County Road B Quarry Metal/Non-Metal

Rock was wedged in the conveyor belt. The on/off switch was bumped to try to free it. Did not say "go", EE said "No". When the conveyor bumped forward the rock bar was in the tail pully resulting in foreman being struck in the chest with the bar throwing them 6-7' in air and landing on the ramp.

Miner was approaching jobsite 3 hours late (10:00am) and just about to start on tasks, and they said they had back pain and needed to go to the office to go to the Dr. Never worked or did anything.

Service Technician was performing maintenance on an impact crusher, rotating the impact blow bars. While handling one of the bars EE lost grip and it slipped out of EE's hand. A sharp edge on the blow bar cut the palm of EE's hand, requiring 5 stitches to close the wound. It was a very hot day and EE decided not to wear protective gloves.

Employee was opening a piece of limestone using a 12 lb hammer & chisel. After striking the chisel/stone, employee saw blood on their shirt and realized they had a puncture wound to lower left quadrant of the stomach area. Foreign Body left in puncture wound

MINER WAS BENDING OVER TO LIFT A PIECE OF LIMESTONE APPROX 30 LBS WHEN EE FELT A SUDDEN SEVER PAIN IN EE's BACK AND SIDE AREA

Miner was bending over using a Hammer & Chisel to trim/open a piece of limestone. As EE was striking the chisel, a piece of the chisel flew into EE's lower leg leaving a FB inside the puncture wound.

Miner was bending over toward ground and opening/trimming a piece of limestone. As EE was striking the chisel, EE felt something hit EE's Right knee area causing pain. Possible piece of chisel broke loose causing a puncture/laceration and FB remained embedded

Miner was sliding/pulling a piece of limestone on cut belt to trim it, and ee had fingers below the stone as ee was maneuvering it into correct position to be cut, and the stone broke from a weak spot and it fell onto Left pinky finger causing contusion.

Miner was feeding the saw table with a block of stone. As ee lifted and set the stone onto the table ee caught the edge of pinky finger against the metal table and the force of the stone causing rupture to finger

Miner was doing handstack to the conveyor. As they were placing a piece of limestone onto the conveyor table, miner must have misjudged the height, smashing the stone in their hand (pinky finger) against the conveyor table.

Miner was banding a pallet of limestone when they lacerated their thumb when adjusting the bander.

Miner was operating a Caterpillar 988 Front End Loader driving between stockpile & crusher. Quarry roads and floor are maintained by him to his liking so they are kept in quite decent shape. Possibly was operating too fast for conditions causing bouncing and/or jarring causing a low back injury.

Miner was opening a piece of stone with a hammer & chisel-a seam broke loose on the bottom of stone and broke off falling approx. 2-3' and landing on his foot. Right Foot contusion and closed fracture to Right Big Toe

Miner was walking around near work site area and stated he stripped over a piece of stone - left knee pain/strain.

Miner was cutting limestone on a Hydra-Split machine. Between cuts, he lifted the extraction plate with his right hand & used his left hand to clear debris out from area underneath. The plate slipped out of his right hand, and fell landing onto his left hand/fingers which were still below in the area of the stone chip & debris causing crush injury to left index finger.

Miner reported on 3/24/15 that he had been seen in a clinic on 1/26/15 due to having pain in the groin area since Mid December. He was working throughout winter as weather would permit, while being on partial layoff. He was informed he had a hernia. He remained working on & off lifting no more than 10 lbs.

Miner was lifting a piece of limestone 4-6" thick weighing approx 100 lbs with the assistance of another miner. He continued working and later stated he didn't feel right in the groin area.

Miner is a quarry Block Worker. He was ground stacking pallets when a piece of limestone slipped from his hands crushing his fourth left finger

Bending & Lifting - low back pain

Minor was using a hammer & chisel to split limestone when he struck his middle finger on the right hand smashing it.

Miner was carring a piece of limestone to be transferred onto a pallet, when he twisted wrong to set it in place.

