Employee was using a pipe wrench to install a hydraulic line on a rock breaker. The wrench slipped striking the employee in a front tooth.

Employee was attempting to clear clay grates with a steel bar. When a section of material broke loose, the employee's finger was pinched between the steel bar and steel clay grate. This required 6 sutures to close the wound.

Employee was driving a bearing out of a loader frame with shop bar. When bearing came out it ee hit wrist on the bore causing laceration

Employee grabbed a hammer drill box from the back of a service truck at the end of his shift. As he lifted the box, he turned and the box struck his left knee. The employee continued working for two weeks but noticed some discomfort in his knee when he would bend it a certain way. He sought medical treatment for the first time on 9/22/15.

The employee was using a hammer to tighten lock down wedges on a cone crusher, when he missed the wedge and smashed his finger between the hammer handle and the wedge.

EE WAS ATTEMPTING TO TIGHTEN SIDE TENSION BOLTS ON A SCALPING SCREEN WHEN THE WRENCH HE WAS USING SLIPPED AND STRUCK HIS CHIN RESULTING IN FOUR STITCHES.

EMPLOYEE WAS CUTTING A PIECE OF RUBBER WITH A UTILITY KNIFE WHEN THE KNIFE SLIPPED AND CUT HIS THUMB, REQUIRING A TRIP TO THE HOSPITAL WHICH REQUIRED 4 STITCHES.

TWO EMPLOYEES WERE MOVING A 30" CONVEYOR FROM THE STORAGE YARD WHEN A CHAIN DISLODGED AND THE CONVEYOR FELL 3', STRIKING ONE EMPLOYEE ON THE LEFT LEG AND ANKLE. THE EMPLOYEE SUFFERED SCRAPES O N HIS LEFT LEG AND A FRACTURE TO HIS LEFT ANKLE.

EE WAS REMOVING A TENSION WEDGE ASSEMBLY FROM THE SIDERALL OF A MATERIAL SHAKER SCREEN. THE TENSION WEDGE DISLODGED AND STRUCK THE EE OVER THE RIGHT EYE. FOUR STITCHES WERE USED TO CLOSE THE C UT.
