Cutter AGG driver (NOT Heidelberg employee) proceeded to the pit loading area and exited the truck to correct an issue with the gate locking system. While attempting to engage the gate latch manually, the latch moved unexpectedly, causing the gate to slam shut and crush the end of the driver's middle finger. The driver immediately left the facility to seek medical attention.

IE and a Peterson CAT Mechanic were torquing bolts on a 980G at the wheel ends. On the last bolt when EE went to torque down there was a miss communication and IE had IE's middle finger on IE's right hand in the way and it got smashed on the downward movement of the torque wrench.
Employee was working in the de watering screen and when they went to lift up a water sprayer it caught on floor level and an abrupt stop happened. They felt no injury but later that night around 7pm they started to have some pain.
Employee was using an air chisel to remove a broken bolt and a piece of shrapnel came out from the area of work and got lodged into the worker's left forearm about an inch deep. The air tool was inspected by a service company and it was determined that debris did not come out of the tool.

Two employees were replacing return rollers on the #3 pit conveyor. When dropping one end of the roller in place an employee felt themselves overextend left arm causing pain in shoulder area. Employee worked the remainder of the shift without further incident. Employee elected self care until decided to be seen at a clinic 03/22/2021 and received physical therapy.

Employee was replacing a return roller with another employee. The injured employee placed his end of the return roller in the bracket the other employee did also but his end did not fall into place instead the return roller fell to the ground pinching the injured employees finger in between the return roller bracket and the return roller. The tip of the finger was amputated.

EE hit a d-7 rail link with a 5lb hammer this caused a piece of shrapnel to fly and lodge under injured EE's skin at the right temple. The piece of metal was 1/4-1/2 inch in length
Employee was attemping to latch down the hood on a motor grader, While doing so the engine fan caught the sleeve on his coverall pulling his forearm into the fan. He recieved 49 stitches 9 of them internal and the rest external

The employee was replacing bolts on the cone screen wear plate. He was using a 1/2" ratchet with extension lying on the catwalk to access the bolt on the bottom side. The socket slipped off the bolt and the ratchet handle hit him in the nose resulting in a contusion.

EE was installing a hood lift assembly on the 988G loader. After connecting the pin & spring clip to the cylinder end of the assembly & the hood, he proceeded to place the clip & pin on the rear assembly end. Pin would not go through. EE attempted to align the pin by activating the hood to relieve pressure. The hood raised, the pin popped out & his hand was caught in the machine.
