Thur - 1/21/16- 4:30 PM - employee left work at normal time 4:30 pm. EE said they went home and took a shower. After the shower EE's eye began to hurt. EE was off work for the following 4 days (1/22/16 thru 1/25/16). On 1/26/16 EE began work at 6:00 AM and came to report eye (5 day late) injury at the office approx 9:00 AM and EE was sent to clinic.

EMPLOYEE WAS SPLITTING STONE NEXT TO A STONE PILE WHEN A LOOSE ROCK FELL DOWN THE PILE AND HIT HIS RIGHT FOOT CAUSING A CRUSH INJURY
EE was splitting rock with a hammer & chisel. He bent over to turn a rock on it's side and he felt a pop in his lower back.
ON 04/27/2015 EMPLOYEE WAS SWINGING SLEDGEHAMMER TO SPLIT ROCK. HE MISSED THE ROCK AND CAUGHT HIS HAND BETWEEN ROCK AND HAMMER AND LACERATED MIDDLE FINGER ON LEFT HAND ACROSS THE KNUCKLE.

EMPLOYEE WAS LOCKING TRASH BIN IN PLACE. HE HIT HIS HEAD ON HANDLE WHEN HE BENT OVER.
Employee was picking up a rock when he felt a pain in his back that extended into his leg. He just wanted to go home and rest for a while, but did finally agree to go to the Dr.
On 3/6/14 IE went to another EE and said his left thumb seemed to ache but 'only' when he drove the forklift. Other EE told him to stay off the forklift and asked if He wanted to go to Dr. IE replied 'no'. On 3/12/14 IE told other EE he wanted to go to the Dr. as his thumb seemed to be aching more. IE said he had con't to drive the forklift.

THE EMPLOYEE WAS BREAKING DOWN SOME WOODEN PALLETS WHEN A PIECE OF WOOD HIT HIM ON HIS NOSE. HE WAS WEARING SAFETY GLASSES WHICH PREVENTED THE INJURY FROM BEING WORSE.
EMPLOYEE WAS FABRICATION STONE. WHILE REMOVING STONE FROM THE AREA, HE SLIPPED AND FELL FACE FIRST ONTO THE ROCK HE WAS HOLDING AND HIT HIS NOSE AND MOUTH.
Injury occurred on Mon. 11/18/2013. It was not reported until Fri. 11/22/2013. EE explained that he was trying to pull on a stone. His hands slipped and he fell landing on a rock.
Injury occurred on Thur. 08/29/2013. It was not reported until Tue. 09/03/2013, at which time employee was sent to the doctor. On 08/29 employee was swinging the large hammer to break a large stone. When he hit the stone some rock fragments flew off and hit his left hand. He said he was wearing only one glove.
EE was splitting stone. He stood up the next piece of stone he was going to split. The stone made some movement, slipped and smashed his 2 fingers against another stone.

Two employees were lifting a rock to place it on a pallet. When they placed the stone on the pallet, EE said he felt a pain in his lower back.
Employee was moving a rock when he dropped it. The rock hit his leg just above the right ankle, slid down and pinned the ankle against another stone.
Picked up rock to move from pallet to conveyor. The rock broke into 2 pieces. One of the pieces fell onto the right ring finger and smashed it causing the finger to need sutures.
05-24-12 Employee was moving stone and felt a pain in his back. He did not report and continued working thru the day. 05-25-12 Employee worked thru the day and did not report. 05-30-12 Employee came to work and reported injury from 05/24/12 and requested to go to doctor. He was immediately sent to doctor and then went home for the day.

EE was finishing stone--He was wearing safety glasses and a full face mask. A hot chip landed on his face. He removed the mask and glasses to wipe away the chip and accidently wiped it into his eye.
Injury was lifting a rock up on its side so he could split it and he said he felt something pull in his side. He said it happened about 2:00 pm but it did not hurt very bad so he did not report it immediately. About 3:00pm he went and told his supervisor and was sent to the Doctor immediately.

EE was pushing rocks forward on the conveyor belt leading to the guillotine. As he pushed, one of the rocks twisted and fell off the conveyor onto his foot. He was wearing his foot guard.
Employee said he was loading the conveyor belt that leads to the guillotine. He bent over to pick up a rock and felt a pulling in his stomach. He was wearing his back belt.
Operating guillotine--moved rock into guillotine, pulled the table lever which pushed the rock up and smashed his fingers.
