A mechanic was working on a Sullair air compressor engine oil leak with the engine running. EE had an inspection mirror in EE's left hand and a flashlight in EE's right hand. EE observed that the engine was not leaking anymore so EE pulled EE's hands back. The fan caught EE's left hand and pulled it into the fan causing injuries to EE's left hand.

Employee was operating a loaded end dump truck, while EE was steering with EE's left arm EE received a sharp pain in EE's left shoulder. The employee did not report EE's injury to a supervisor until 2 hours after EE's shift ended at 4:00 p.m. EE sought medical attention hours later and was informed by that doctor to take the rest of the week off and got referred to an ortho dr.

05/09/25 emp tipped a scraper onto its side while working on a slope. EE stated EE was not injured. On next scheduled shift 05/12, emp stated that EE injured right shoulder. Emp went to ER, X-rays done, was diagnosed with right shoulder pain, and released to return to work with no restrictions. The emp worked until 05/27 when MRI indicated EE could not work without restrictions.

While climbing into a loader and using both hands, the employee placed their right foot onto the first step. While pulling themselves up, they felt a pop in their right lower leg followed by pain.
A welder was using an air chipping hammer to remove picks on the primary crusher at the Coal Handling Facility, when the electrician that was working with EE felt something hit EE's forehead. The electrician was 5 feet away from where the work was being done. The employee was contacted on the left side of EE's face above EE's eyebrow.

An employee was driving a OB truck when EE hit a soft spot on the road. The employee reported to EE's supervisor at 9:45 a.m. that EE was experiencing back pain and wanted to seek medical attention. Employee was taken to a clinic for physician observation and was referred to their clinic's chiropractor.
10/28/20 Employee walked into the boot wash room at the end of shift to wash boots off. Allegedly they stumbled falling forward and landed on right shoulder. The employee sought medical treatment later that day, no injuries were found, and EE was released immediately for regular duty. 03/09/21 Employee had surgery on right shoulder.

Employee had removed mud from a loader's hitch pin and was trying to push it down with hands to get it back into place when the pin let loose. The employee grabbed the head of the pin which caused an injury to a finger on both hands. Employee was transported for medical attention and received stitches.

Employee got fingers caught in the door frame of a CAT 777D End Dump truck while employee was trying to open the driver's side door which was stuck. Employee was transferred to hospital where employee was diagnosed with 2 fractured fingers on employee's left hand.
An employee was operating a 20,000 gallon CAT 777D water truck when the truck lost traction on a watered road. When the truck reached a dry section of the road it caused the truck to tip on its side. The operator was wearing a seat belt and was transported to the hospital where ee received 3 stitches for a laceration on right arm.
An employee was cleaning the area in between the hoist motors on a dragline using a 3-step ladder. Employee finished cleaning and was descending the ladder when the left foot got stuck causing the employee to lose balance. Employee fell off of the ladder and hit their head on the elevated sub floor nearby.

Employee was washing a metal Warehouse window with a rag when EE received a cut on EE's left thumb from the edge of the window frame. Stitches were required to close the wound.

Worker was re-spooling cable back into Ericson, P/N # 6123-100, cable reeler in the machinery house space of the 8400 dragline. The end of the cable whipped into his right eye, breaking his safety glasses and caused trauma to the eye. Worker was taken to the Sakakawa Medical Center in Hazen and then to a hospital in Bismarck. He was released the night of 10/23/15
Around 7:30 AM employee was walking to the dewatering pump to dewater the excavated hole when he slipped on a chunk of dirt and twisted his right knee.
Traveling to park loader at end of shift. Hit a rut in the haul road and jarred him in the seat. He did not feel any pain at that time. By mid-morning the next day he was feeling pain in his back and sought medical treatment.
Employees were working on a dragline under construction. Three employees were moving 36"x36"x1" plate on the house floor. Employee flipped plate and floor plate gave way. Injured employee and another, fell appox. 7 foot. Injured employee received sutures on elbow.
