An unplanned roof fall occurred on 2 Gas Mainline 3 section at 38 crosscut, #3 intersection, in the last open break.

An unplanned roof fall occurred in the West Mains Return, #3 entry between 7&8 cross cut. Approximately 20'x75'x6'

Employee was operating a continuous miner on 2 Gas Mainline 3 section when they were struck in the left foot by a rib roll measuring 7' long by 15" wide by 3" thick. Employee returned to work after the accident but was placed off work on 11/5/2025.

Employee fell from ladder access door approximately 8' landing on their back.

An unplanned roof fall occurred on J7 section, 38 crosscut, #5 entry approximately 20'x20'x7'.

Employee was installing a roof bolt when EE stated EE's back was hurting. Employee went to the hospital and was released and returned to work. Employee went to EE's primary care physician on 6/9/2025 where an MRI was conducted and produced negative results and employee returned to work. Employee returned to EE's primary care physician on 7/9/2025 where EE was placed off work.

While removing CLA bolts from a continuous miner, the bolt broke loose causing the employee to jam EE's wrist. Employee originally went to ER on 4/1 and to orthopedic on 4/4 where EE received a full release and returned to work both visits. At a follow-up with orthopedic on 4/17/2025 x-rays were conducted for a third time with a diagnosis of a fracture to the wrist.

CM operator was injured when a piece of rock fell from the roof, hitting the floor then rolling onto the operator's right leg and foot.

Employee was waiting for the bolt crew the finish spotting a bolt when they were struck in the back by a rock from the rib.

While operating a roof bolter, a rock fell causing the employee to turn away and stumble. The employee fractured right wrist as employee attempted to stop self from falling.

While walking the shuttle car cable during pre-op inspection employee's nose was caught by a rope hanger.

While positioning a shuttle car tire to be loaded by a forklift in the warehouse, the tire fell over landing on the employee's right leg.

Employee was walking away from the rock duster when employee slipped and fell landing on side.

While dropping a roof bolter cable out of a rope hanger the hanger swung and caught the employees nose.

Employee was operating continuous miner when they were struck in the left foot by a rock that came off the boom of the miner.

An unplanned roof fall has occurred on J4 panel, #1 entry, 17 crosscut, approximately 13 feet high.

An unplanned roof fall occurred at 2 Gas Sub1, 24 crosscut, #6 entry, and approximately 7' thick.

Employee was removing rock from the top of the continuous miner when a rock fell from the roof striking the employee on the right arm.

Employee alleges employee was injured at work on 09/30/2023. Employee did not report nor notify anyone of an alleged incident until employee sought medical care on the evening of 10/1/2023. Employee has not identified how employee was injured and is unable to provide any details of the alleged incident.

Employee was opening a conveyor torque shaft tube for a continuous miner when the screwdriver EE was using struck EE's left hand between the thumb and index finger.

An unplanned roof fall was discovered at 9:50pm and the 2 gas seam 19 xcut #1 entry on J3 section.

Employee was pumping grease into the idler pump on a continuous miner to insert a shim. While inserting the shim, the grease hose fitting broke causing the hose to become detached. The employee was struck in the side of the face by the pressure released from the fitting.

Employee was using a prybar to remove the tail shaft from a shuttle car. When the tail shaft dislodged, the employee fell forward as the shaft dropped causing their hand to be pinched between the shaft and the mine floor.

An unplanned roof fall has occurred on Alma #3 belt in the #3 entry, crosscut 1-2 that is approximately 50'long by 16' wide by 7.5'.

Employee was walking and stepped in a hole and hyper-extended EE's knee. Employee continued to work until taken off for surgery on 2/2/2023.

Employee was using a chain ratchet on the slope belt take-up when the hook and load chain came loose from the anchor point and struck the employee in the left side of abdomen.

