Claims back soreness from climbing onto truck.

He was spraying to kill weeds and felt something in his clothing. He knocked out a spider. Later, at home, he discovered he might have been bitten by the spider. He reported it on Monday, 6/23/2014.

Employee began experiencing discomfort. He left the property for the store for Rolaids. There the discomfort worsened. He stepped across the street to the fire station. They checked him out and took him to a hospital in Memphis. There it was discovered that he was suffering a heart attack.

Employee claims he suffered a hernia from lifting and pulling a shaft. Did not report it until 09/27/2012.

Installing the last tooth on an excavator bucket with a sledge hammer. A sliver of steel from the tooth broke off striking him in the left leg.

While installing a drive shaft in the bulldozer, he felt a pain in his back.

While placing the gravel chute it fell over and pinned his right hand against the handrail. Received sutures at the base of his thumb.

Removing a bearing from the housing by striking a hammer with another hammer. A sliver of metal from one hammer struck him in the abdomen.

While changing out a conveyor belt on the portable crusher, the roll of belting slipped off the supporting chain and fell against his leg, pinning his foot and ankle under him.

EE WAS ATTEPTING TO MOVE THE TOP OFF THE CONE CRUSHER. EE STATED THAT HE STRAINED A MUSCLE IN HIS BACK. HE TOLD HIS SUPERVISOR TWO DAYS LATER.

EE WAS HELPING TO MOVE SCREENS AT THE WASH PLANT AT DEADFALL. WHEN HE PICKED A SCREEN UP, HE FELT THE SCREEN SLIP AND HE ADJUSTED HIS BODY TO CATCH IT. WHEN HE ADJUSTED HIS BODY HE FELT A TWIN GE AND SLIGHT PAIN IN HIS BACK.
