Employee was standing in some belt structure pulling on a cable when he got his feet tangled in structure causing him to fall. He said he twisted his upper body as he fell and that he felt pain in his right hip and leg.

EE alleges to have injured his back while going thru a mandoor. This is a re-occurrence of a pre-existing injury.

Employee was struck by a wire rope that had broken loose from the center of a snatch block. They were pulling a conveyor belt.

Employee was awarded 15% benefits (additional) for occupational pneumoconiosis.

While bolting, a rock fell from the mine roof & struck employee's arm(L). This required sutures.

Employee was shoveling loose coal on shaft 2 belt ways when a rib roll came loose and struck him on his right shoulder.

Employee pushed the wrong lever, he also had his hand on the machine near moving parts.

Employee claims to have injured his back when the scoop that he was operating trammed over a crib block causing him to bounce into the canopy & back into the deck.

Employee was lifting an overcast over his head and twited his neck.

Employee was standing beside the roof bolter (making up bolts) when the rib popped off & struck the employee in his lower back & legs- thus fouling him against the roof bolter frame.

Unloading overcasts off of track car which were stacked on top of one another; the overcasts were getting stuck by the bolts connecting them & they were sliding them before lifting when his helper pushed at the wrong time casing a pair of over- casts to fall on EE's foot.

EE alleges to have injured his back when he moved a Heintzmann Jack. This alleged accident was not reported until the end of his shift on 5/10/2006.

Employee claims to have injured his back when he jumped to avoid a piece of rock that had fallen from the rib. He did not start missing work until 5/9/06.

Unintentional roof fall in #4 Entry @ spad #12701 on the Spruce South Section. 55'(L) x 20'(W) x 6'(H).

Employee was struck by a piece of sandstone while walking from controls back to the front of the bolter. He had repositioned the bolter.

Employee was bolting top when a rock measuring 2"x4"x22.5" fell & struck him on his foot(L).

Employee was standing behind the roof bolter and was knocked off his feet when the other operator took up the cable. Management questions the validity of this accident. Employee notified management on 4/13/2006, that the reason he'd missed work was due to this accident.

While bursting rock with a pick hammer, employee struck his hand(L) with the hammer.

Employee injured his finger (R-Index) when the mantrip that he was riding was struck in the back by another mantrip. (Finger smashed - struck by batteries).

Unintentional roof fall 40'(L) x 30'(W) x 6'(H) in #3 Entry @ spad # 10613.

While installing a roof bolt, the drill wrench was in a bind & flew from the drill pot & struck employee in the mouth.

Employee was struck by a piece of rock while he was nailing a rib nail.

Employee was injured when the jack bar slipped out of his hand & struck his cheek (L) while he was putting a supply car back on the track.

EMPLOYEE WAS OPERATING A UNIHAULER WHEN HE STRUCK THE RIB, UNDERCUTTING IT. THE RIB ROLLED INTO THE DECK, STRIKING HIM IN THE BACK.

Unintentional Roof Fall measuring 22'(L) x 24'(W) x 3' to 5'(H). No. 3 Entry of Spruce #4 Belt @ Spad 8692.

Employee was struck by a rock that fell out from between roof bolts while he was walking to the back of the bolting machine.

Employee had a drill steel stuck in the chuck on the bolter. When he pulled it free, it struck him in the upper lip.

Unintentional roof fall measuring 60'(L) x 20'(W) x unknown height. It occurred on the Spruce West Section @ Spad # 11736.

Employee was bumped by a shuttle car while he was squatted down making notes approximately 10' from the loading end of the shuttle car. The shuttle car was in the process of being re-anchored.

Employee was attempting to put a crib under a feeder which was being blocked up on section tail piece when feeder tilted to left away from him. He was pushing a crib block under the frame when feeder struck crib block pinching his right thumb between feeder and crib block, crushing thumb.

Unintentional roof fall on Spruce Section, # 1 Entry at Spad 11608. Measurements: 80'(L) x 40'(W) x 7'(H).

Employee was granted a .55% Hearing Loss on 9/15/2005. Management became aware of this on 9/27/2005.

