On 10/31/2018 employee claims another employee jumped up on back while ee was walking through the plant on 10/25/2018. Employee did not report incident until 10/31/18 (6 Days Later) at which time EE was sent for evaluation & FA. This incident is listed as "Horseplay". Employee claims back is not better. EE has been evaluated again and placed on restrictions on 11/15/18.
Steel plate released from crusher due to stored energy striking employee in the right elbow.

Employee was standing on a six foot ladder. He over extended to reach out and fell over hurting his left arm and wrist.

Employee was using the man lift. Employee mis-stepped with his right foot and fell backwards toward the ground. His lanyard was still attached to the manlift and kept his upper body from falling all the way to the ground. However, his right foot and leg were on the ground and his left leg was still on the channel iron. At this time employee felt pain in his left knee.

EE WAS DISMOUNTING FROM THE FRONT END LOADER WHEN HE STEPPED ON A ROCK TWISTING HIS ANKLE. THIS RESULTED IN A SPRAINER LEFT ANKLE. HE MISSED THE BOTTOM STEP AS HE WAS DISMOUNTING.

VICTIM CLIMBED ONTO PIPE TO CLEAR ACTERIAL FORM THE FEED CHUTE. VICTIM SLIPPED AND FELL APPROX. 9 1/2 FT TO THE FLOOR, LANDING ON THIS SIDE. VICTIM RETURNED TO WORK FOR HIS NEXT SHIFT, HOWEVER ON 9-12-01. HE REPORTED THAT HE COULD NOT COME TO WORK AND WAS INSTRUCTED TO SEE A DAY.
THE 50" RAYMOND MILL WAS BEING TAKEN APART TO REPLACE THE MILL BASE. THE 5 ROLLER JOURNALS HAD BEEN REMOVED AND SET ON THE FLOOR. EE WALKED BY AND BUMPED INTO ONE JOURNAL CAUSING IT TO FALL OV ER BEFORE HE COULD MOVE OUT OF THE WAY. THE JOURNAL FELL ON HIS LEFT FOOT SMASHING HIS STELL TOE DOWN FRACTURING ONE OF HIS TOES.