Employee stepped into some mud when EE's foot got hung. Employee attempted to pull EE's foot loose from the mud when EE twisted EE's knee. Employee continued to work up to and including owl shift 2-14-2022. On 2-14-2022 at a follow up appointment EE was placed off work until surgery for a meniscus could occur.

Employee was standing in #16 track spur waiting to chain off ballast cars. Once the cars stopped, the employee began to connect the clevis to a car when the cars moved pinching the index and middle finger on right hand.

Employee bent over to re-position a 72" piece of belt structure for loading into a scoop when they felt pain in back. Employee was diagnosed with a protruding disc and placed off work on 11/19/2021.

Employee was operating a mantrip on the track when employee struck another piece of track equipment. The mantrip operator was transported to the hospital.

Employee was hanging 10" waterline when the waterline came out of the chain holding it and struck employee in the right knee. It was determined through MRI results on 9-22-2021 that employee has a partially torn ACL and required therapy.

Employee was helping build a stopping. While attempting to wedge the stopping the block kicked out and fell striking employee's left foot.

Employee was changing a mast jack on a roof bolter. The jack had been put in place when the mast slid down mashing employee's hand between the mast and jack.

Employee was standing next to the feeder when a rock struck the skirt board and broke the weld causing the skirt board to strike the employee in the left arm.

Elevator down on what appeared to be the overwind device.

The employee was roof bolting in 6 left. As EE was in the process of installing the 2nd bolt in the last row, a portion of the rib rolled out behind EE. The rib rock struck the employee in the back, knocking EE into the bolter, pinning EE against the machine.

The employee was in the #6 entry when a portion of the right rib rolled out. The rib hit EE in the lower back, knocked EE to the ground, pinning EE's feet under the rock.

The shuttle car had pulled under the continuous miner to be loaded; the continuous miner operator turned to glance at the S\C operator, and at that same moment, a large rock came off the miner boom, hit the shuttle car, and landed on the miner operator's right foot.

Roof fall occurred in the face of #3 entry that was approximately 20 feet in length.

The employee was operating a track mounted mantrip and struck a parked motor that the dispatcher cautioned them about being parked on the track. The employee was taken off work July 8 after they were evaluated by an orthopedic physician.

The employee was removing tie wraps from a socket set with a pocket knife, the knife slipped and the blade tip punctured left hand.

Employee was walking across the track at Alma 5 to start the belt. EE tripped on the track and fell on right knee and right hip. *Note: EE has been under referral to a specialist, and on 6-29-2020 they received results of the MRI. EE had a strained right hip and degeneration in right knee.

The employee was riding as a passenger in the #10 track mounted mantrip, on EE's way to the section. As the mantrip entered the curve at #35 Spur switch they were struck by the L4 locomotive that was traveling in the opposite direction. *Note: The employee was under referral to a specialist, on 6-04-20 it was determined by the doctor that EE was unable to work.

The employee was riding as a passenger in the #10 track mounted mantrip, on the way to the section. As the mantrip entered the curve at #35 Spur switch they were struck by the L4 locomotive that was traveling in the opposite direction.

Employee was knocking down a brattice and a rock fell out and hit them in the head causing injury

Employee was moving a hopper jack from the elevator to the track supply car by using a parts cart. As they were positioning the cart next to the parts car, the cart's wheel dropped into a broken section of the concrete pad, causing the jack to slide off and drop onto the employees foot.

Employee was tramming a scoop when the canopy caught a waterline coupler. This caused the waterline to break, and the end of the waterline struck the employee in the head.

The employee was helping dislodge an obstruction in the discharge line of a dewatering pump. When the blockage broke free the sudden pressure release caused the end of the line EE was holding, to whip back, striking EE in the face.

The employee was removing a broken continuous miner bit by trying to knock it out with a hammer and a punch. Unsuccessful with the hammer and punch, EE used another miner bit in an attempt to remove the broken one. As EE was striking the miner bit with EE's hammer, EE missed the bit and struck EE's left thumb.