Unintentional roof fall 50'(L) x 18'(W) x 9'(H). # 7 Entry on Spruce- 1 break outby 11527. Roof fall was found on 9/15/2005- date & time unknown.

Employee was laying track and was flipping over a piece of track with a slate bar when he felt a pain in his lower back.

While removing twist drill steel from a hole that had just been drilled, EE injured his left hand & wrist when he contacted the spinning drill steel (fractured thumb).

While loading cable bolts on a scoop, employee stepped onto a bundle of combination bolts & lost his footing. He fell onto his right side.

Employee was shoveling mud out of take-up and twisted back.

EE slipped & fell against the scoop's operator deck while he was changing a tire on the scoop. He lacerated his lower lip, but did not miss any work.

UNINTENTIONAL ROOF FALL IN #2 ENTRY ON 2 LEFT SPAD 11326. 28'L X 20' W X 7' H. NO PEOPLE AFFECTED.

Unintentional roof fall on 2 Left #5 entry 1 break inby spad #1413. Dimensions:90' (l) x 20'(W) x 10'(h).

Unintentional roof fall occurred on Main First Right in #6 Entry near spad #833. It was 40'(L) x 10'(W) x 5'(H).

Employee was struck by a piece of rock that fell from the mine roof, struck the machine's canopy & bounced onto the employee's lower back.Employee did not start missing work until 6/4/2005.

Employee was moving material when he tripped. His leg got caught under a stack of metal, fell and thus injuried his ankle(R).

Employee was struck by a bolter cable when the cable that he was hanging broke loose.

EE WAS BACKING UP BELT RIDE (3-WHEELER USED FOR THE BELT CREW). EE SWUNG HIS FOOT OVER TO APPLY BRAKES AND GOT HIS FOOT CUAGHT BETWEEN BELT RIDE AND ROOF JACK.

EMPLOYEE WAS MOVING ONE END OF TRACK RAIL WITH A SLATE BAR WHEN HE FELT A BURNING PAIN IN HIS RT. LOWER BACK.

Employee was coming around the canopy (making a turn to the right) when his finger tips were caught in the swivel point. This resulted in two finger tips being pinched & crushed.

EE alleges to have injured his knee (R) while moving a can of hydraulic oil. He claims to have stepped in a hole & twisted his knee. THIS ACCIDENT WAS REPORTED ON 3/21/2005 & MINE MANAGEMENT QUESTIONS THE VALIDITY OF THIS ACCIDENT.

Injured was backing a miner up when the head of the miner came in contact with the mine roof. This contact with the mine roof caused a rock to fall from the top & to strike employee - knocking him down and causing his face to strike a large rock.

Employee was operating an underground forklift when a piece of rock fell from between roof bolts & struck EE.

EE WAS HANGING MINER CABLE AND MRS CABLE, BENT TO PICK UP CABLE AND MOVE IT AND CABLE WAS TIGHT AND EE PULLED OR FELT A PULLING SENSATION IN LEFT LOWER BACK AND BURNING IN STOMACH

EE injured his back when he bent over & picked up a drill. HE DIDN'T START MISSING WORK UNTIL 3/21/2005.

EE injured his back while attempting to change a center section on a scoop. (EE WAS UNDER SCOOP; TWISTED HIS BODY WHEN USING WRENCH.)

A timber slipped out of Employee's hand and struck his left hand as he was loading timbers.

Unintentional roof fall on Spruce #2 Entry near spad # 10068. Dimensions: 40'(L) x 20'(W) x 8' to 9' (H).

While employee was pulling a feeder cable out of the way, he slipped & fell- thus twisting his knee.

Employee alleges that he injured his back while operating a shuttle car over what he claims to have been a rough roadway. He did not report this accident until 1/29/2005. Mine mangement feels that this accident should have been reported before the employee left the mine- if that is where the injury occurred.

Employee was plastering inside top of track overcast. He was throwing plaster in corner top when some fell in his left eye.

