EMPLOYEEWAS HELPING TO RETRIEVE BROKEN LAGGING FOR HAND PULLEY PLANT OPERATOR. HIS HAND WENT THROUGH BETWEEN BELTS WHEN HE REMOVED HIS GLOVE IT HAD STRIPPED HIS LEFT LITTLE FINGER. THEY AMPUTATED LEFT LITTLE FINGER AT HOSPITAL.

EE WAS CHANGING OIL ON ENGINE OF CAT 980F. HE WAS STANDING ON BATTERY BOX ON BACK OF MACHINE. HIS FOOT SLIPPED OFF BATTERY BOX AND HE FELL ABOUT 3' TO GROUND, LANDING ONHIS LEFT ARM, BREAKING HIS LEFT WRIST.

EE WAS CHANGING HOSE ON ACTALENE TANK THAT WAS MOUNTED BEHIND A PORTABLE WELDER. HE TURNED GAS OFF BUT THE VALVE DID NOT CLOSE ALL THE WAY OFF. THE WELDER EXHAUST SET FIRE TO ACTALENE REQULATO R WHERE HIS HAND WAS RESULTING IN FIRST, SECOND, AND THIRD DEGREE BURNS ON HIS LEFT WRIST AND HAND.

EMPLOYEE HAD JUST CUT 3 FEET OFF A 20 FOOT PIECE OF 16" I BEAM. THEY LIFTED THE I BEAM WITH A LOADER ABOUT THREE FEET OFF THE GROUND. THE THREE FOOT END HAD NOT BEEN CUT COMPLETLY OFF. EE WAS STANDING ON THE SIDE OF THE LONG END OF THE I-BEAM. WHEN THE THREE FOOT END FELL OFF THE I-BEAM WAS OFF BALANCE AND ONE END CAME DOWN ON HIS TOE.