The employee was moving the MRS back to set breaker timbers, when a portion of roof rock fell out from between roof bolts, striking employee on the right ankle and foot.

Employee was walking the beltline and discovered a piece of drawrock in the walkway, under the lifeline. EE bent over to flip the rock out of the walkway, and felt a pain in EE's lower back and left leg. An MRI indicated the employee had a herniated disc in the lower back.

Employee was replacing shields on the continuous miner and had pulled the rub rail pin to provide enough clearance to slide a shield into place. As EE was doing this, EE had left hand on the bottom of the rub rail when it unexpectedly came loose and dropped, pinching EE's left hand between the rub rail and a crib block.

The employee was standing next to the continuous miner in the #4 entry. The employee had placed left hand on the machine's light guard as employee was reaching for a can of oil, when a piece of roof rock, fell from between roof bolts, striking employee's left hand.

An unplanned roof fall has occurred at 21 xcut Cedar Grove Mains #8 entry.

An unplanned roof fall has occurred at crosscut 37 on the cedar grove seam in the track entry. The fall measures approximately 40' long and 10' high.

An unplanned roof fall occurred in the #6 entry, 18 cc, outby the North Elevator. The fall was approximately 20 feet long, 40 feet wide, and 8 feet high.

An unplanned roof fall has occurred in the Alma seam, #5 entry, #38 x-cut; fall is approximately 18 ft. wide, 90 ft. long, and 6 ft. high.

An unplanned roof fall has occurred on the Cedar Grove #1 belt at 15 crosscut and is approximately 18' wide, 5' thick, and 130' long.

As the employee was walking to EE's work area, EE stated EE twisted both EE's knees when EE slipped and fell. Employee continued to work until doctor took EE off.

Unintentional roof fall occurred in the #2 Entry, #12 crosscut of the south mains area. The fall is approximately 20' long x 18' wide x 6' high.

Employee was building an overcast and carrying a solid cinder block on shoulder; employee was packing block to an area where the entry height was a little lower than employee was accustomed to. As employee continued to walk down the entry, employee misjudged the clearance between the block and the roof, and jammed right hand on a roof bolt.

While ee was using a drill press in the shop, they cut two of their left hand fingers requiring stitches.

While roof bolter operator was unloading supplies out of a scoop bucket, injured struck EE's left elbow on the side of the bucket. Employee was referred to an Orthopedic Specialist and then diagnosed with a fractured elbow. Employee was told by specialist there was no treatment and was released with no restrictions.

Employee was unloading concrete and got finger caught between guard and concrete causing injury to pinky finger

An unintentional roof fall occurred in the Alma coal seam, #2 entry, #129 XC. The fall is approximately 6 feet high, 20 feet long, and 20 feet wide in the intersection.

While injured was walking ee fell to the ground on knees. When ee got up, ee couldn't put pressure on right knee. Injured was put off from work pending an MRI.

While bolting top, a piece of rock started to fall. Injured started to move out of the way when EE slipped and fell onto left shoulder. Inujured was seen and released with no restrictions, and continued to work. Injured was later seen by a specialist and diagnosed with a torn rotar cuff, at which time caused this incident to become reportable.

While operating the shearer, injured placed hand on the pan line and a rock hit left ring finger, resulting in a laceration that required 8 stitches.

While placing pin in a scoop bucket jack, injured was lining pin up by placing finger in the hole. The jack fell, striking the injured employees finger resulting in a fracture and avulsion of the right index finger.

Employee was lifting block and felt pain in shoulder

The employee was picking up a cinder block, and as ee turned, ee felt a pain in back.

Employee was throwing cinder block off supply car when they felt a sharp pain in the left shoulder.

Employee states they hurt their back moving fire box and belt structure.

Employee was working on stopping and pushed ladder through the mandoor causing injury.

While putting headdrive cover back in place, the employee could not get the cover to seat properly and used screwdriver to pry the cover up. The employee discovered a small rock under the cover and tried to remove it when the cover dropped, pinching the end of employee's ring finger.