Unintentional roof fall 40'(L) x19.5'(W) x 5' to 6' (T). # 4 Track Entry & # 5 Entry on Spruce Section near spad # 8696.

EE was sitting on a conveyor belt while the belts were down. When he attempted to stand up his knees buckled causing him to fall & twist his left quadracep muscle.

Employee was injured when the shuttle car that he was operating in #2 Entry (near spad # 10686) hit a hole in the bottom & jammed his head into the canopy. Management became aware of this accident on 1/17/05 & he started missing work on 1/20/05.

EE was awarded 5% for an occupational pneumoconiosis claim.

Employee notified mine management & started missing work on 12/22/04, due to Carpal Tunnel Syndrome.

Employee claims to have injured his back as he loaded timbers into a scoop.

Unintentional roof fall in the #1 entry of old 1st. Right Mainline - Mouth of Old Glory. Length unknown X 19' wide X 7' High. Spad #893.

EE was struck on his right arm by a piece of rock that fell from the mine roof while he was roof bolting. He received 4 sutures.

EE was struck by a piece of rock that had fallen between two rows of roof bolts as he was walking back to the roof bolter.

EMPLOYEE WAS TRAMMING SHUTTLE CAR TOWARDS THE INNER WHEN HE STRUCK A TIMBER CAUSING THE TIMBER TO ENTER THE OPERATOR'S DECK AND STRIKE THE EMPLOYEE.

Unintentional roof fall that occurred sometime between 10/20/04 to 10/28/04. Location #1, #2, & #3 Entries; inby spad #'s 10303, 10279 & 10265. Dimensions: 19' wide X 7' thick- lenght unknown.

Employee was walking when he stepped into a water hole, slipped & fell injuring his right knee.

While roof bolting, Employee was struck by a rock that fell from the mine roof, struck the canopy of roof bolter and then struck him.

A roof fall was discovered on the Old Spruce West Section in the #1 Entry @ Spad # 9060. The dimensions of this fall are: 90'(L) X 20'(W) X 7'(H). This fall occurred sometime between 10/12/04 to 10/19/04.

Employee claims to have injured his back when he ran over a rock in the roadway while operating a scoop. He did not start missing work until 10/21/04 & did not report accident to mine management until 10/22/04.

While setting the ATRS on the roof bolting machine, a piece of rock came down and struck EE's left foreman.

Employee alleges to have injured his back while loading cinder blocks into a scoop bucket.

Employee was injured when he was struck by a rock that fell from the mine roof and bounced off the drill head.

Employee was roof bolting the mine roof.The wrench that he was using to tighten a bolt came loose, fell against the drill pot of the roof bolting machine & bounced off - thus striking injured in his mouth.

Ee alleges to have injured his back & neck while hanging a roof bolter cable.

EMPLOYEE WAS CLEANING BELT AND BACKED INTO LOOSE ROOF BOLT AND A PIECE OF ROCK FELL AND BRUISED HIS NECK AND LEG.

USING LIFTING JACK, JACK SLIPPED AND JACK BAR HIT EMPLOYEE ON LEFT WRIST.

Employee was awarded 6% disability for Carpal Tunnel Syndrome. This accident has been previously reported.

While operating a continuous miner on a pillar section, employee was injured when a rib rock measuring 5'x 2'x 6" fell & struck him on his left leg. He was preparing to take the 1st lift to the left in the #7 Heading.

Unintentional Roof Fall in the return (old works) app. 7200' from face in Pit II, #1 & #2 Returns. Dimensions: 300' Long X 20' Wide X 5' High. No spad # available. Fall discovered during weekly exam.

EE WAS WALKING IN THE TRACK ENTRY WHEN HE STEPPED ON A ROCK AND TWISTED HIS LEFT KNEE.

Roof fall occurred in #3 entry at spad #9660, on the spruce section, MMU 004-0. Dimensions 22' long by 40' wide by 8' high.

Employee was bolting top when he turned, striking his left elbow on a bundle of roof plates.

Employee was awarded 12% ppd benefits for carpal tunnel syndrome.