While stacking a piece of quarry limestone on a pallet, miner lost his footing and slipped on the uneven quarry floor. He then dropped the stone on his left leg just above the knee and twisted his left leg.

Minor was transferring limestone from the ground & then stacking it on pallets when he later noticed his back was hurting. No significant injury at the time just continuous bending, lifting, & twisting throught the day.

EE WAS DRIVING THE WATER TRUCK UP AN INCLINE. TRUCK STOPPED RUNNING, THEN STARTED TO ROLL BACKWARDS HITTING A WALL. EE HEAD SLAMMED INTO THE BACK WINDOW RESULTING IN HEAD AND NECK PAIN.

EE CUT HIS RT. FOREARM ON A PIECE OF METAL THAT HAD BECOME JAGGED FROM CONTINUALLY BEING HIT BY STONE.

SMASHED LT. RING FINGER BETWEEN 2 PIECES OF STONE.

A SMALL PIECE OF STONE THAT WAS BEING SPLIT HIT EMPLOYEE'S EYEBROW.

EE WAS LIFTING PIECES OF STONE FROM A PALLET AND PUTTING THE PIECES ON A CONVEYOR BELT TO BE CUT. EE FINISHED THE WORK DAY. THE NEXT DAY HE SAID HIS BACK WAS SORE & WENT TO CHIROPRACTOR. HE SH OULD NOT HAVE LIFTED PIECES OF STONE THAT WERE TOO HEAVY. A FORKLIFT IS AVAILABLE AT ALL TIMES.

EMPLOYEE WAS TAKING PIECES OF STONE (APPROX 20 LBS) EACH OFF A PALLET AND PLACING ON THE CONVEYER BELT WHEN HE PINCHED HIS FINGER BETWEEN ANOTHER PIECE.

EMPLOYEE WAS CUTTING A PIECE OF STONE WITH HYDRA SPLIT. AS HE PUSHED A PIECE INTO THE MACHINE IT GOT CAUGHT ON THE ROLLERS AND HE JAMMED HIS ARM.

EMPLOYEE WAS STACKING STONE ON PALLETS WHEN HE CUT HIS LEG ON A PIECE OF STONE THAT WAS ALREADY ON A PALLET. EMPLOYEES ARE ENCOURAGED TO WEAR LONG PANTS.

EE WAS STACKING CUT STONE ON A PALLET WHILE WALKING BETWEEN TWO PALLETS HE CUT HIS LEG ON A PIECE THAT WAS ALREADY STACKED ON A PALLET.

EE WAS CUTTING A PIECE OF STONE. AS HE PUSHED A PIECE INTO MACHINE HIS HAND SLIPPED OFF STONE & CAUGHT & CUT HIS LT. FOREARM. EE WAS TOLD TO MAKE SURE HE HAS A GOOD GRIP ON STONE.

EMPLOYEE WAS TRYING TO SPLIT OPEN A PIECE OF STONE WITH A HAMMER AND WEDGE WHEN HE HIT THE STONE A PIECE BROKE OFF AND HIS NATURAL INSTINCTWAS TO TRY AND CATCH IT. HE SHOULD HAVE LET THE PIECE FALL TO THE GROUND OF LET THE LOADER SPLITTHE STONE.

EMPLOYEE WAS REMOVING STONE FROM CONVEYOR & STACKING ON PALLET. HE CUT ARM ON A PIECE THAT WAS STICKING OFF PALLETS. EMPLOYEES WERE REINFORCED TO MAKE SURE STONE IS STACKED SQUARELY ON PALLETS SO NOT TO CATCH ANY SHARP EDGES & ALSO MAKE SURE THERE IS AMPLE ROOM BETWEEN PALLETS OF STONE.

PICKED UP A PIECE OF STONE TO STACK ON A PALLET AND CORNER BROKE OFF AND FELL AND HIT HIS LEG.
