THIS FALL WAS IN THE RETURN IT WAS CLEANED UP AND CRIBBED THIS AREA HAS LITTLE COVER FALL OCCURRED IN A 4 WAY 20 BY 25 BY 12 FALL OCCURRED IN A AREA BOLTED WITH 5 FT RESIN.
THE MEN WERE LOADING BELT SUPPLIES ONTO THE SCOOP.THE OPERATOR MOVED THE SCOOP AND THE INJURED WAS STANDING OPPOSITE THE OPERATORS SIDE AND THE SCOOP RAN OVER THE LEFT FOOT.
THE SECTION IS ADVANCING BY THE HEAD OF LITTLE U ISASC CREEK COVERING IS MINIMAL OVER THE #6 AND 5 ENTRYS WATER WAS PRESENT IN THE ROOF AT THERE LOCATION #6 ENTRY HAS BEEN DISCONTINUED AND #5 ENTRY HAS BEEN NARROWED TO 15' WIDE AND THE INTERSECTION OF #5 IS BEING POSTED DOWN AS SOON AS IT IS CREATED.