Ee was bolting top when the suction on the bolter quit. He dropped the head and when he placed his hand on the head of the bolter to check suction, the steel fell from hole and struck his right hand.

EE was on offside of bolter stated he was not paying attention when operator moved up to next row of bolts, bolter tire ran over his foot.

A roof fall was discovered in the #7 entry at spad #9751. The roof fall measured 80' long by 18' wide by 6' high.

Employee was awarded 5% ppd benefits for occupational pneumoconiosis.

A ROOF FALL WAS DISCOVERED AT 3:00 AM BY THE FIREBOSS. THE ROOF FALL WAS LOCATED IN THE NO.1 ENTRY AT SPAD NO. 9686. THE FALL MEASURED 70'X 19.5'X 8'THICK.

EMPLOYEE WAS SHOVELING BELT AND WAS LIFTING A ROCK ON THE SHOVEL WHEN THE ROCK SHIFTED AND HE TWISTED HIS WRIST.

Employee was bolting top when a rock fell, striking employee on right forearm.

ROOF FALL MEASURING 60-65'L X 5 1/2-20'W X 4-5'H. LOCATION: OUTBY SPRUCE MAINS IN #5 ENTRY NEAR SPAD #8632.

MOVING 60 LB RAIL HIT EE ON RT LEG & KNEE CAUSING BRUISE.

Employee was awarded a 9.16% Hearing Loss Award on 11/19/04.He last worked on 3/19/04.

Ee reported being struck by a rock that fell from between bolts on 3-6-04. He claims to have been struck on top of his head by rock. However he did not lose any work. He contacted mine mgmt on 6-15-04 alleging that he needed an MRI & was having problems due to previous injury. We feel that this alleged injury & lost time are result of altercation w/police 6-12-04 & not reportable.

WHILE ROOF BOLTING TOP, EE PLACED A SHORT MAT ON THE ATRS. (THE MAT WAS NEEDED TO SUPPORT A KETTLE BOTTOM). WHEN EE TURNED TO PICK UP A ROOF BOLT, THE MAT SLID OFF THE ATRS, STRUCK HIM IN THE FACE. THIS IS NOT A LOST TIME ACCIDENT.

EE STARTED MISSING WORK TO HAVE CARPAL TUNNEL SURGERY, CO WAS NOT AWARE OF REASON FOR ABSENCE UNTIL 2-24-04.

EE WAS CUTTING A PIECE OF BELT WHEN THE UTILITY BELT SLIPPED AND CUT HIS LEFT THUMB. THIS IS NOT A LOST TIME ACCIDENT.

EE WAS AWARDED 5% PPD BENEFITS FOR OCCUPATIONAL PNEUMOCONIOSIS.

CARPAL TUNNEL SYNDROME IN BOTH WRISTS.

EE WAS MOVING BELT TOOLS FROM ONE PLACE TO ANOTHER WHEN HE STRUCK HIS HEAD AGAINST A CABLE BOLT CAUSING HIM TO TWIST AND JAM HIS NECK, RESULTINGIN PAIN. HE WAS TRANSPORTED OUTSIDE AND HAS BEEN OFF WORK SINCE.

EE CLAIMS TO HAVE INJURED HIS BACK WHILE DRIVING A PIN INTO THE RUB RAIL OF A CONTINUOUS MINER. HE WAS USING A FIVE POUND SLEDGE HAMMER.

A ROCK FELL FROM BETWEEN ROOF BOLTS STRIKING EE'S RIGHT HAND.

EE WAS PULLING LOOSE ROCK & COAL FROM THE RIB TO BUILD A STOPPING. ONE OF THE SHARP EDGES HIT HIS THUMB CAUSING THE INJURY.

THERE WAS A FUST THAT FLEW IN THE SPRIKE LAUREL SUBSTATION THAT RESULTED IN LOSS OF POWER TO THE MINE ELEVATOR FROM 1:30 PM TO 3:42 PM. THREE MINERS WERE ON THE ELEVATOR DURING THIS POWER LOSS .

