MOUNTAINEER POCAHONTAS NO 1 MINE Coal

An unplanned roof fall above anchorage zone occurred at #3 cross cut in the #5 (belt/track) entry on the No. 2 West Mains. No one was injured as a result of the fall, and ventilation was not disrupted. This is an idle mine that is being maintained. All approaches to the fall have been supported and dangered off and the travel way for weekly examinations rerouted.

An unplanned roof fall above anchorage zone occurred at 19 break on the #8 belt/track in the #5 entry. Ventilation was not affected and there were no injuries. This is an idle mine that is being maintained.

An unplanned roof fall was discovered between x-cut 71 & 72. The fall measured 18' wide x 6' length x 4'high. Two bolts were dislodged from the top.

An unplanned roof fall was discovered 1 block inby spad # 5379 in Old No. 1 West Mains. The fall is limited to one intersection and measures approximately 20'W x 20'L x 8-10'T. The fall fell above anchorage.

EE was assisting in helping to shovel the tailpiece. When EE and the other employee lifted the guard to access the tailpiece EE felt a pain in lower back. EE went to the ER where they were taken off work for two weeks.

At approximately 9:20 pm Midnight Shift discovered a roof fall contained to the intersection at spad # 7603. The fall fell above anchorage.

An unplanned roof fall was discovered adjacent to spad # 5303. The fall is limited to one intersection and measures approximately 18.5 W x 19 L x 5-7 T. The fall fell above anchorage.

Employee was walking through a fly-pad in the # 7 Entry, and struck the end of a roofbolt with head, causing a laceration to forehead

EE stepped off of a flat car on left leg, and hyper extended knee.

Employee was walking to shuttle car so as to start hauling coal. EE stepped in a hole in wet muddy mine floor twisting ee left ankle.

AN UNPLANNED ROOF FALL WAS DISCOVERED INBY SPAD # 7073 IN THE #8 ENTRY RIGHT RETURN AIRCOURSE ON WEST SECTION. THE FALL MEASURED APPROX. 90'X20' AND FELL ABOVE ANCHOURAGE.

EE was installing outside bolt and had placed the drill steel against the top. As ee was preparing to begin drilling the hole a piece of the top broke loose and fell down bouncing off the drill pot and struck the employee below right eye causing a laceration. EE was transported outside and to GVMC where ee required 6 stitches to close the laceration.

Injured was getting ready to rock dust # 4 heading; a piece of rock fell from the roof striking ee on the head, knocking ee to the ground. EE walked from # 4 entry to # 2 entry to tell the boss about the accident. They immediately treated ee, and transported ee to the surface.

EE had just completed washing down feeder. EE hung wash down hose back up and was turning to walk away. Their foot slipped on a rock and into a hole and they twisted their knee. EE continued working regular job duties until they sought medical treatment on 7/30/2018 and was placed on restricted duty.

EE was traveling down the elevator at full speed to the bottom of the shaft. When the elevator reached the bottom it came to a sudden stop resulting in EE's knees to buckle and injure EE's left knee. EE finished shift and went to med express on 4/17/2018 and placed on restricted duty.

EE was assisting in setting a belt head. The belt head was raised using a track jack. EE was placing a half header between the timber and crib. EE raised the timber with one hand while sliding the half header between the timber and crib with the other hand. The track jack tipped over causing the belt head to sit down on blocking material pinning EE fingers on both hands.

EE was helping install rub rail back on the miner. EE was holding the rail in place while another employee was striking the rail with a hammer. The rail jumped into place smashing the right ring finger between the rail and miner. Employee was treated and released from GVMC. EE was diagnosed with a fractured tip of the right ring finger and required 4 stitches around fingernail.

EE was on # 14 belt at 6 BK. gathering belt tools to perform a belt splice. As EE was carrying the tools to a ride for transport, EE stepped on a rock and rolled ankle. EE was diagnosed with a sprained ankle.

