Part of crew was working on cone crusher & had an opening in the catwalk to access the underside. Loader operator not involved in the repair wanted to see work & came up to the cone & was not looking closely at his steps, and stepped/fell partially into the opening.

Loader operator did not see the pedestrian in his path. Pedestrian on phone was not aware that loader was approaching from behind. Loader ran over pedestrian.