A PILLAR FALL OCCURRED IN #3 ENTRY ON THE MF SECTION THAT ENTRAPPED TWO MRS'S. A RETRIEVER WAS USED TO RECOVER THE MRS'S. THE FALL MEASURED APPROXIMATELY 10' TO 15' HIGH.

EMPLOYEE WAS PICKING UP TIMBERS WHEN HE FELT PAIN IN HIS LOW BACK.

EE WAS HELPING RECOVER BELT STRUCTURE BY PICKING IT UP AND SWINGING IT OVER THE SIDE OF A SCOOP BUCKET WHILE DOING THAT HE STARTED FEELING PAIN. HE FINISHED THE SHIFT AND WENT TO THE DR THE NE XT DAY, HE HAS BEEN OFF WORK SINCE.

EMPLOYEE WAS AWARDED 12% PPD BENEFITS FOR CARPAL TUNNEL SYNDROME.

EE WAS INSTALLING ROOF BOLTS ALONG THE TRACK ENTRY. WHILE INSTALLING A BOLT THE CANOPY FOOT SLIPPED OFF THE RAIL CAUSING THE CANOPY TO DROP HITTING HIM ON THE HEAD AND SHOULDER WHICH CAUSED TH E INJURY.

EMPLOYEE WAS WALKING WHEN HE WALKED INTO ROOF BOLT, INJURING HIS LEFT EAR.

EMPLOYEE TURNED WHILE SETTING OIL CAN ON RIDE AND TWISTED HIE LEFT KNEE.

A ROOF FALL OCCURRED IN THE TRACK ENTRY CROSSCUT BETWEEN SPAD #2025 AND SPAD #2020. THE FALL MEASURED 60' LONG BY 19' WIDE BY 6' THICK.

EE WAS AWARDED 5.335% PPD BENEFITS FOR OCCUPATIONAL NOISE INDUCED HEARING LOSS.

WHILE WELDING, EE CHIPPED SOME SLAG & IT WENT IN HIS LEFT EAR. EE BEGAN MISSING WORK ON 9/23/03.

EE STARTED MISSING WORK ON 9/18/03 WITH CARPAL TUNNEL SYNDROME. THIS IS AN OCCUPATIONAL ILLNESS.

EMPLOYEE STARTED MISSING WORK ON SEPTEMBER 2, 2003 WHEN HE WAS OPERATED ON FOR CARPAL TUNNEL.

EMPLOYEE WAS INSTALLING ROOF BOLT WHEN A ROCK FELL FROM TOP, HITTING EMPLOYEE ON LEFT FOOT.

EE WAS WALKING WHEN HE SLIPPED AND FELT PAIN IN HIS BACK.

EE WAS HELPING MOVE THE MINER BY PULLING THE CABLE OUT OF THE FACE. HE WAS PULLING THE CABLE WHEN HE FELT PAIN IN HIS BACK. HE WENT OUTSIDE SOON AFTER. HE HAS BEEN OFF WORK SINCE THEN.

EE WAS UNLOADING SUPPLIES OFF OF THE SCOOP. APPARENTLY THE SCOOP BUCKET WAS SITTING ON SOME CHUNKS OF COAL. HIS FOOT WAS UNDER THE BUCKET AS THE COAL CHUNKS FAILED - THUS RESULTING IN HIS INJU RY.

EE ALLEGES THAT HE HAS EXPERIENCED WORK RELATED PAIN IN HIS LEFT KNEE.

A ROOF FALL WAS FOUND ON 6/25/03 AT SPAD #371 ONOLD NO.3 PIT BELT. FALL WAS 19'X40'X7.5'

EE WAS DRIVING THE RUBBER-TIRED MANTRIP WHEN HE RAN OVER A ROCK, CAUSING THE STEERING WHEEL TO TURN, STRIKING HIS LT. HAND.

EMPLOYEE WAS WALKING BESIDE SCOOP WHICH WAS LOADED WITH CRIB BLOCKS. A CRIB BLOCK FELL FROM BUCKET AND FRONT TIRE. THE CRIB BLOCK FLIPPED UP AND STRUCK EMPLOYEE ON HIS LEFT SHOULDER AND FACE.

