An unintentional roof fall has occurred 8 breaks outby spad 4225 Horse Creek 4 Belt break 107. The roof fall was approximately 18ft x 18ft x 6ft. There were no injuries reported and no equipment involved.

While Employee was putting water line together his hand caught between the water line and coupling pinching his hand. Employee was treated at Raleigh Boone Medical Center where he received eight (8) sutures.

Officer was making security inspections at Marsfork Mine when he was bitten by a copper head snake on lower right leg. He was transported to Charleston General Hospital where he was treated and released.

Individual was unloading first aid boxes from a van and his finger was smashed between two boxes. Individual was treated at Raleigh General Hospital and was diagnosed with a fracture to the tip of the left middle finger and also received thirteen (13) sutures. Individual is not an employee at this location (works at Maxxim Shared Services).

Employee was loading top rollers off belt rails onto a flat car when roller hit him on the side of his left knee

Installing head shaft in section feeder, jacking frame to get head shaft in place. Jack bar slipped out of jack smashing right thumb between bar and mine roof.

Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department was notified February 2015 of the diagnosis. Employee retired on June 1, 2013.

Employee has been diagnosed with Occupational Hearing Loss. Marfork Safety Department was notified March 2015 of the diagnosis. Employee retired on June 1, 2013.

As Employee was hanging roof bolter cable a piece of draw rock fell striking Employee on his back and left shoulder. Employee was treated at Raleigh General Hospital.

As Employee knelt down on his knees at the section phone he felt a sharp pain in his lower back. Conducted an internal investigation and determined this is a reportable incident.

Employee has been diagnosed with Occupational Pneumoconiosis.

While moving a pallet out of the way, Employee felt a pain in his back. Employee was treated at MedExpress.

An unintentional Roof Fall has occurred on the #1 Section #2 Face spad 10358. The fall was approximately 30ft x 18ft x 24" to 28". There were no injuries reported and no equipment involved.

While gathering belt tools to make a splice on the #1 belt at the old Marsh Fork portal, employee was walking bent over because it is low when he caught his ear on the bottom of a cable bolt resulting in a cut to his left ear. Employee was treated at Med Express in Beckley where he received thirteen (13) sutures.

An unintentional roof fall has occurred on the Horse Creek #5 Belt and Track outby spad 4288. The roof fall was approximately 40ft x 30ft x 6 to 8ft. There was no injuries reported and no equipment involved.

Employee has been diagnosed with Occupational Pneumoconiosis. Marfork Safety Department received the diagnosis on May 16, 2014.

An unintentional roof fall has occurred on the Left Return, 4th Left Panel #7 Entry Moles Sub Mains 1 break inby spad 9904. The fall was approximately 20ft x 26ft x 5 1/2ft. There were no injuries reported or no equipment was involved.

An unintentional roof fall has occurred in Neutral Entry on #3 Belt spad 3139 - 3159 inby. The roof fall was approximately 40' x 6ft. There were no injuries reported and no equipment involved.

An unintentional roof fall has occurred on the #1 Section #5 Entry spad 10013. The fall was approximately 30ft x 18ft x 5ft. No injuries were reported and no equipment was involved.

Contractor slipped and fell on a piece of belt causing the right knee to fold behind him. Contractor was treated at CAMC General Division where he was diagnosed with cartilage damage.

An unintentional roof fall has occurred at the Horse Creek #5 Belt #2 Entry one break outby spad # 4284. The roof fall was approximately 35' x 30' x 52". No passage of men were affected and there were no injuries.

Employee was getting out of roof bolter when his left shoulder popped and felt like it dislocated. Employee was treated at Raleigh General Hospital where he received x-rays which were negative.

Employee was diagnosed for occupational pneumoconiosis. This office received this notification on 01/22/2013 of diagnosis.

Employee was installing track. Employee picked up the rail with a slate bar to install a track tie when the rail rolled smashing his finger between the rail and tie. Employee was treated at Raleigh General Hospital where he received six (6) sutures to his right finger.