At 7:45am Outby Foreman discovered a roof fall in #7 Entry West Mains #1 Section. The fall fell above anchorage point and measured 110'x19.5'x8' thick. Cribs/timbers were built in #6, #7 and #8 entries and stopping between #7 and #8 entries. This area was mined mid March 2017.6' glue bolts and 8 - 10' cable bolts were utilized.

EE was lifting 6" I-beam placing on top of another I-beam when EE lost hold & dropped the I-beam, crushing left ring finger between I-beams.

EE was using a Dewalt Hand Grinder to cut down and adapt a top roller when the grinder kicked back and struck EE in upper right thigh (leg). EE received 9 sutures.

EE was clinching end of splice in move belt with hammer when piece of metal flew in his right eye over top of his glasses. EE received sutures in right eye. EE still suffering from blurred vision.

EE was moving metal roof straps so electrician could remove shield on scoop. EE lifted straps onto area light. Straps slid off area light and smashed left pinky finger.

EE was kneeling down beside side dump of feeder while electrician was backing Mac-12 rubber tire personnel carrier towards him. EE did not get out of the way in time and the bumper of the Mac-12 pinned his lower left leg against the feeder. EE sustained fractured tibia in left leg.

EE was operating supply motors traveling inby 10 Head when motors and cars lost traction and began to slide. EE made attempt to stop and then EE jumped off supply motor. EE sustained bruised hip and fracture in left leg.

Service Hoist went into Alarm Mode at approximately 4:30am when hydraulic system cooled due to glitch in heater. Service Hoist was reset and operational at approximately 5:30am. No injuries and no miners trapped.

EE was drilling hole in No 6 face on 1 Unit to install roof bolt and a piece of rock 13"x7"x3"thick fell from mine roof and struck EE on back of left lower arm. EE RTW on 10/20/2014 then went back off on 10/21/2014 for same injury and RTW on 11/3/2014.

Alleged incident occurred while EE was cleaning No. 7 entry with Fairchild scoop. Scoop bumped into rib and coal fell from rib into operator's compartment and EE alleges it jarred him forward hurting his back. EMT examined EE and could not visibly see any indications of coal/rib striking his back. EE finished his shift.

A fuse blew in Service Hoist towards the end of evening shift. No employees were on the Service Hoist. No injuries and no miners trapped. Had to wait for replacement fuse and service hoist was repaired at approximately 5:30am.

EE was installing a wash down hose at #10 Belt Head and when he stepped off Belt he stepped onto a rock and twisted right ankle.

An unintentional roof fall occurred on 1 west mains at approximately 9:42am. Fall occurred @ block 74 from intersection of No.8 to intersection of No.9. Fall measured approximately 80' long by 18' wide by 10' high. Roof Bolter was in No. 8 Face. Plan was submitted and approved to retrieve Roof Bolter.

An unintentional roof fall was found in #2 Entry Return Aircourse & crosscut to #1 Entry one break inby spad2865,on #2 West-Submains.Fall measured approx 7' high 28' wide 10' long.Fall will not be cleaned up.Approaches dangered and cribbed/timbered. Fall does not interrupt ventilation/block escapeways/no injuries reported.Fall is due to weathering of mine roof.

An unintentional roof fall was found in No 2 Entry Return Aircourse, one break inby spad 4709, on No. 2 West-Submains. Fall measured approx 7' high, 19' wide, 10' long. Fall will not be cleaned up. Approaches dangered and cribbed/timbered. The fall does not interrupt ventilation/block escapeways/no injuries reported. Fall is due to weathering of the mine roof.

An unintentional roof fall occurred on 2 Unit at 35 crosscut in the 5 to 6 crosscut and the 6 to 7 crosscut and the intersection of No. 6 entry. The fall was cleaned up and supported with permanent supports to retrieve 4 obstructed MRS machines.