EMPLOYEE RECEIVED BENEFITS & STARTED MISSING WORK DUE TO CARPAL TUNNEL SYNDROME.

EMPLOYEE WAS CUTTING OPEN A BOX WHEN HE CUT HIS LEFT HAND.

A PILLAR FALL OCCURRED JUST INBY SPAD #7845, ONTO A MRS UNIT, ENTRAPPING IT UNTIL IT WAS REMOVED.

EMPLOYEE WAS PICKING UP BELT STRUCTURE WHEN HE FELT A SHARP PAIN IN HIS LEFT SHOULDER.

EMPLOYEE WAS TRAMMING MINER WHEN THE BOOM OF THE MINER CAUGHT HIS RIGHT HAND AGAINST RIB. EE BELIEVES THAT HE MAY HAVE CAUGHT TRAM LEVER WITH HIS LIGHT CORD. ALL EMPLOYEE'S HAVE BEEN REPEATEDL Y CAUTIONED ABOUT STAYING CLEAR OF THE MINER WHEN IT IS ENERGIZED.

EMPLOYEE WAS PULLING A SLACK IN CABLE & THROWING IT ON THE RIB. HE STEPPED IN A HOLE & TWISTED HIS ANKLE.

EE WAS AWARDED 12% BENEFITS FOR CARPAL TUNNEL SYNDROME.

Employee was shoveling belt when he stepped on a 3" water lime and twisted his ankle.

EE WAS REMOVING BELT STRUCTURE. ANOTHER EE BUMPED SOME STRUCTURE & WHEN IT WENT TO FALL, EE TRIED TO CATCH IT. HIS HAND WAS CAUGHT BETWEEN TWO PIECES OF STRUCTURE.

EE WAS MAKING A BELT SPLICE ON THE SECTION BELTWHEN EE ALLEDGES HE INJURED HIS BACK.

EE WAS LIFTING & TOSSING BELT STRUCTURE WHEN HE FELT PAIN IN HIS LOW BACK.

EE CLAIMS TO HAVE INJURED HIS LEFT KNEE WHEN HE STRUCK HIS KNEE WITH A METAL JACK. EE STARTED MISSING WORK ON 2/19/03. MANAGEMENT BECAME AWARE OF THIS ON 2/27/03 WHEN A WORKERS COMP. REPORT WAS RECEIVED.

EE WAS WORKING ON A BELT MOVE, WHEN HE FELT A TWINGE IN HIS SHOULDER. LATER IN THE SHIFT, HE WAS WORKING NEAR A CRIBBED AREA WHEN A CRIB FELL ON HIM. EE FINISHED THE SHIFT & FIRST MISSED WORK 1-13-03.

EMPLOYEE WAS AWARDED 10% PPD BENEFITS FOR OCCUPATIONAL PNEUMOCONIOSIS.

A ROOF FALL OCCURRED ON THE 1ST RIGHT BELT AT SPAD #924. THE FALL MEASURED 20' WIDE BY 12' LONG BY 5' THICK.

EE STATED THAT HIS KNEE WAS SWOLLEN AND PAINFUL HE DID NOT INJURE HIS KNEE IN THIS MINE, BUT STATED THAT THIS WAS AN OLD INJURY THAT BECAME INFLAMED

EMPLOYEE STEPPED IN A HOLE AS HE WAS EXITING THE MANTRIP. HE FELT PAIN IN HIS LEFT KNEE AND BACK OF LEFT LEG. HE DID NOT MISS WORK UNTIL NOV. 12 2002.

EE WAS LIFTING A PALLET WHEN HE FELT PAIN IN HIS LOW BACK.

EMPLOYEE WAS CUTTING DUST HOSE WHEN HIS RAZOR KNIFE SLIPPED, CUTTING HIS RIGHT THIGH.

EMPLOYEE WAS BEATING TWO PIECES OF METAL TOGETHER WHEN HE SLIPPED AND SMASHED HIS FINGERS. EMPLOYEE WAS THROWING A TRACK SWITCH.

