Working on plant, wrench slipped and finger struck bolt.

Cleaning flok room floor with 4lbs hammer and struck left hand.

Foreign object entered eye while pressure washing. Flushed eye out but still irritated the following day.

Cleaning up screen rack, lifted 1" screen and felt a pull in his left shoulder.
Putting wear iron in feed shaker and hit left elbow on top of shaker while climbing out.
Employee was using a bearing puller. As he applied pressure it slipped off striking him in the right cheek.

Miner was tightening a loose bolt. The ratchet slipped off the bolt causing the miner's right finger to be smashed.

Part of crew was working on cone crusher & had an opening in the catwalk to access the underside. Loader operator not involved in the repair wanted to see work & came up to the cone & was not looking closely at his steps, and stepped/fell partially into the opening.

Operating heavy equipment, very fast, busy day. Ride control was not working, repetitive movement caused pain in lower back.
Loader operator did not see the pedestrian in his path. Pedestrian on phone was not aware that loader was approaching from behind. Loader ran over pedestrian.
Employee climbed up on the sand feeder to adjust it and when he jumped down he jarred his left knee