EE was installing roof bolts in 3-left crosscut on 1-Unit when a piece of the coal rib fell, striking his lower left leg. EE was transported to Appalachian Regional Hospital. EE sustained a fractured tibia and fibula in lower left leg.

An unintentional roof fall occurred on 2-Unit in the No. 2 Entry. The unintentional roof fall was approximately 14' wide by 6' high by 10' long. The fall was cleaned up and supported with permanent roof support to retrieve two obstructed MRS machines.

During roof bolting operations on 2-Unit No. 1 Entry, a rock from mine roof slid down ATRS and struck EE on his left arm. EE received 5 Running-Sutures for laceration.

EE was carrying a crib block in number 5 entry and stepped in a dip in mine floor and twisted/sprained right ankle. EE did finish the shift.

EE was attempting to install a metal roof strap in regular bolt pattern when strap flexed and sprung back striking EE on left forearm. EE suffered a laceration and received 7 Sutures.

EE was on board #4 Mac-12 and as the mantrip proceeded down the slope a few hundred feet the Mac-12 lost brakes. The mantrip traveled approximately 950 feet before being driven into the mine wall where it struck steel set supports. EE suffered a fractured left hip.

Employee attempted to kick a rock off continuous miner and injured his right lower leg (calf). Employee sustained a strain/pulled muscle.

While in #5 Entry, employee inadvertently walked into part of a roof strap that was broken and hanging down from mine roof. Employee sustained a laceration to his neck and Left Ear. He received 14 sutures in his neck and 4 sutures in his left ear. Employee returned to work his next scheduled shift on 3/1/2014.

Employee was standing on a bucket while retrieving leaky feeder cable and fell onto side of body injuring his neck.

Employee was handling cribs to place them under a scoop tire and he strained his lower back.

EE was assisting foreman make belt splice, while hammering spindle a piece of metal chip flew off hammer and went into left arm. EE had to have piece of metal surgically removed from left arm.

An unintentional roof fall was found by mine examiner in the intersection of Spad 4346 of old 2 West Mains panel. Approximate dimensions are 18w x 18L x 10h. No ventilation or travelway blocked. All approaches dangered off. Fall will not be cleaned up.

EE was in process of installing roof bolt when bearing plate caught drill guide causing plate to jerk back catching employees left thumb and bending his left thumb backwards (hyper-extension).

EE was walking in entry and he felt a pop in lower left leg (calf). EE finished shift.

EE was assisting another EE to remove metal guarding from belt when one end of the guarding unexpectedly dropped. This caused the metal guarding to fall onto EE left middle finger causing a laceration. EE received stitches due to incident.

EE was lifting a P20 de-watering pump into scoop bucket when he felt pain in his lower back.

Mine Examiner found a fall at Break 16 in No. 9 Entry at spad no. 2858 on West Mains. Fall measured 18'x12'x6'. No travel blocked, no ventilation blocked, and no injury. Fall was dangered off.

EE walked up to roof bolter had his right hand on bolter tray when a small piece of rock fell striking his right index finger.

During the weekly mine examination a roof fall was observed in an intersection located in the old 1-south sub-mains in No. 3 entry, one crosscut inby spad No. 3865. This is a non-active area of the mine. The fall measured approximately 18 feet wide by 40 long and 6 to 8 feet high. No travel blocked, no ventilation blocked and the fall will not be cleaned. Only breakers posts set.

A roof fall was discovered by the mine examiner in the intersection of No. 8 entry approximately 800 feet outby the dumping point at spad No. 2857. No ventilation, travelway or workers were affected by the fall. Fall measured approximately 20'x 20'x 8'high. Fall area cribbed and dangered off. Mine is non-producing.

Roof fall in intersection at spad No. 4372. Approximately 30' long; 20' wide; 6' to 8'.