EE WAS SCALING TOP WITH A SLATE BAR WHEN HE FELTPAIN IN HIS LOW BACK.

EE WAS OPERATING A SHUTTLE CAR WHEN HE FELT PAIN IN HIS LOWER BACK.

AN UNINTENTIONAL ROOF FALL 80'X19'5' OCCURRED ON OLD CASEY AT SOAD #3528.

ELECTRICAL PROBLEMS WITH ELEVATOR FROM 0740 TO 1130.

VICTIM WAS TAKING A PIECE OF DRAW ROCK OFF OF THE MINER WHEN HE STRAINED HIS GROIN. HE DID NOT START MISSING WORK UNTIL 9/3/02.

EE WAS MOVING MINER CABLE WHEN HE PULLED A MUSCLE IN HIS ABDOMEN. EE FIRST MISSED WORK ON 8-19-02.

EE WAS HOLDING CABLE & PREPARING TO PUT CABLE ON CONTINUOUS MINER. ANOTHER EE PULLED HIM AWAY FROM THE CABLE. THIS RESULTED IN EE BEING IN AN AWKWARD POSITION & STRAINING HIS BACK.

EE WAS TRAMMING A SHUTTLE CAR TO THE CONTINUOUS MINER. HE RAN OVER A ROCK IN THE ROADWAY, CAUSING HIM TO STRIKE HIS HEAD ON THE CANOPY.

EE WAS BOLTING TOP WHEN THE BEARING PLATE ON THEROOF BOLT HE WAS INSTALLING SWUNG AROUND AND STRUCK HIM ON THE LT. FOREARM.

A ROOF FALL WAS DISCOVERED IN THE #6 AND #7 ENTRIES, 1000' OUT BY THE FACE. THE FALL MEASURED 20' BY 20' BY 7-8' THICK.

EE WAS WORKING AS A GENERAL LABORER, WHEN HE CUT HIS HAND & WRIST ON A METAL BAND.

EMPLOYEE WAS CHANGING A CAT IDLER ON THE MOBILE BRIDGE CARRIER, SWINGING A CRIB BLOCK WHEN HE FELT PAIN IN HIS RIGHT SHOULDER. EMPLOYEE DID NOT MISS WORK UNTIL 8/16/02.

WHILE BUILDING CRIBS, EE FELT A PAIN IN HIS BACK.

EE FRACTURED HIS THUMB WHILE BLOCKING UP A MBM WITH CRIB BLOCKS.

EMPLOYEE WAS GETTING OUT OF SCOOP DECK WHEN HE STEPPED INTO A RUT IN THE ROADWAY. HE STARTED TO FALL AND WHILE RATCHING HIMSELF HE TWISTED HIS LOW BACK.

EE ALLEGES TO HAVE INJURED HIS NECK WHILE OPERATING A SHUTTLE CAR. HE CLAIMS TO HAVE HIT A HOLE IN THE BOTTOM THUS CAUSING HIS HEAD TO STRIKE THE CANOPY.

A ROOF FALL OCCURRED ~4600' OUTBY THE SPRUCE MAINS SECTION NEAR SPAD #5298. THE FALL MEASURED 200' L X 19.5' W X 7' H. IT AFFECTED ENTRIES 4,5,6, & 7.

EE WAS DRIVING A WEDGE INTO A STOPPING WHEN A PIECE OF PLASTER FLEW INTO LEFT EYE.

A ROOF FALL MEASURING 20'X 20'X 8-9'HIGH OCCURRED 1600 OFF OF THE SORUCE SECTION- ONE BREAK INBYSPAD #5627.

EMPLOYEE WAS OPERATING A MOBILE BRIDGE CARRIER. HE BACKED OVER SOME UNEVEN BOTTOM & BOUNCED BRIDGE CARRIER. HE STRUCK HIS HEAD AGAINST CANOPY.

EE WAS ATTEMPTING TO GET ROCK OFF OF CONTINUOUS MINER. HIS BAR SLIPPED AND HE FELL INTO PAN AREA NEAR THE HEAD MOTORS.