Employee has been diagnosed with Occupational Pneumoconiosis - During a Part 50 Audit it was determined a 7000-1 was not submitted for this Employee

Employee slipped but did not fall while walking to the power center. Employee slipped on corner inby power center but used hands to place against roof to keep from falling. Did not feel any pain until next day 4/21/12.

Roof fall in the #1 entry at spad 4034. the fall measured 35' in length 19' in width and 5' high. The fall was breakered off tall approaches never affected ventilation or passage

As Employee was walking from 27 to 28 seal he placed his left foot on edge of a hole and twisted his foot causing his ankle to roll. He was transported by EMS to CAMC General Division where he was diagnosed with multiple fractures of the ankle and had surgery to repair the damage.

As Employee was lapping belt into scoop bucket his foot was under the bucket resting on a rock. The scoop operator trammed the scoop forward and caught Employees foot between the bottom of the bucket and the rock. Employee refused ambulance and went to Raleigh General Hospital for treatment.

Employee was diagnosed with Occupational Pneumoconiosis - During a Part 50 Audit it was determined a 7000-1 was not submitted for this Employee

EE was replacing filter housing on continuous miner. As ee disassembled the new housing using a ratchet the filter cap was stuck. Employee was pressing down on ratchet the filter housing broke loose smashing his finger between the ratchet and frame of the machine. Employee was treated at MedExpress and diagnosed with a fractured left middle finger in three (3) places.

An unplanned roof fall has occurred at 1A Head intake between 5 and 6. It was located one break outby spad 8929 and 8928. The timbers and jacks were set and the roof fall was dangered off. The water line and power was rerouted. No injuries were reported and no equipment was involved.

Employee strained his back while putting shield back on reel of the roof bolter. Employee was treated at Raleigh General Hospital.

While Employee was lifting a roll of lifeline he slipped in mud and strained his lower back. He was treated at Raleigh General Hospital.

Employee was using a slate bar to lift shield up on shuttle car to reset breakers. As he was lifting he felt a pain in his lower back. Employee was treated at Princeton Community Hospital. Xrays were negative.

Employee was running the motor to section when the canopy roofed out. When the canopy came down it struck Employee on the right elbow. Employee was treated at Raleigh General Hospital. Xrays were negative.

Employee was cutting belt with a utility knife. As Employee was cutting the knife slipped causing a laceration on the right leg above the knee. Employee was transported to Raleigh General Hospital where he received 10 sutures.

An unplanned roof fall has occurred at Horse Creek #4 spad 4022. The roof fall was approximately 40' x 30' x 5'-7'. This area is a weekly travel way. No injuries were report and no equipment was involved.

Employee was in #3 entry hanging miner cable. As he was hanging the cable he felt pain in his back. Employee was transported to Raleigh General Hospital and was diagnosed with a back strain.

Employee was helping make a belt splice on the section belt. While cutting the belt the utility knife slipped striking employee on his left forearm. Employee immediately ceased work. He was treated at Raleigh Boone Medical Center where he received three (3) sutures.

Employee was getting out of a personnel carrier when his arm slipped, causing him to fall to the ground approximately 30 inches straining his left arm and right leg. Employee drove himself to Appalachian Regional Hospital for treatment.

Member was setting bits on the left continuous miner and smashed his ring finger with a hammer which cut his finger and cause his fingernail to fall off. Member immediately ceased work and was transported by paramedics to the hospital. At Raleigh General Hospital he was told to remain off work in order to avoid the risk of infection.

Member was in the deck operating the bolter. He went to reposition himself and stuck his hand on top of the canopy to pull himself up and hit the tram. The bolter moved, smashing his right hand into the top. Member immediately ceased to work and was taken by ambulance to CAMC General where he was told his right hand/middle finger had to be amputated.

An unplanned roof fall has occurred on #1 Section #4R to #6 face at spad 8821 one break outby fall. The roof fall was 80' x 19' x 4'. No injuries or equipment was involved.

