A roof fall occurred in the 7A return air course at station number 1655. The fall was approximately 25 feet long x 18 feet wide x 7 feet high.
The man was walking down the entry when he struck his ear against a metal roof strap lacerating his ear. Report filed for Medical Treatment Only ( 1 suture)
The man was stepping off a belt crossover when he lost his footing and twisted his ankle.
The man was riding on the mantrip toward the surface at the end of the shift. While making a turn, the operator turned the mantrip close to the rib causing the man's arm to be pinched between the rib and side of the mantrip causing a contusion.

The man was riding on a personnel carrier as a passenger. As they approached a run through check, the man raised his foot to deflect the check. The man's foot contacted the roof causing his leg to jam and twist his knee.
The man was hanging a power cable. The man assisting him was sitting on a personal carrier and inadvertently struck the tram pedal and the carrier moved. The man's rt. leg and lt. hand were caught between the carrier and rib. On 10/17/10 it was determined there was a fracture of the lt. hand. Report filed for Medical Treatment Only.
A roof fall occurred at survey station 2069 which is adjacent to B2 belt drive. The fall was approximately 20 feet long x 20 feet wide x 9 feet high.
The man had a coal chain laying on the mine floor and was installing a new flight. While hammering a pin throught the flight, a piece of coal flew up and into the man's eye and scratched the eye.
The man was conducting an examination at the face. As he turned around he struck his ear against a roof strap.

The man was moving belt structure when he slipped on wet bottom and felt a pain in his left groin area.
The man was pulling on a belt roller that was stuck in mud. While pulling the roller, it broke free, causing the man to jerk. The man felt a pain in his right groin. The man continued to work until 9/14/06, at which time he reported off.
The man was exiting the scoop tractor when he tripped and fell to the bottom striking his left elbow and lacerating it.
The man was bending a roof bolt on the side of the roof bolter machine when the roof bolt slipped causing the man to lose his balance and strike his forehead on the machine. Report filed for medical treatment only.
The man was walking when he struck his left upper back against a roof bolt. He continued to work until 5/10/06 at which time he reported off work.
On 4-17-06, the man reported that on 4-12-06 he felt a pain in his lower back while lifting belt structure.
Employee stated the bridge operator bumped the tail of the low belt while backing up. The low belt framework moved and the framework struck his elbow.
While lifting the top plate off of stabilizer jack on continous miner, the plate slipped and caught the man's finger between the plate and swivel shaft that is attached to the top plate.
While lifting a heavy iron bar to hang a roll of conveyor belting, employee felt sprain to middle of lower back. He went for treatment 2-2-06, received medication, returned 2-3-06.

EMPLOYEE WAS USING ROCK BAR TO REMOVE FROZEN BOTTOM ROLLER LEANING AGAINST BOTTOM ROLLER STRUCTURE. STATES HE FELT SHARP PAIN IN LEFT RIB AREA. SOUGHT TREATMENT ON 10/26/05.
EE WALKING IN FACE OF #5 ENTRY. WEST MAINS WHEN HE SLIPPED ON WET MUDDY SIDE OF PITCH AND FELL ON SELF RESCUER.
While riding out of mine from daylite shift mantrix was backing up when operator foot slipped off brake. Mantrip struck rib. Two days later employee said he felt dizzy wen to doctor. He stated he had sore neck from incident 2 days prior.
While moving a length of miner cable, employee felt sprain to left shoulder.
While operating roof bolter employee dropped drill boom on right knee causing large bruise with contusion.
WHILE REACHING BEHIND ROOF BOLTING MACHINE TO MOVE TRAILING CABLE. HIS LEFT HAND WAS PINCHED BETWEEN BOLTING MACHINE AND RIB. HE RECIEVED INJURY REQUIRING 4 STITCHES
While carrying can of hydraulic oil handle on can broke throwing employee of balance, causing him to injure back.
When using hand tools employee experiences pain in right wrist and fingers. Diagnosed with carpal tunnel syndrome.
EMPLOYEE STEPPED OFF THE BOTTOM STEP OF THE LADDER ON THE FRONT END LOADER. AS HIS RT. FOOT HIT THE GROUND IT TURNED, ROLLING TO THE SIDE & TWISTING HIS RT. ANKLE. INJURY HAS CAUSED LOST TIME & MEDICAL TREATMENT. EE SUSTAINED TORN LIGAMENTS.

EE WAS TRYING TO REMOVE A PUMP FROM A POND WITH ANOTHER EE OPERATING A BACKHOE. THE BACKHOE OPERATOR HAD THE MACHINE IDLED UP AND COULD NOT PICK UP THE PUMP. INJURED MAN WAS NEAR THE BUCKET OF THE BACKHOE WHEN THE OPERATOR BUMPED A LEVER AND THE PUMP HIT INJURED MAN'S HAND SMASHING IT BETWEEN THE PUMP AND BACKHOE BUCKET.

WHILE TRAMMING GOLF-CART MANTRIP INTO SECTION CART BECAME STUCK. EE WAS PUSHING VEHICLE WHEN HE FELT PAIN IN LOWER BACK. EE FINISHED SHIFT, WORKED THE NEXT DAY THEN WENT TO CHIROPRACTOR AND ST ARTED MISSING TIME ON 10-20-99.
WHILE USING HYDRAULIC DRILL TO DRILL HOLES IN BOTTOM TO ANCHOR BELT TAIL, DRILL STEEL BECAME CAUGHT BEFORE EMPLOYEE COULD RELEASE FINGER PRESSURE DRILL HANDLE ROTATED STRIKING EMPLOYEE ON RIGH T CALF CAUSING LARGE BRUISE.
EE WAS CHECKING PUMP WHEN HE SLIPPED ON HIS RESCUER AND 1 CRACKED RIBS ON LEFT SIDE. BOTTOM CONDITIONS IN THIS AREA ARE PITCHED AND WET.

OPERATOR GOT ON SCOOP AND TRAMMED IN REVERSE. HE TURNED IS HEAD AND HIS HARD HAT GOT CAUGHT BETWEEN THE ROOF AND FRAME OF SCOOP. HIS HARD HAT CAME OFF ALLOWING HIS HEAD TO HIT THE SCOOP FRAME CUTTING THE BACK OF HIS HEAD. (4 SUTURES)