HE WAS TRYING TO PUT A CHAIN ON A SHUTTLE CAR WHEN THE SLATE BAR SLIPPED AND STRUCK HIS LEFT HAND. HE WENT TO THE DR ON 2-22-02 AND RECEIVED PAIN MEDICATION.

A ROOF FALL OCCURRED ON SPRUCE SECTION NEAR SPAD NO. 5609. THIS FALL WAS LOCATED BETWEEN #6 & #7 ENTRIES APPROX. 1,200' FROM THE FACES. IT MEASURED 60' X 20' X 7'.

EE WAS AWARDED 5% PPD BENEFITS FOR OCCUPATIONAL PNEUMOCONIOSIS.

EE WAS BRINGING OUT SCOOP TO PICK UP TRAM MOTOR. AT PORTAL, ONE DOOR WAS OPEN, ONE WAS CLOSED. WHEN HE ATTEMPTED TO OPEN DOOR, THE OTHER SWUNG CLOSED, CATCHING EE S HEAD AND SHOULDERS BETWEEN DOORS.

EE WAS TRAMMING SHUTTLE CAR WHEN HE RAN OVER A 10" OFFSET IN THE ROADWAY. THE SHUTTLE CAR BOTTOMED OUT, JARRING EE'S LOW BACK. EE DID NOT MISS WORK UNTIL 2-11-02, WHEN HE NOTIFIED COMPANY OF I NJURY & SOUGHT MEDICAL ATTENTION.

EE WAS AWARDED 2.% PPD BENEFITS FOR NOISE INDUCED HEARING LOSS.

EE WAS BENDING A ROOF BOLT WHEN HE FELT PAIN IN HIS BACK.

EMPLOYEE WAS PULLING MINER CABLE WHEN HE FELT PAIN IN HIS UPPER BACK.

EE WAS PUSHING ROCK OFF OF MINER WHEN HE CUT HIS LEFT ARM ON A SHARP ROCK.

A ROOF FALL OCCURRED IN BY SPAD #5505 ON THE CASEY SECTION OVER THE CHRISTMAS HOLIDAY. THE FALL MEASURED 8' HIGH BY 30' LONG BY 20' WIDE.

EMPLOYEE WAS GOING THROUGH A DOOR WHEN HE BENT OVER AND FELT PAIN IN HIS LOW BACK.

EE WAS STRUCK IN THE LOWER BACK BY A RIB ROLL WHILE OPERATING THE MINER.

EMPLOYEE WAS BOLTING TOP WHEN A SMALL PIECE OF DRAW ROCK FELL, STRIKING EMPLOYEE ON ANKLE AND CUTTING THROUGH BOOT.

EMPLOYEE FELL FROM A BELT CHUTE STRIKING HIS HEAD ON THE GROUND. THE BELT INADVERTANTLY STARTED AFTER HE TURNED THEM OFF.

WHILE CRAWLING OVER ROCKS EMPLOYEE TWISTED HIS KNEE. EMPLOYEE THOUGHT THAT HE HAD A BRUISE AND WORKED THROUGH IT. HE WENT TO THE DOCTOR ON 10-12-01, REPORTED THE INJURY TO MANAGEMENT ON 10-22- 01. NO LOST TIME UNTIL 10-22-01.

EE STATED THAT HE HURT HIS BACK PULLING CABLES. TIME OFINJURY IS UNKNOWN IN THAT PAIN OCCURRED AFTER THE SHIFT.

EE WAS TRAMMING SHUTTLE CAR WHEN HE HIT A HOLE IN THE ROADWAY. HE STRUCK HIS HEAD AGAINST CANOPYAND SAID HE FELT PAIN IN HIS NECK.

EMPLOYEE INJURED HIS BACK WHEN HE HUNG THE MINERCABLE IN THE TRACK ENTRY.

PULLED A MUSCLE IN HIS LOWER BACK WHILE LIFTING A ROCK. EMPLOYEE DID NOT START MISSING WORK UNTIL 10-1-01.