A re-write of the incident indicated that the employee was jacking a rail supply car on the track and the edge of the car struck the rib area. CT scan in April indicate old rib fracture. Comp.denied claim. Received similar medical treatment/off 48 days from Jan. 2012 through June 2012 (prior to incident). Continue to investigate claim as to work related.

Employee states that he was moving miner cable out of the way, he knelt down to move cable and felt a sharp pain in lower back.

Employee was operating roof bolter when a rock fell hitting drill pot and fragmented flying up into face hitting safety glasses, a small rock fragment became embedded in right side of right eye, was seen at ER and object was removed. no vision problems were reported by employee.

Employee was helping loading rail on flatcar when left middle finger got caught between two rails pinching finger. Was seen at ER 8 sutures and positive fracture in three places on left middle finger.

Employee was walking to power center on section, walked through flypads, and hit a roof matt that was not firmly against roof,resulted in laceration to top part of ear, employee was taken to ER and 9 sutures were placed.

Employee was prying with slate bar, slate bar slipped, causing employee to fall forward putting his body weight on his upper arm, employee did not report accident to magmt. until 5-21-12, employee was seen by a physcian for muscle strain.

Operating a shuttle car and slid into the corner rib and pinned his upper left arm between rib and shuttle car

EE was shoveling what he called heavy mud under beltline, twisted back by a turning motion to unload shovel on belt, he stated back went numb, was seen at ER for back strain.

EE was lifting large rock off of track with slate bar. pain progressively became worse over the next day, was seen at ER for back strain

Employee was using a metal punch to score a notch for drilling through a piece of metal, when he hit the punch a piece of metal came off embedding in left hand

EE was cutting allthread on the collar beam, as he was cutting the second allthread it jumped and landed on his finger hitting the tip of it.

Employee stated that when trying to get out of scoop his left knee popped out of place.

Employee was cutting open a bag of rock dust and cut his finger in the process, was seen at ER, received dermabond (glue) to close wound, note: Employee is currently in hospital for unrelated illness and has not returned to work as of 2-2-12

Employee was beating bearing off of roller assembly on a fletcher RB,A piece of metal flew off striking right hand and becoming embedded under knuckle of index finger, He was seen by a doctor and DR. advised to leave metal in, that it posed no complications.

Employee went to Dr. over sore small finger, physician lanced and drained finger, found a small metal sliver embedded in finger, employee was unaware of injury and stated that he has not been working on anything metal, other then work, no sutures were placed, no lost time.

An unintentional roof fall above anchorage point was found by a fire boss while performing Pre-shift inspections. The fall occurred on the deactivated MMU of 006 of the number 1 west mains, the fall was in the number 12 entry intersection approx. 1 crosscut outby the face, spad number 3511

Unloading crib blocks from track flatcar. Small left hand finger caught between crib blocks. blocks were described as being frozen from being outside, Block slide off of stack striking his left hand and finger, employee was seen by physician and had surgery performed and pin placed in finger

While closing airlock doors the air force door shut on rt hand.

Employee stated that he had just finished last bolt(inside) in middle of cut and accidental swung roof bolter boom toward himself pinning right leg under boom causing contusions to lower back and ankle, x-ray proved no fractures.

A piece of rock fell out of the roof and mashed thumb

Walking through curtain at end of shift, hit bucket of scoop and rolled left ankle.

Unloading supplies on roof bolter when another employee tossed a bundle of plates striking him on the right index finger.

Tractor slid into beam going down slope after being washed down. Employee alleged back pain but felt it was due to farm tractor incident. Medical submitted through insurance. Now alleged as compensation injury. Returned next scheduled shift, worked following week but is presently off work.

Employee was in the process of removing a crib block, used as blocking material, from under the bumper of a continuous mining machine. The crib blocks placed under the stab jack rolled allowing the CM to fall catching the employee's right little finger between the bumper and crib block.

Shuttle Car left the face and tore down a section of ventilation curtain. The CM operator was in the process of repairing the curtain when a second shuttle car arrived in the face and hit the CM operator on the left leg.