Member was lifting come alongs above head to put into monkey face in order to bring a wrecked flat car over and onto track rail. While lifting he experience a "pop" sensation in back, he immediately ceased to work. He came outside and reported injury.

Member was in the process of stepping over the miner cable near the stand off of the miner when he tripped over the cable and stumbled landing on his right foot in an adverse way causing a fracture.

Member was walking by scoop and tripped and fell hitting his head on the scoop canopy.

Member was trimming rubber around the edge of a pick up truck window to replace with a new window when the utility knife slipped a struck member in the leg. One suture was required.

Member was braiding splices and striked his finger

While making splice in belt using a hammer, piece of metal from the head of hammer broke off and struck him in the neck, requiring surgery to remove metal.

Employee was atempting to clean the dumping point with a scoop. As he passed underneath a low hanging cable he attempted to raise it up causing his left little finger to be mashed between the scoop canopy and the mine roof. This injury required stitches to be installed.

While cutting a piece of belt for a jumper in 1-A belt line, knife slipped and cut finger.

Roof fall in the intersection of #6 entry , one crosscut out by spad # 8205. Fall was 50'x5'x18'. No injuries or equipment involved.

Roof fall in the #4 entry, 3000' outby the active section. Fall was 20' x 20' x 4'. No injuries, entrapment or blockage of ventilation.

Rock was hung on side of shuttle car boom. Member shoved rock into shuttle car conveyor. Rock tilted up & pinched finger into top.

On 08/21/2009 employee was tightening wedges in a brattice and hit his middle finger, left hand with a hammer. Marfork Safety was not aware until notified by Brickstreet Insurance that this accident involved a fracture until 09/08/2009.

As determined by the results received on 08/11/2009, EE was diagnosed with a reportable hearing loss.

While welding on the dipstick tube, a piece of slag popped off, hitting his right eye lid. He was wearing glasses but the slag came in from the top of the frame and stuck to the eye lid. He was also using a welding helmet. EE did not stop work but later he did go to the clinic to make sure the burn did not hurt his eye.

member attached the fire hose to a value and turned the water on. The value was leaking so he was going to turn off the water and use the next value down the hill. value off it exploded into several pieces. Part of the value, still attached to the hose, struck him in the mouth. He was also struck in the left wrist and inside the right arm.

Employee was coming down the steps to the parking lot. As he stepped to the ground, his left ankle rolled to the outside, causing a popping noise.

While tramming a scoop in the #3 entry EE hit a hole in the bottom causing his head to hit the canopy. Due to experiencing pain in his neck he went to the hospital and was referred to a specialist for an MRI. On 03-05-2009 EE informed the Safety Department that he had been diagnosed with a bulging disc in his neck and not to work and attend therapy.

Injury was attempting to unload a bundle of roof bolt plates from a scoop bucket. As he removed a piece of wood from the bundle, some plates fell of the bundle stricking his left wrist causing a laceration that require 9 stitchs

EE was helping to install a belt head rock box, when a sheet of metal kicked out mashing his left thumb against another piece of metal, causing a laceration that required 11 stitches.

Employee was shoveling at a belt tailpiece when a chunk of coal rolled off the rib and struck his left knee area.

While making a belt splice on the Horse Creek # 2 Belt, Injury attempted to move the nailing plunger, causing the person doing the nailing to hit his hand with a hammer causing a fracture to his right index finger.

Members were working on mantrip to free a drive shaft that was fouled by the key steel from the brake disc. When the key steel was removed, the mantrip rolled away. Employee attempted to set the brake and was struck in the knee by the trip causing an injury that required surgery to repair a torn meniscus.

EE had dumped his shuttle car's load into the feeder. While changing seats in the shuttle car, he stepped into a hole which caused him to stumble and fall backwards onto the shuttle car striking his lower back and ribs on the canopy.

While pulling a steel chokee in the mine EE fell. EE states that due to a prevous injury that happened in 2005 causing a herniated disk that he was having problems with, his DR required him to remain off work.