An unplanned roof fall occurred on the South Mains Track at 11 crosscut. The size of the fall was approximately 75 feet long x 18 feet wide x 5-1/2 to 6 feet high.

An unplanned roof fall occurred in the Cedar Grove Mains Return, at #29 X-cut, #8 entry. The fall is approximately 20 feet long x 20 feet wide x 5 to 6 feet high.

A unplanned roof fall was discovered on the South Bleeder Track, at #22 X-cut. The approximate size of the fall was 18 to 20 feet long x 18 feet wide x 4-1/2 - 5 feet high.

As the injured employee walked behind the section power center, EE's foot got caught on the ground cable that was attached to the power center and high voltage sled, causing EE to fall. Injured was transported to the hospital and it was revealed that EE had fractured EE's fibula.

An unintentional roof fall occurred in the Cedar Grove Mains, at #11 XC, #5 belt entry in the intersection. The fall is approximately 18 feet wide, 20 feet long, and 6 feet high.

An unintentional roof fall occurred in the Cedar Grove Mains, at #110 XC, #4 belt entry and through the crosscut to the intersection of the #5 track entry. The fall is approximately 18 feet wide, 30 feet long, and 6 feet high.

An unplanned roof fall has occurred on the Cedar Grove #1 mainline belt at 14 crosscut. The roof fall is approximately 25' long by 6' thick.

The employee was realigning the track by using lifting jacks to raise the track, and slate bar to slew it into place. One jack did not kick over and employee took the slate bar to move it and the track fell on their foot.

Employee was struck on the hand by a link from a broken chain. The employee was standing 25 feet from the continuous miner as the CM backed out of #3. A chain, that had been of the miner, got wrapped around the offside spray block chassis and the other end was run over by the miner, putting tension on the chain causing it to break.

Employee was recovering 72" belt structure. Employee dropped to the ground, placing the structure on top of right hand which was also on top of a rock. The employee suffered a laceration to the right ring finger which required stitches. Employee's physician put employee off work until employee can be re-evaluated on 5/15/2018.

The employee was underground preparing to go a belt drive. On the way to the mantrip, they stepped between the track rails into some mud, which caused them to fall injuring the left knee. Employee continued to work until 06-05-18, at which time they underwent arthroscopic surgery to repair a partially torn meniscus.

Employee had just turned the exhaust fans in the shop off, as they turned, the left foot caught a cable holding cap, causing them to fall. As they fell, they extended the left arm to catch themselves, and injured the left shoulder. Employee continued to work unrestricted until 04/24/2018, when employee was taken off work for surgical repair.

Employee bent down and grabbed a bed rail and tossed in out of the roadway and felt pain in lower back.

A roof fall occurred in the Cedar Grove Mains track entry, #104 XC, that was approximately 45 feet long extending into the X-cut, 20 feet wide and 10 feet high.

A roof fall was discovered in the Cedar Grove Mains Intake, #2 Entry, #34 XC. This is not an intake escapeway and ventilation was not disrupted or affected in any way. All approaches to the fall have been dangered off and all persons affected notified.

Employee opened a door in order to move supplies with a forklift. When employee was getting ready to close the door, a rock fell out from the roof and hitting employee on the head causing an injury.

Employee was standing directly against the section power center, looking for an item on top of power center. As ee turned to walk, foot got caught on a communication cable, causing ee to twist ankle and fall to the ground. Employee suffered a broken left ankle and is off work until 11/09/2017.

Employee was picking up a stage loader flight out of a scoop bucket when EE slipped and smashed right ring and little finger between the flight and scoop bucket. This resulted in a fracture to the tip of the right ring finger and sutures in both the right ring and little finger. Employee is awaiting to see a specialist.

Employee was attempting to level the impact bed on the belt discharge, using a track jack and slate bar as a handle. EE was operating the jack bar with one hand, trying to insert the leg pin with the other hand, when EE's hand slipped from the bar and the body of the bar sprang up, striking the employee behind the left ear.

