First aid was administered at the time of the injury.

Employee was on Dozer's catwalk in order to clean the back window. As he moved toward the right side of cab, he stepped onto the track, then slipped from the track to the ground. He stated he was using three points of contact and there was no material on the track. He drove the CAT dozer over to another Dozer working in the pit and received first aid. Taken in for Medical Care.

Employee was walking on the deck and water tanks to the rear of the drill to replace the air filters for the engine and compression slipped when on the water tank and twisted ankle on the deck of the drill.

While changing a 637 Scraper Tire, the employee lost his grip on the tire flange that was positioned against the tire and on a 8" X 10" X 48" long block. The tire flange drop onto the employee's left foot. The caregiver diagnosed a foot fracture.

Getting onto a dozer ee neck popped causing instant tingling in extremities. On 8/19 saw doctor and was given a release to return to work with no restrictions. On 9/2 MRI showed cervical stenosis but uncertain to cause or if work related. EE has remained home due to the uncertainty, Reporting at this time, does not fit any category and may not be work related.

EE was closing cab door on fuel truck when injury occurred. A witness stated EE was standing on cab side step; when he shifted his body to close door his foot slipped, causing him to fall to frozen ground. EE may have slipped due to ice &/or bentonite residue on step. Med. attn. was at the clinic where x-rays showed a hairline fracture. No work restrictions & complete shifts.
Mechanic was working on a wiring problem on the front engine of a 637 Scraper. A strong gust of wind blew the engine hood down on the mechanic, who was standing in front of the equipment cab. The hood pushed the mechanic's face onto the small side door cutting his nose. The injury was treated @ clinic, stitches were used to repair the cut. No work restrictions or lost time.
As EE was lowering pickup, jack handle slipped out of their hand hitting EE in the safety glasses, cutting the left side of their nose. On 01/18/2010 this moved from a First Aid to a Reportable due to EE seeing a Specialist due to Cartilage Damage to the nose.

The operator was helping mechanic repair a leaking hydraulic line. EE was building a more stable area to stand. While placing the building materials, he caught his left index finger between the rocks. Care for the injury was needed by the local clinic. Stitches & an X-ray, showing a small fracture were the medical care received. No work restrictions. No lost time.

While driving along a flat stretch of haul road, the operator lost control of the haul truck. The haul road was wet due to light snowfall. The truck skidded, the driver overcorrected and ran the truck up onto the dryer prairie beside the road. The trucks momentum caused the right tires to align and lay the truck on its right side.

Employee has been employed as a truck driver since 1998. He was watering haul road going into Pit 915 on BPM Minerals haul road. He had watered into the pit, turned around & was leaving the pit with a half load of water when he came around a corner & the load shifted on the water truck & the truck & water tank tipped on the driver side.

INJURED TWISTED HIS BACK AFTER JUMPING ACROSS A NARROW BUT STEEP GULLY. WHILE SURVYING THE TOPGRAPHIL SURFACE FOR A NEW PIT. THE INJURY WAS FURTHER AGGRAVATED AS HE RODE ACROSS RUGGED TERRAIN IN U-WHEEL DRIVE.
LACERATION LEFT PALM ON SHARP FAN SHROUD.
ACCIDEHT OCCURRED AS AUGER-DRILLNG RIG WAS TRIPPING AUGERS OUT OF DRILL HOLE, WHEN WIND BLEW DIRT OFF AUGER DRILL STEM INTO EYE. INJURED WAS WEARING SAFETY GLASSES.