Member was injured while working on a repair of a Joy 21 shuttle car. The shuttle car was in a raised position on top of crib blocks when the cribs shifted causing the shuttle car to fall on him.

Employee was picking up trash 2 crosscuts outby loading point when a bundle of plates fell out of the trash hitting him in the left hand requiring stitches.

The fall was discovered by mine foreman and state inspector while traveling the airways. The size of the fall was 40' x 20' x 4'. No injuries.

EE was attempting to pull a 1 1/2" water hose through the hole in a stopping when the end suddenly came through and the coupling struck him in the mouth.

Member was helping cut down roof bolts from belt head hole that were bent. A bolt fell and struck member on wrist just above gloves, requiring 3 stitches. Marfork safety did not know about accident until 7/10/2006 due to miner's vacation from 6/30/06 - 07/9/06.

Member was carring 7' timbers to #3 entry to timber the roadway and felt pain in lower back. The accident was not reported to the safety department until 7/4/2006.

Employee was backing up an A.L. Lee fork lift to align it with a man door. As he was backing up he ran over a rock in the roadway causing the forklift to bounce into rib. He went to Raleigh General hospital where x-rays were negative. He was referred to a doctor.

Employee was hanging shuttle car cable in the #4 Entry. As he lifted it his knee gave out. He continued to work and later on during the shift he was carrying two 5 gal. cans of oil and his knee gave out again causing him to fall. He visited Raleigh General Hospital where X-rays were negative.

Roof fall 130'X20'X5' in the #5 entry inby spad #172. No injuries, ventilation and passage were not effected. Roof support being used in area was 48" resin bolts.

Roof fall in #2 entry 18' Long, 5' Wide, 42" thick. No injuries were reported. Roof fall occured 640' Outby section, reported to Lincoln Selfe on 4/7/2006 at 5:20 PM. Fall will not be cleaned up. All approaches have been timbered off. Ventilation and passage of people not affected. Area was supported with 42" fully grouted bolts with 16' J-channel.

Employee was hooking a cable onto the scoop bucket as the scoop started to tram the scoop canopy caught on a roof bolt causing it to move to the right pinning him against the rib. He was transported to the hospital and released to return to work. On 3/30/06 He notified Safety that he had returned to the doctor an was told to remain off work.

Roof fall was discovered across from spad 3630 near Horse Creek #3 belt. Fall was Approx. 13,000 ft outby the #1 section. Fall was at the Intersection of the #5 entry. Did not disturb Primary escapway. Fall was 40' long X 19' wide X 6' high. Area timbered off at all approaches.

EE was attempting to pull slack on the feeder cable to allow the section electrician to connect it to the breaker. A rock Measuring 52" long X 16" wide X 7" thick fell striking him on right ankle. He was then transported to CMAC General where x-rays showed a fractured ankle.

Was operating a 988F Loader. He felt the over head A/C unit to see if it was producing cold air. When doing so his left middle and ring finger struck the fan blades causing a laceration that required stiches and a fracture to the tip of his middle finger of his left hand. This medical attention was reported to Marfork Safety

ON 8/22/05 DUE TO AN OCCUPATIONAL ILLNESS SUBSTANTIATED BY MEDICAL RECORDS AND REPORTS EMPLOYEE WAS DIAGNOSED WITH AILMENTS DESCRIBED IN THE MEDICAL REPORTS. THIS WAS NOT DUE TO ONE ACCIDENT BUT REPEATED TRAUMA

Roof fall in #3 entry along #10 belt conveyer at adjacent to spad #4517, fall was 40' by 20' by 48" high. Fully grouted 42" bolts were being used in this area. No injuries occured.

Roof fall in #4 entry at spad #3198 fell on horse creek #3 belt. Fall was 4' x 20' x 30' 4' fully grouted resin bolts were being used in the roof fall area. no injuries occured.