Employee was rolling track rails with a metal slate bar and the bar kicked back striking ee in the R cheek area causing a laceration requiring stitches.

Four (4) men became stuck on the elevator when it stopped working.

Employee was on knees and reached to get a crib block, as ee twisted upper body to place the crib block on the tailpiece, ee felt a pain go down through right leg. The employee continued to work and was later seen by a chiropractor. The pain in leg persisted and ee was seen by a neurosurgeon on 8-11-17; it was during this visit the neurosurgeon put ee off work.

A roof fall in the intake escapeway was discovered at 2:15 PM at 37 X-cut, #3 entry in the Cedar Grove Mains Intake.

Employee was recovering belt structure and it hit employee in the face causing an injury

Employee was attaching a tie to the track using the bucket of a scoop to lift the rail. When the scoop operator raised the rail with the bucket. The rail slipped out of the scoop bucket and fell on foot.

The injured employee was assisting with dropping high voltage cable from hangers to check plugs. While dropping the cable, the injured employee was struck in the head, shoulder and back with the high voltage cable. The employee continued to work until April 5th 2018, at which time, a physician placed them off from work until May 4th 2018.

In the process of transporting men when elevator stopped working.

The employee was assisting others lift a steel frame. During the process the frame slipped out of their hands and fell onto the employee's foot causing a contusion.

Employee was making a splice on a roof bolter cable and struck left thumb with a hammer. Employee went to the hospital and x-rays showed no broken bones. Employee continued to work until 7-18-2017 when they returned to the doctor for a follow up visit. No new x-rays were taken, but the employee was diagnosed with a fractured left thumb based on the initial x-rays.

Continuous miner operator was struck by a rock that fell from the roof that broke the operator's right fibula.

Employee was pulling transmission in a mantrip when the torque converter slid out. After reaching down and sliding the torque converter back onto the transmission, EE straightened up and felt tension or pressure on lower back. The employee continued to work until 06/04/2018, at which time the doctor took EE off work.

While operating roof bolter employee was struck by a glancing blow from a piece of drawrock. Employee finished regular shift, continued to work and did not go to the doctor until 6-14-17. Employee was released full duty. During a follow-up visit on 7-24-17 employee was taken off work pending an MRI. It is at this time the incident became a reportable injury.

Operator of Brookville locomotive violated movement instructions thus hitting mantrip head on. Employee was hospitalized 3 days with back injuries. X-cut 83.

The employee was helping load a roll of conveyor belt. The "D" ring, that ee had placed onto the bar that was inserted through the belt spool, slid off and struck ee in the mouth.

Mine elevator became stuck with workers on the elevator; the elevator first stopped at 7:30am, with maintenance personnel working on the elevator, getting it to move a few times. At 8:30am the elevator was deemed stuck and MSHA was notified.

While installing a new head shaft on a shuttle car, injured was using a sledge hammer to position shaft into place when a piece of metal broke off and went through injured's clothing and lodged beneath the skin in employee's lower left leg. The metal was removed by a doctor and the injured was placed off work for 7 days.

The employee was on knees pulling back on a slate bar EE was using to get slack out of miner cat pads. The bar slipped off and the end of the slate bar stuck EE in the forehead above the left eye.

Employee told boss that they thought they strained their back but continued to work full shift and 8-5 hrs the following day. Told boss to turn in vacation day for the next day. Went to doctor that evening then missed the next day. Returned the following day and has worked since.

An unintentional roof fall occurred in the Alma Seam, North Mains track entry, at the No. 6-7 X-cut. The fall was approximately 70 feet in length, 18 feet wide, and approximately 6 feet high.

Striking steel without gloves

Coupling on rock dust fill hose separated and struck person in side of head.

An unintentional roof fall occurred in the track entry, at #2 xc of the Cedar Grove coal seam.

