Miner was using a portable band saw while fabricating guards and as the saw reached the end of the desired cut the saw kicked back and the blade contacted the fleshy thumb area of his left palm.

Miner was operating a 4.5 inch grinder. When the wheel touched some rubber belt material it jumped and the miner lost his grip and the wheel contacted his left little finger causing a laceration.

While performing routine cleanup he noticed that the twin scrubber to the wash plant was plugged with mud. While attempting to climb up onto the edge of the scrubber his foot caught on a lip or bar and caused him to lose his balance falling into the scrubber. While trying to brace his fall with his left hand it struck the scrubber screw.
