EMPLOYEE STATED THAT HE WAS CRAWLING AROUND SHUTTLE CAR, HIS KNEE WAS ON THE EDGE OF A HALF HEADER WHEN IT ROLLED OFF, TWISTING HIS KNEE.
ROOF FALL 35' X 20' X 3'. LOCATED APPROX 500 FT OUTBY FACES. 3RD RT MMU 005-0, NO. 4 ENTRY INTAKE 1 BREAD INBY SPAD 1717. DID NOT AFFECT VENTILATION OF PASSAGE OF MEN. AREA DEVELOPED APPROX 5- 1-94. 36" CONVENTIONAL ROOF BOLTS HAD BEEN USED FOR SUPPORT. FELL AT ANCHORAGE POINT. FALL WILL NOT BE CLEANED UP. ENTRANCES TO FALL HAVE BEEN TIMBERED.
EMPLOYEE STATED THAT HE WAS LOADING BELT STRUCTURE. HE LIFTED STAND AND FELT PAIN IN HIS RIGHT LOWER BACK.
EMPLOYEE STATED THAT HE WAS CRAWLING AROUND BOLTER TO TRAM MACHINE AND TWISTED HIS KNEE ON A ROCK.
EE STATED THAT HE PICKED UP A CASE OF BATTERY WATER AND TWISTED HIS BACK WHEN HE SET WATER ON BATTERY.
LUNG CONDITION THAT MAY HAV ERESULTED FROM EXPOSURE TO DUST.
EMPLOYEE STATED THAT HE WAS OPERATING THE MINER AND A ROCK WAS HUNG IN THE CONVEYOR CHAIN. HE WAS TURNING A RIGHT CROSSCUT. HE REVERSED THE CONVEYOR CHAIN AND THE CHAIN CAME UP OUT OF THE BOOM AND OVER INTO THE DECK WHERE HE WAS SITTING, HITTING HIM ON THE BACK.
EMPLOYEE STATED THAT HE WAS OPERATING THE CONTINUOUS MINER, AS HE WAS BACKING OUT, HE STOPPED AND GOT OFF THE MACHINE TO MOVE THE CABLE. HE PICKED UP THE ROPE HOOKED TO HIS CABLE AND PULLED I T OVER TO THE MACHINE. HE SLIPPED AS HE WAS PULLING AND FELL AGAINST THE HOOK INBY THE DECK AND BRUISED HIS RIBS. *FIRST DAY OFF - 1/10/94.

EMPLOYEE STATED THAT HE WAS UNLAODING BOLTS OUT OF SCOOPY BUCKET WHE HE PULLED A MUSCLE IN HIS SHOULDER.
TWO ELECTRICAN WERE TIGHTENING BOLTS IN A WHEEL UNIT ON A SHUTTLE CAR EE INJURED EE WAS LYING ON HIS LEFT SIDE, REACHING IN AND HOLDING RATCHET ON BOLTS OTHER HAD A CHEATER PIPE ON RATCHET TOR QUENING BOLTS. CHEATER PIPE SLIPPED OFF AND RATCHET HIT EE IN RIGHT RIB CAGE.
EMPLOYEE STATED THAT HE WAS OPERATING SHUTTLE CAR IN LOW ENTRY, ROUGH ROADWAY WITH WATER. SHUTTLE CAR CAME OUT OF HOLE ONTO HIGH SPOT AND EMPLOYEE CAUGHT HIS RIGHT SHOULDER IN TOP.
EMPLOYEE STATED THAT HE WAS OPERATING MANTRIP WHEN HE HIT A HOLE, CAUSING HIM TO HIT HIS HEAD ON TOP. STIFFNESS AND PAIN IN NECK.
EMPLOYEE (ROOF BOLTER) BACKED INTO INTERSECTION TO STRAIGHTEN HIS CABLE. SHUTTLE CAR HAULING OUT OF NO.4 ENTRY CAME THROUGH FLY AND HIT THE BOLTER IN THE SEAT AREA WHERE EMPLOYEE WAS TRAMMING MACHINE. APRON FROM CABLE HIT EMPLOYEE (BOLTER) IN UPPER RIB AREA UNDER LEFT ARM.
EMPLOYEE STATED THAT HE WAS USING SLEDGE HAMMER TO PUT HEAD SHAFT IN FEEDER MISSED HITTING SPROCKET WITH HAMMER PULLED MUSCLE IN LEFT ARM FIRST DAY OFF 08-30-93
EMPLOYEE STATED THAT HE WAS OPERATING SHUTTLE CAR WHEN HE WAS BOUNCED INTO THE TOP SPRAINING HIS NECK. EMPLOYEE WAS KNOCKED UNCONSICIOUS.
ROOF FELL ON CONTINUOUS MINER (REMOTE CONTROL) WHILE MING PILLAR PUSH-OUT. NO INJURIES. SIZE: 15' X 12' X 30". 36" CONVENTIONAL ROOF BOLTS AND POSTS WERE USED AS SUPPORTS. DID NOT AFFECT VENT ILATION OR PASSAGE OF MEN. DEVELOPED APPROX. 4-15-93. LOCATION JAMES MAINS 002-0, NO. 7 ENTRY- INTAKE- 140' INBY SURVEY STATION NO. 275. CLEAN UP JUST ENOUGH TO FREE CONT. MINER. PLAN POSTED.
EMPLOYEE STATED THAT HE WAS GOING TO SECTION IN MANTRIP WHEN A LOW HANGING CHECK FLY BOARD CAUGHT THE TOP OF HIS HAND- SPRAINING HIS NECK.
EE STATED THAT HE WAS OPERATING SHUTTLE CAR COMING OFF DUMP HE LOOKED BACK TO SEE IF BOOM WAS DOWN AND HIT HIS HEAD ON FLY BOARD SPRAINING HIS NECK.
EMPLOYEE STATED THAT HE WAS REACHING ACROSS BOLTER TO GET A BOX OF RESIN. HE SLIPPED AND HIS KNEE ACCIDENTALLY HIT THE ATRS LEVER, DROPPING THE ATRS ON THE EMPLOYEE- STRAINING HIS NECK.
EMPL STATED THAT HE WAS GOING ACROSS THE FRONT OF FEEDER, FEEDER WAS DOWN TO RETURN TO HIS SHUTTLE CAR. ANOTHER SHUTTLE CAR OPERATOR STARTED UP HIS CAR & PULLED UP TO THE FEEDER HITTING THE EMPLS RIGHT LEG.