The employee was using a forklift to load pallets of cinder block into the scoop bucket. Unknown to the operator some blocks had come loose and had fallen in the roadway. As ee trammed the forklift ee ran over one of these cinder blocks and ee felt a pain in back from the jarring action. (Note: Received information that doctor had taken employee off work on 02/15/2017).

Employee was using a knife to trim continuous miner conveyor lead, the knife slipped forward cutting the employee's hand.

The employee was walking up the #2 entry, employee turned towards #1 entry at 16xc on the B-North Section and slipped on a crib block that was buried in the mud, causing employee to twist employee's right knee and fall to the ground. *Note: Employee continued to work until 03/30/2017

The employee was walking along the shield line and as EE turned EE bumped head on a shield. Employee continued to work until the date of 1/10/2017 when EE was put off from work by doctors recommendation.

While walking, injured step on a crib block and it rolled out from underneath ee, causing ee to fall on left hip. Injured continued to work until 12/21/2016 when a follow up doctors appointment revealed a hair line fracture.

An unplanned roof fall has occurred at Cedar Grove Mains #4 entry 19-20 xcut that is approximately 6 1/2 feet high by 70 feet in length.

While employee was standing on the off-side of roof bolter #13, employee was struck in the right side of head and face by a rock that fell from the top which was 9' high. Injured went up to the bolter to ask the bolt crew if they needed any supplies when the rock fell from the top. The rock measured 42"long x 18"wide x 3".

Fall of roof, crosscut, next to track entry.

Employee was walking through the inby set of track doors on 18 tail section. While turning to close the track doors, employee twisted left ankle. Employee returned to work next scheduled work shift.

Employee was changing a fitting on face shield using a socket and ratchet. The socket broke causing employee to strike elbow on the adjacent shield. The employee continued to work until April 20, 2017 at which time a physician applied a cast to immobilize the elbow to aid healing which makes this incident reportable. The employee continues to work under restrictive duties.

While employee was setting bits on the longwall shearer, a rock approximately 4" thick x 3' wide x 7' long rolled out of the top striking the employee.

Employee was traveling from the track entry to the Return entry, as employee cleared the mandoor employee slipped on some ground rock and the air pressure caught the mandoor closing it, the employee was struck in the back by the mandoor handle as the door closed.

A roof fall occurred at this mine on 7-25-2016. It was discovered at 12:16pm on said date that a roof fall approximately 30 foot long, 5 foot high, 18 foot wide, occurred on the Cedar Grove Main Line Track between 34 and 35 crosscuts.

Employee was getting out of forklift to spot up supply cars when EE stepped on a piece of rock causing EE to twist left knee. Employee continued to work after the accident until EE had surgery on 10/20/2016.

While pulling bolts off roof bolter to load tray, injured felt pain in back. The injured was scheduled to see a specialist on June 14, 2016, at which time, this injury became a reportable injury.

An unintentional roof fall occurred on the Mainline Cedar Grove #1 Belt at crosscut #15. The roof fall was approximately 6 feet high, 20 feet wide, and 15 feet long.

The employee was roof bolting and as employee pushed a piece of drill steel into the drill hole employee felt a pain in employee's back.

Employee was sitting inside a diesel man trip traveling to the 1 north elevator at the end of his shift when he man trip he was sitting in was struck by a motor.

An unplanned roof fall occurred on the cedar grove mainline #2 belt at crosscut 106 #4 Entry.

An unplanned roof fall occurred on the Cedar Grove #1 Mainline belt between crosscut numbers 30 and 31.

While tramming the shuttle car to the miner EE hit the panic bar as the shuttle car passed an open break. The brakes locked up on the shuttle car bringing it to an immediate stop. The sudden stop caused EE to jam his right index finger into the shuttle car controls, spraining his finger. He continued to work until 8/20/15 & is off taking physical therapy for his finger.

Injured was on a 6' ladder, on the 2nd or 3rd step, when he reached out to put a nail in a fly pad he lost his balance, fell, landing on his SCSR that was on his belt.

