On 2.20.24, EE was pulling & lifting on an empty battery tray on a CAT 349 Excavator trying to remove it. It did not move, so EE used pry bar to remove. EE reported back soreness the next day but did not want to go to doctor. EE requested to see doctor on 2.24.25 & doctor released to return work. On 2.25.25 doctor required physical therapy and allowed EE to return to work.

PM Technician completed their Pre-Shift Inspection on their truck at the beginning of their shift. The hood did not close properly, so the operator rocked it with their hands in an attempt for it to close properly. When the hood closed, it smashed the operator's right middle finger, causing a laceration on the top and bottom of the operator's finger requiring stitches.
On 5/15/24, EE removed a car jack stand from work area. Put their right hand around the center column, while using their left hand to remove the safety pin holding the center column. When pin was removed, the column dropped, causing right thumb to be smashed by the Saddle. EE did not report injury until 5/28/24. Took EE to have thumb evaluated and found it fractured.

Employee was working with a crew clearing accumulation at the 8x20 primary screen. Employee walked away from primary screen to inspect #3 Drive Motor Belts. While looking up to inspect the belts, a falling rock struck the employee on EE's hard hat / face.

While removing/ attempting to throw a 20 lb rock from the cone crusher, the miner struck right index finger on a lifting eye with a sharp edge. EE was treated at the hospital diagnosed with partial amputation, returned to work same day, operated truck for 8 days until EE saw a hand surgeon 5/12 and was placed off work for 3 days returning 5/16.

After detaching an impact idler assembly, the miner attempted to push the assembly through the center of the conveyor frame for removal. While doing so the miner pinched and lacerated right thumb between the assembly and the conveyor frame requiring stitches.

Contract employee noticed smoke coming from haul unit. EE placed unit in park and exited cab to inspect engine. After noticing a fire in the engine compartment EE jumped over the rail of the 770 haul unit and fractured left leg when landing on the ground.

While removing rocks from a crusher 1/30/2017 and welding in a crusher the next day employee reported a strain to right knee. On 2/16 ee was evaluated and the doctor prescribed Physical Therapy making the injury reportable. EE continues to work full duty.

After locking out the jaw crusher system, operator was attempting to remove a large flat rock from beneath the jaw crusher discharge chute when rock pinched his finger causing a laceration and broken tip of right pinky finger. He was wearing gloves.

On 4/17 QC samples were being collected from a conveyor belt. Lockout requirements were not followed. The belt was accidentally activated by another employee for a few seconds. IP's knee contacted a conveyor x-member. No reportable injury was communicated. On 4/28, IP reported that while walking at a lake his knee gave out on 4/26. IP came to work on 4/28 on crutches & light duty.
Technician was installing torque converter which was on blocks, into a 769B off road dump vehicle. He placed straps under converter attempting to center it. When technician tried to push converter into position, strap slipped off to the side. Converter fell on technician's left foot causing fracture.

Employee was cleaning the windshield of his haul unit. He was in the process of accessing the right side of the truck from his cab when he fell 8.5' to the ground fracturing his left arm. The employee noted that he was not paying attention to where he was walking; his attention was on another employee.

EMPLOYEE WAS USING THE BUCKET OF A LOADER AS A TOOL AND EQUIPMENT CARRIER. EMPLOYEE WAS STRADDLING THE BUCKET AND CATWALK, WHEN THE HYDRAULICS LEAKED ON THE BUCKET AND CAUSED HIM TO LOSE HIS B ALANCE AND FALL BETWEEN THE BUCKET AND THE CATWALK STRIKING HIS ABDOMEN ON THE BUCKET.
EMPLOYEE WAS STEPPING OVER SCREEN TO MOVE TO OTHER SIDE OF CATWALK. AS HE STEPPED OVER HE STEPPED TOO CLOSE TO SCREEN AND STRUCK HIS KNEE ON ONE OF THE BOLTS ON SCREEN.
EMPLOYEE WAS REMOVING ROCKS FROM A SPILL-OVER BOX. AS EMPLOYEE WAS MOVING ONE OF THE ROCKS TO REMOVE IT FROM THE BOX, ONE OF THE OTHER ROCKS ROLLED DOWN AND CAUGHT HIS THUMB BETWEEN THE TWO RO CKS.

EE PARKED THE 769 LOADER HE WAS OPERATING. AS HE WAS DESCENDING THE LOADER BY LOADER LADDER HE STEPPED TO GROUND ON A ROCK WHICH CAUSED HIS RIGHT ANKLE TO TWIST.
EMPLOYEE WAS USING A NON LOCKING POCKET KNIFE TO CUT HOLE IN A PIECE OF CONVEYOR BELTING. WHILE BEARING DOWN ON KNIFE TO CUT HOLE, THE KNIFE CLOSED ON HIS FINGER.
EMPLOYEE WAS USING A PRY BAR TO LOOSEN SOME MATERIAL BUILD-UP. AS HE BROUGHT BAR DOWN IT STRUCK A ROCK AND THE SUDDEN STOP AND JAR CAUSED HIS LOOSE GRIP TO SLIP AND TWIST HIS LEFT WRIST.
EMPL CHANGING TIRE ON DRAGLINE.WIND WAS BLOWING AND SOME DUST CAME LOOSE FROM EQUIP AND BLEW AROUND GLASSES INTO EMPL EYE.
EMPL & WELDER WERE WORKING ON CRUSHER ON TOP LEVEL. BOTH STARTED DOWN STAIRWAY AT SAME TIME. STAIRWAY WAS NOT SECURED PROPERLY. WEIGHT OF BOTH MEN CAUSED STAIRWAY TO BREAK LOOSE FROM ITS UPPER MOST SUPPOSRT & FALL TO GROUND WITH BOTH MEN ON STAIRWAY.
EMPL WAS ASSISTING ANOTHER EMPL IN REPAIR OF BEARING ON SCREEN. THEY WERE USING AIR WRENCH TO TIGHTEN BOLTS. BOLT TURNED BEFORE EMPL COULD GET NUT ON & THIS BOLT CAME IN CONTACT WITH FINGER & CUT IT.

HE WAS WORKING ON TH JAW CRUSHER. ANOTHER MAN WAS WORKING ABOVE HIM WITH A PRY BAR. THE PRY BAR SLIPPED AND HIT HIM IN THE MOUTH. IT CRACKED ONE TOOTH AND BROKE ONE TOOTH.

WHEN THE AIR WAS RELEASED, HE JUMPED UP AND BACK TO AVOID THE DU ST THAT OCCURRE8. HIS HARD HAT FELL OFF AND WHEN HE RAISED UP HE HIT HIS HEAD ON THE CORNER OF THE OPEN DOOR.
HIS EYES STARTE8 SWELLING SO HE WENT INTO TO SEE THE EYE DOCTOR. THERE WAS NO SPECIFIC INCIDENT THAT HAPPENED FOR HIS EYES TO SWELL. DUST HAD CAUSED HIS EYELIDS TO BECOME INFECTED.
HE WAS USING A 8LEDGE HAMMER TO FREE MATERIALS BLOCKING FINES FEEDER. HE HIT THE ANGLE IRON ON FEEDER BY ACCIDENT AND CUT HIS MIDDLE FINGER ON RIGHT HAND.
