AS HE PICKED UP THE POST AND TWISTED TO TURN AROUND WI TH THE PIST HE FELT A CATCH IN THE MIDDLE OF HIS LOWER BACK. HE COMPLETED HIS SHIFT AND WORKED THE NEXT SHIFT BEFORE MISSING WORK.
THE INJURED HAD TRAVELED TO #3 BELTHEAD TO PICK UP A SHUTTLE CAR TIRE. THE TIRE WAS LIFTED UP TO ROLL TO CAR FOR TRANSPINTATION OUTSIDE. INJURED STRAINED HIS LOWER BACK WHILE UPRIGHTING THE TI RE. INJURED COMPLETED HIS SHIFT & CONTINUED WORKING.

A ROOF FALL OCCURRED AT THE #29 BREAK ON THE SOUTH EAST MAINS IN THE TRACK ENTRY. THE FALL PARTIALLY COVERED A ROCKDUSTER. THE FALL WAS 45\ LONG, 37\ ACROSS THE INTERSECTION, AND 42" TO 48" (I NCHES) HIGH.
ROOF FALL OCCURRED ON SW MAINS.IN X-CUT AND WAS 15\HIGH X 66\WIDE X 40\LONG.AREA BOLTED WITH 4-5\ BOLTS AND WAS CRIBBED.RESIN BOLTS WERE BEING USED.NOT GOING TO BE CLEANED UP AT THIS TIME.

A PRIVATELY OWNED JEEP DRIVEN BY THE INJURED, WAS TRAVELING TOWARD THE COUNTY ROAD INTERSECTION AT THE NO 1 GUARDHOUSE AND WAS STRUCK BY A RHODEWAY TRUCKING CO. MACK TRUCK. THE INJURED STATED THAT HE DIDNT SEE THE MACK TRUCK AND WAS TRAVELING AT A NORMAL RATE OF SPEED. A STOP SIGN IS POSITIONED ON THE MINE SITE SIDE OF THE INTERSECTION
THE INJURED WAS CUTTING A CRIB BLOCK WITH A HYDRAULIC SAW AND THE SAW JUMPED OUT OF BLOCK AND FELL BACK ON THUMB CUTTING IT OFF AT THE JOINT.
A ROOF FALL OCCURRED AT THE FACE OF THE #6 ENTRY ON THE SOUTH WEST MAINS, 56 FT. INBY SPAD #2793. THE FALL WAS 12 FT. LONG, 16 FT. WIDE AND 4 FT. HIGH.
ON A-3 PANEL (PILLAR SECTION), #5 BLOCK, BREAK #34, #4 ENTRY A ROOF FALL OCCURRED AFTER THE PUSHOUT HAD BEEN TAKEN. PART OF THE MINER WAS COVERED UP. THE CONTINUOUS MINER WAS REMOVED IN 4 HOUR S. NO INJURIES.
THE INJURED WAS TRAMMING THE MINER TO THE WORKING FACE WHEN A ROCK ROLLED OFF THE RIB CAUSING CONTUSIONS ON THE ANKLE AND FOOT.

1979 CHEV PICKUP OPERATED BY THE INJURED WAS STRUCK BY 2 CHESSIE SYSTEM LOCOMOTIVES OPERATING IN TANDEM LOCOMOTIVES DID NOT HAVE ANY CARS ATTACHED AT THE TIME #3538 LOCO WAS THE ONE THAT IM IMPACTED WITH THE TRUCK TRUCK WAS PUSHED SIDEWAYS DOWN THE TRACK 100 FT AT THIS POINT TRUCK AND LOCOMOTIVE DISENGAGED AND TRUCK ROLLED OVER RESTING ON ITS SIDE
ROOF FELL AT APPROX 5 30 AM AT #8 BREAK INBY THE PORTAL FALL WAS 18 X 18 X10 CONVEYOR BELT AND STRUCTURE WAS DAMAGED THE PLACE HAD BEEN BOLTED WITH 4FT RESIN BOLTS
THE INJURED WAS ATTEMPTING TO LIFT A LARGE ROCK FROM THE CONVEYOR OF A CONTINUOUS MINER WHICH APPARENTLY CAUSED A HERNIA