The employee was helping remove the drive chain from the belt drive speed reducer. As he was lifting the chain off the sprocket the slack in the chain dropped to the bottom causing his hand to be pulled into the sprocket, severing the tip of hid left ring finger.

Injured was drilling a hole for a 12' super bolt when the insert for his third piece of steel broke, causing the steel to bend near the chuck. As the steel rotated, it struck the injured on the lower right arm, causing two fractures to the lower right arm.

The employee was building a crib in the #1 entry; after completing the crib he was descending an 8-foot step ladder when the ladder shifted. As the ladder shifted, the employee jumped from the ladder (approx 4-6 feet high) & fell to the ground injuring his neck & right shoulder. (Note: The employee continued to work until 7-16-15, when he underwent surgery for rotator cuff repair)

An unintentional fall of the roof occurred on the B North panel at cross cut #40 in the return air course. The fall located at spad #169 was 80 ft long x 18 ft wide and 18 ft high. A plan to clean up this fall has been submitted to MSHA.

An unintentional fall of the roof occurred on the A North panel. the fall occurred in the #3 entry Crosscut #4 neutral entry spad #145. It was 50 ft long, 18 ft wide and 6 ft high. This area has been cribbed off and dangered off. It will not be cleaned up.

Fell backwards while uncoupling a flat car landing on his hand.

While installing a planetary in the continuous miner, the employee struck his right index finger against the frame of the continuous miner.

This mine experienced an unplanned roof fall in the #1 Entry, 25xc, on the A-North section. The fall is approximately 100' long x 10' high x 18' wide.

While building a stopping on southwest mains 33 crosscut #5 entry, the stopping fell over striking the employee.

An unintentional roof fall occurred at the #14 crosscut between the track and belt entries. The fall measured 6 ft high, 12 ft wide and 30 ft long. This fall will not be cleaned up. It has been cribbed and barricaded off on both ends.

While putting a car back on the track, EE stated he stepped on a small area of ice, causing him to slip. EE then stated he tripped over 2 crib blocks, causing him to fall to the ground. When EE went to the hospital, the doctor stated the EE suffered a sprain and suffered from degenerative conditions. EE started missing work on 4-15-2015.

Injured was in the process of installing a new tail roller assembly on a continuous miner machine when it fell striking the injured on the top of the right foot.

While lifting on airlock doors, EE strained his back. He was injured on Sept. 30, 2014. He worked up until Oct. 7, 2014. The doctor wants to evaluate him Monday Oct. 13, 2014. On the 13th the doctor wants him to have an MRI to see if there's a problem. At that time I'll know if this is a reportable accident. I'm sending this to stay within my 10 days.

The employee was attempting to raise the battery lid of a scoop. As he lifted the lid he lost his grip letting the lid fall, catching his finger between the battery lid and the frame of the scoop.

An unintentional roof fall occurred at the #4 entry #145 cx Alma West Mains, spad #144. The fall was 20 feet long, 18 feet wide, and 12 feet high. A plan was submitted and approved to clean up the fall. No equipment or employees were involved in the fall.

Employee was replacing the top lid cover on a continuous miner when his left ring finger was smashed when it was caught between the lid and a stop.

While pulling a shuttle car to the end of the track, a chain broke, lashing back and hitting employee on the right side of his face, cutting his eye and nose.

An unintentional fall of the roof occurred at the #4 entry #36 CX Cedar Grove main line belt. The fall was 70 ft. long, 18 ft. wide and 5 ft. high. A plan was submitted and approved to clean up the fall.

An unintentional roof fall occurred at the #4 entry #30 cx Cedar Grove mail line #1 belt. The fall was 20 ft long, 19 ft wide and 6 to 8 ft high. A plan was submitted and approved to clean up the fall.

