Victim,entered the property passing the No Trepasssing Signs with four other individuals. Our understanding is they began to swim in the pond and he drowned before reaching the shoreline. The incident occurred after the end of our shift which ended at 4:30 pm and no miners where on site at the time. Victim was NOT a miner or an EMPLOYEE of the mine.

3 employees were installing a roller on a conveyor. One employee lost his grip and the injured employee tried to maintain his grip, resulting in pinching of finger between the roller and conveyor frame.

Lost balance while crouched down - Kneeled on wrench. The incident was a first-aid only when it happened. It did not become reportable until 5-31-05 when employee went to the doctor to have knee drained.
Employee was clearing sand off the side boards of his dump truck with back of shovel. The wind blew and sand flew into employee's right eye.

Incident occurred on 6/5/04 and treated as First Aid only until doctor visit on 6/21/04. Left brake caliper was partially disassembled and on mounting pedestal. He bent down to pick up a fallen Allen wrench and caliper fall off and hit top of knee.