Employee stated that a rock fell out between bolts striking him in the head, as per a part 50 audit report was not filed to show 1 day lost time

EE was drilling on shaft floor blew out hole bottom when debris went up under glasses and in right eye.

Employee stated that while using pick hammer to punch hole in oil can, he struck his thumb on left hand. Employee was seen at ER where physicians lanced nail bed to drain blood. Report is 255 days late as per a part 50 audit.

EE was building a stopping and while lifting blocks pulled muscle in left arm. 18 days restricted duty was found as a result of a Part 50 Audit.

Employee was drilling hole for roof bolt when a piece of rock, 6"x12" fell from top striking left hand.

Small piece of rock fell beside roof bolter when employee walked by the machine hitting him on the arm.

Employee was hanging ventilation curtain and cut his knuckle on a jagged edge on a roof strap.

Employee filed and was granted an occupational hearing loss claim on May 9th 2011.

Employee stated that he was walking bent over and struck a bent roof bolt plate causing pain on his right side of head and neck and busing hard hat.

Employee was pushing a coal hauler with a coal hauler, traveled through a hole in the mine floor causing operator to hit his head on the canopy.

Lifting acetylene bottle and felt pain in groin & low back. Continued to work until 6/21/11.

Employee states that while walking through flypads and striking flyboard a nail became impaled in back, he was seen at ER and nail was removed by physician.

Pulling CM cable and felt pain in low back

Carrying a solid cinder block; tripped and fell on arm.

Employee stated that a piece of drill steel was stuck in the drill hole, while attempting to remove the steel it dropped from the roof striking left hand, employee was seen by physician for contusions and abrasions, report is 355 days late as a result of a part 50 audit.

Employee was pulling on CM cable and strained back.

Employee was installing a 10" roof strap; his hand slipped and cut his left middle finger.

Employee was starting to drill a roof bolt hole when a small piece of rock fell striking him on the right thumb.

While working on a battery powered coal hauler, employee stated that his fingers became pinched between bolt and crib block.

Unloading crib block and one smashed left hand. Possible fractured finger.

Alleged that while operating a mantrip his SCSR or hammer caught a timber and drug him off the man trip. Investigative facts do not support what happened.

EE filed claim on 1/25/2011, no percentage has been granted. Person filing this report was unaware this need to be reported without compensation being awarded. EE has filed with State only.

Employee was on his knees working when a oxygen bottle became dislodged from where it was tied up, fell hitting his left ankle. Report is 355 days late as a result of a Part 50 Audit.

EE filed claim 1/14/2011, no percentage has been granted. Person filing this report was unaware this need to be reported without compensation being awarded. EE voluntarily ended his employment with WVMP on 7/28/2011 due to other injury not related to Occupational Pneumoconiosis.

While bolting roof, EE stated that a rock fell from top striking right hand causing a fracture of small finger on right hand.

Stretching out data line in Belt/Track entry stepped in hole and overextended knee. Did not start missing until 2/28/11

The employee was sitting in a rubber tire mantrip with one leg on the ground. The operator advanced the mantrip forward causing a twist in the lower back. The injury was not reported until 12/13/10.

Employee was pushing a coal hauler with the scoop bucket and the bucket slipped causing a quick jerk slamming the operator into the canopy.

Opening an airlock door and it closed, hitting the employee on the ear.

Employee was holding a crib block with his thumb on top of the crib block placing it under the head of a CM. When the head was lowered his thumb was mashed.

Employee was completing last row of bolts in face, drilling outside bolt hole, 38" from last row and 37" from face, when a piece of roof rock fell and hit his right thigh causing a laceration. Stitches were required.

Employees was working with scoop operator hanging check curtains on a curtain board installed to the roof. A piece of rib/rock fell striking him on the leg and foot.

Lifting a charger for a 2-man A.L. Lee personnel carrier and strained back.