EE LOST CONTROL OF HIS TRUCK. HE HIT ANOTHER TRUCK & THEN TURNED OVER INTO A POND. EE BROKE BOTH LEGS. TRANSPORTED TO MED. CENTER BY AIR. ALL DAMAGES TO EQUIP. ARE NOT EVAL. AT THIS TIME. (TRUCK LANDED ON SIDE; COWORKER HELD EE'S HEAD ABOVE WATER UNTIL THEY COULD GET HIM OUT. UNKNOWN (7/1/05) IF EE HAD ADDL. INJURIES IDENTIFIED AT HOSP. 2ND TRUCK DRIVER NOT INJURED.)

Member, was moving rt. roof bolter when atrs extension came out its slots. When moving it back in place the end came up smashing finger on roof.

Carrying bucket of plaster, stepped in a hole and heard foot pop. The roadway was muddy and wet.

Member was removing shield on scoop & dropped shield on right hand breaking rt middle finger.

Member was pulling a piece of curtain up #6 entry when he stepped on a loose piece of coal twisting his right foot.

MEMBER AND FOREMAN WERE PULLING CABLE OFF A 21 S/C REEL TO ALIGN LEVELING ARM WHEN MEMBER CONTACTED A CUT PLACE IN CABLE RECEIVING AN ELECTRICAL SHOCK. CAUSING MEMBER TO FALL DOWN INJURING HIS BACK RIGHT HIP.

EE WAS USING SCOOP TO UNLOAD PALLET OF 6" BLOCK FROM FLAT CAR, WHILE PULLING BLOCK INTO BUCKET, CHAIN GOT UNDER BUCKET. EE LOWERED BUCKET TO RELEASE CHAIN. WHILE BACKING UP, SCOOP FELL OFF OF CAR CAUSING EE TO BE BOUNCED AROUND IN DECK STRIKING HEAD AND RT SHOULD ON CANOPY.

12/29/04 EE MADE A SECTION BELT MOVE AFTER EXITING THE MINE HE COMPLAINED OF SORENESS IN HIS LOWER BACK. HE WORKED ON 12/30/04 & TOLD HIS SUPERINTENDENT THAT HE WOULD BE OFF DUE TO OTHER COMPLICATIONS. 0N 1/10/05 DUE TO SORENESS EE VISITED A DR & WAS TOLD THAT HE MIGHT HAVE A PINCHED NERVE.

PUTTING CLAMP ON BELT RUBBER, WHEN TIGHTENING CLAMP WITH A HAMMER, HE HIT HIS OTHER HAND ON INDEX FINGER WHILE HOLDING CLAMP IN PLACE.

THE DRIVER WAS HAULING COAL FROM DEEP MINE TO PLANT DUMP SITE & THE TRUCK GOT LOOSE. HE BECAME SCARED & JUMPED FROM THE TRUCK, BREAKING HIS LT. ANKLE & CUTTING HIS HEAD. THE TRUCK STOPPED IN THE DITCH.

Member was helping put a wrecked flat car back on track. Jack flew out striking member on left hand.

WHILE ROOF BOLTING IN #4 X LEFT, EE STRUCK HIS RIGHT ARM ON A METAL STRAP THAT WAS LAYING IN THE DRILL STEEL TRAY, CAUSING A LACERATION ABOVE THE ELBOW THAT REQUIRED 7 STITCHES.

Member was attempting to pull a belt pin. member was stricking two hammers together part of hammer chipped off stricking member in forearm right imbeded object

EMPLOYEE WAS CUTTING CABLE HOLDING CABLE WITH A PAIR OF CHANNEL LOCKS. THE CHANNEL LOCKS SLIPPED & EE CUT THUMB ON LT. HAND.

EMPLOYEE WAS HELPING UNLOAD TIMBERS & FELT PAIN IN RT. SHOULDER. DID NOT THINK IT WAS HURT VERY BAD. EE HAS CONTINUED TO WORK UNTIL 8/3/04 UNTIL DR. HAS TOLD HIM THAT HE MUST OPERATE ON SHOULDER TO DETERMINE WHY HE CONTINUES TO HAVE PAIN & WHY HE CANNOT LIFT HIS RT. ARM.

