OF TRACK (OVERALL COAL HEIGHT IS 42") WHEN HE ALLEGEDLY RAISED HIS HEAD UP AND CAME IN CONTACT WITH A PIECE OF TIE WIRE HANGING FROM THE TOP SCRATCHING HIM ON THE L EFT EYE. EMPLOYEE DID NOT GO TO THE DOCTOR UNTIL 12-14-91.

ROOF FALL OCCURRED NO PERSONAL INJURY OR PROPERTY DAMAGE SUSTAINED FALL WAS APPROX 70FT LONG X 18FT WIDE X 4FT HIGH FALL AREA HAD BEEN SUPPORTED WITH 42IN BOLTS TEST HOLES DID NOT INDICATE ANY PROBLEMS WITH ROOF STRATA ABOVE BOLT ANCHORAGE
SLIPPED AND FELL GOING I BATHHOUSE FELL AGAINST BEND STRUCK LOWER BACK
THIS INCIDENT WAS THOUGHT TO HAVE OCCURRED WHEN THE EMPLOYEE WAS EXITING THE MANTRIP.HE HAD PLACED HIS LEFT FOOT OUT OF MANTRIP ONTO THE BOTTOM AND WAS IN THE PROCESS OF TAKING HIS RIGHT FOOT OUT WHEN HE FELT A POPPING IN HIS LEFT HIP.EMPLOYEE STATED HE HAD NOT INJURED HIS HIP BY ANY PHYSICAL EXERTION.
EE WAS LOADING 42 FT ROOF BOLTS AND 6X6 ROOF BOLT PLATES ONTO ROOF DRILL WHEN HE EXPERIENCED PAIN IN HIS LOWER BACK. THIS MAN COMPLETED THE SHIFT AND RETURNED TO WORK ON 1/7/91 AT WHICH TIME H E STATED HE COULD NOT WORK DUE TO HIS BACK.

MAN ALLEGED HE WAS OPERATING SHUTTLE CAR OVER ROUGH HAULWAY & EXPERIENCED PAIN IN GROIN & LOWER BACK. INJURED WENT TO DR ON 10/28.
THIS REPAIRMAN SAID HE PLACED HIMSELF ALONGSIDE SHUTTTLE CAR ON BOOM AND OPPOSITE THE OPERATOR DECK TO CHECK A BEARING AND OIL LEVEL, WHEN THE SHUTTLE CAR OPERATOR RETURNED TO HIS PIECE OF EQ UIPMENT, HE WAS IN THE PROCESS OF MOVING THE CAR, WHEN THE REPAIRMAN WAS ALLEGEDLY CAUGHT AGAINST THE BOOM SUPPORT FRAME, CAUSING SPRAIN TO HIS NECK.
THIS MAN SAID HE WAS OPERATING A SHUTTLE CAR WHEN HE ALLEGEDLY INJURED HIS LOWER BACK.