Moving a water pump and bumped middle finger on pump. Very small scrap that became infected. Employee has past history of blood/heart concerns. Low blood count may have caused infection.

The employees was helping install new cat pads on the CM when the chain hook came loose striking the employee on both hands.

Climbing down from a ledge in the mine floor developed for an undercast and twisted his ankle.

Building Brattice & noticed sharp pain in lower left abdomen. Did not start missing work until 6/4/10.

Roadway/coal rib material had been pushed into the face during the clean-up cycle. The CM operator started loading the material with the CM and picked up a piece of metal, located in the material, through the conveyor chain that flew out of the chain and hit him in the head.

Mashed finger with hammer while beating a bit out of the drill steel

Picking up a board off the roof bolting machine and felt a pain in rt shoulder. Continued to work until 1/14/11. Not reportable at the time of injury.

Employee was making a conveyor belt splice and while skiving the belt with a razor sharp knife he sliced the end of his left index finger.

Employee was in the process of putting a pin into the scoop tonge and got his middle finger caught resulting in stitches to close the wond.

Employee was removing a U-Joint from a scoop with a hammer. While hammering a piece of metal, neddle bearing, embeded into his left forearm.

Employee fell in the mantrip and fractured his pinky finger on his right hand

Employee only worked for 3 shifts and stated that he got his foot tangled in the CM cable and twisted his knee. Operated was not aware of him being off due to the injury until 7/20/09.

Employee was loading under the CM and a piece of rock from the CM conveyor chain hit him in the right eye. Doctor removed material from eye.

Employee shoveled coal ribs through the shift and stated that his right knee hurt at the end of shift. No single event occurred. Past history of knee problems with a previous knee operation.

While attempting to replace one side of a belt scraper he caught his small finger between the scraper and tail piece. Belt not running;during belt move.

The employee was loading supplies (fly boards) from the scoop to the roofbolter, with the bucket down, when the scoop traveled forward and pinned his foot between the bucket and the roof bolter.

While using a pry bar to aid in the installation of a pick breaker in a coal feeder the support chain broke and pinned the employee's hand between the pry bar and top of the feeder.

The employee was cleaning the deck of the Shuttle Car when the scoop came through the check curtain and caught him between the the scoop and shuttle car.

The employee was acending a belt crossover ladder when his foot slipped and wedged between the belt/belt structure. The belt was not in operation and as he fell foreward his foot did not release. This resulted in a sprained ankle with a simple leg fracture just above the ankle.

The employee was in the process of blocking under the undercarriage of a raised 14/15 CM continuous miner to replace the crawler pads. As he stuck his hand under the undercarriage to retrieve a wooden block the previously installed wooden crib under the cutter head shifted allowing the machine to fall catching his hand between the wearing strip and the mine floor.

Pulling on Continuous Miner cable to clear the roadway and felt a pain in low back.

While roof bolting the drill steel fell out of the hole, bounced off the machine, and hit the employee in the head, above the eye brow. Required 3 stiches.

While lifting the plugger onto the drill steel, felt a muscle twinge in lower back. Mentioned this to Supervisor and went home at end of shift. Employee woke up the next morning and felt pain in lower back.

While getting ready to load holes drilled on bench, slipped and fell on left hand while trying to support himself when slipping.

Employee was putting away a plugger (rock drill) at the shaft bench when he slipped & fell catching himself causing a twisting of the torso which resulted in a sprain to the lower back. Employee did not seek medical attention until Oct 2, 07 and did not miss any time away from work.

After finishing drilling mucker anker located mid way down the air shaft approx. 35', co-worker inadvertently turned the jack leg air on striking deckman's left ear and head area, resulting in a laceration requiring 12 stitches to his right ear.

While plugging hole on the bottom of airshaft, ee using an air/water jack hammer drill. Was splashed in the eyes with damp shavings produced from drilling. He was wearing safety glasses at the time.