EE WAS INSTALLING LAST ROOF BOLT IN #7 ENTRY. AFTER INSERTING GLUE INTO HOLE, EE PROCEEDED TO PUT BOLT IN HOLE. WHILE PUSHING BOLT INTO HOLE, HE EXPERIENCED PAIN IN HIS SHOULDER. X-RAYS WERE TAKEN AND DIAGNOSED WITH A STRAIN.

FALL ON #4 BELT INBY SPAD #850 APPROX. 60'. FALL APPROX. 20'W X 30'L X 4'. NO INJURIES. 42" RESIN BOLTS WERE BEING USED.

EMPLOYEE WAS USING A SLEDGE HAMMER TO BUST A PIECE OF ROCK OFF THE TOP OF THE SCOOP. A PIECE OF IT SLID OFF MASHING HIS LEFT INDEX FINGER REQUIRING SIX STITCHES.

EMPLOYEE WAS CHECKING THE INCH TRAM VALVE CHEST ON A ROOF BOLTER WHEN A PIECE OF ROCK MEASURING 6" X 24" X 1" - 4" FELL OUT LACERATING HIS RIGHTFOREARM.

On the 7-17-03 ee was loading 7' round timbers into a scoop bucket when he felt a pain in groin area. he continued to work until 5-4-04. at that time he was informed by a dr he had a hernia that would require surgery.

ROOF FALL 100' 18'X 36". 1 CROSS CUT INBY SPAD #743 ONT HE TRACK AND BELT ENTRY #4 ENTRY. 42" RESIN BOLTS WERE BEING USED.

EE LEANED OVER TO RAISE A HINGED SHIELD ON A CONTINUOUS MINER . WHEN HE FELT A PAIN IN HIS MID. BACK. APP WEIGHT OF SHEILD 25 LBS.

EMPLOYEE WAS MOVING THE CONTINUOUS MINER CABLE OUT OF THE ROADWAY. WHEN HE PICKED THE CABLE UP HE FELT A POPPING IN HIS ANKLE. X-RAY REVEALED A FRACTURED FIBULA.

EMPLOYEE WAS HANGING A SECTION OF MINER CABLE WHILE MOVING TO THE NEXT ROOF BOLT HIS LEFT LEG SLIPPED OUT AND HIS RIGHT LEG BUCKLED UNDER HIM. CAUSING A MUSCLE STRAIN. HE CONTINUED TO WORK UNTIL 10-8-02. AT THIS TIME HE WENT TO THE DOCTOR AND X-RAYS WERE TAKEN.

EE WAS UNLOADING 2 CANS OF 460 GEAR OIL. WHEN HE PICKED THEM UP & TURNED TO SET THEM IN THE RIB HEARD HIS BACK POP & FELL TO THE GROUND. HE WAS DIAGNOSED WITH A MUSCLE SPASM & TOLD TO REMAIN O FF UNTIL 9-28-02.

EE WAS OPERATING A UNDERGROUND FORKLIFT. HE ATTEMPTED TO PUT THE FORKS UNDER A BUNDLE OF BOLTS. IT CAUSED THE FORKLIFT TO RAISE STRIKING HIS HEAD AGAINST THE ROOF. HE CONTINUED TO WORK UNTIL 9 -14-02. HE WAS ADVISED BY DR TO REMOAIN OFF UNTIL 9-14-02.

EMPLOYEE WAS RIGGING A MAGNET TOWER FOR LIFTING WITH CRANE WHEN TOWER SHIFTED CASUING EMPLOYEE TO SLIP ON STEEL, CAUSING A RED PLACE BEHIND HIS RIGHT KNEE. HE DID NOT SEEK MEDICAL TREATMENT UN TIL 8/21/02. HE HAS BEEN REFERRED FOR A MRI FOR A DIAGNOSIS. NO WORK UNTIL FURTHER NOTICE.