While changing the ram jack on a longwall shield, got finger caught between the ram jack and the pan

While carrying a drill along with another employee the injured employee fell when walking backwards with the load striking his back on the rock bottom. Injured continued to work until being referred to a neurologist, who has him off work as of 10/1/14.

Employee was hanging the miner cable when it fell and struck the employee on top of the head.

EE states while removing belt clamp on slope belt the bottom clamp fell off and struck EE on the right shoulder and left hand causing a severe contusion. Medical treatment obtained 4/22/14 and placed on modified duty 4/23/14. No lost time associated with this injury.

While moving an ATRS extension, it fell on his right foot, cracking his metatarsal bone.

An unintentional roof fall occurred at 25 cx #3 entry, spad #58 on the Cedar Grove 3 South section. The fall was 12 ft. long, 19 ft. wide and 6 to 8 ft. high. A plan was submitted and approved to clean up the fall.

Strained himself while lifting beams onto cribs.

Door switch closer failure

While unloading batteries off of a flat car EE lifted a battery while turning around and strained his knee. On 2/27/14 I was notified by EE that he is going to have surgery on his right knee and it will become a lost time injury.

The employee was walking and tripped and fell extending his left arm to attempt to catch himself and resulting in a strain and sprain of his left shoulder. Employee began missing work on May 22, 2014

Walking to the manbus to get a drink of water. He fell striking the ground.

While lifting a crib block, EE picked up a crib block and heard something pop in his back.

While walking, stepped on a rock, rolling ankle outward.

Injured received an Occupational Hearing award due to a low hearing impairment.

While building overcast wall. EE was laying a 8" block. While bending over to pick up block he was bending and twisting at the same time and strained his back. The injured body parts are from a re-occurence of an old injury.

While building a stopping, a rock fell form the top striking him in the head.

Security officer was retrieving personal items from vehicle. When returning to guard shack tripped over a rubber mat.

EE was operating the tail side of the shearer and crossed out of the 6th shield zone to confirm that the shield was against the top. When EE stepped on the 7th shield the shield advanced and caught EE's leg between the relay bar and the shields pontoon.

An unintentional roof fall occurred at #95CX # 4 entry spad #667 of the Alma north Mines. It was 10' long 15' x wide x 6' high, a clean up plan was submitted and approved.

An unintentional roof fall occurred at #123xc, #4 Entry, of the Cedar Grove Mains. The dimension of the fall is approximately: 100' (L) x 19' (W) x 12' (H). The area has been dangered off, and a clean-up plan will be submitted.

While scattering structure, employee got his left index finger caught between two (2) pieces of structure.

While shoveling belt, his arm got caught against the belt. *EE said he was not going to the doctor, but he went on his own time without notifying me. EE was referred to another doctor, and quit before his doctor appointment on 10/27/12.

An unidentified roof fall was discovered at 26xc, #4 entry, of the Z-right Section Panel. The fall was approximately 40' in length x 18' in width x 8' in height. The fall was immediately reported to the regulatory agencies, via telephone hot line. The fall area was dangered off.

As the employee was attempting to remove the last section of ventilation tubing from the shuttle car, he stepped onto the car, slipped, fell forward, striking the left side of his face against the shuttle car's sideboard.

An unintentional fall of the mine roof was discovered at #12 c.c. #3 entry of the South Mains Belt. The fall was approximately 50' long x 18 ' wide x 5' high. The fall was immediately reported to the regulatory agencies, via telephone hot line. A roof fall clean up plan will be submitted.

Unintentional fall of the mine roof was discovered at #10 crosscut, #5 entry of the Z-right section. Fall was approx 36'l x 18'w x 5' h. Fall was immediately reported by phone to the regulatory agencies and the crew was withdrew from the section. The primary life-line was re-routed and proper procedures were followed before any one was allowed inby the area.

A roof fall occurred in the break between #6 and #7 entries at #22 XC in the Cedar Grove Mains. The size of the fall is approximately 60 long, 18 feet wide, and 5 feet high.

While lifting a set of track doors, EE strained his back.
