Dilltown Coal

Employee was using an air chisel to remove hardened co-gen ash from around the pad bolts on a D-8T dozer. Employee was installing new track pads. A small piece of the air chisel bit broke off the bit and struck the employee in the forehead just below the brim of hardhat. The small piece of metal was removed at a hospital emergency room.

Employee notified employer that knee was sore on 11/1/2019. No medical attention was needed or received at that time. Employee sought medical attention on 11/5/2019 because knee was swollen and sore. Employee diagnosed with bursitis on 11/5/2019 and an infection in the knee on 11/6/2019.

Employee was cutting open a transmission filter to check for metal cuttings. EE's hand made contact with the sharp edge of the filter and caused a laceration to left thumb.

Employee was tightening a nozzle on the water cannon at the truck bed wash. EE was using a pipe wrench to tighten the nozzle. The pipe wrench slipped causing EE's finger to be caught between the handle of the wrench and the safety railing. This caused a laceration to EE's left little finger and required stitches.

Employee was lifting the pin on the rear of a Komatsu WD-600-3 wheel dozer to attach a tow cable. When lifting up the pin, employee felt discomfort in right side lower abdomen. Employee was diagnosed with an inguinal hernia on 1/12/2017. Employee is having surgery today, 1/20/2017 to repair the hernia. Employee did not have any lost time until today, 1/20/2017.

Mechanic was standing on the rear bumper area of a Euclid R40C water truck and was connecting air lines to the spray valves. He reached into his pocket to get a wrench and inadvertently stepped back and his foot slipped off the bumper area. He fell to the ground and landed on his right side, causing fractures to his ribs.

Mechanic was using a sledge hammer to remove a pin from the blade of a Komatsu WD-600-3 wheel dozer. He was holding a driving bar with one hand and hitting it with a sledge hammer with the other hand when he felt a strain in his lower back.

Employee was operating a wheel dozer and plowing snow from the roadways. Approx. 12 inches fell overnight. He ran over a large pot hole or low spot along the roadway, which caused the dozer to bounce suddenly. He felt pain in his lower back and later down his right leg.

Employee was applying considerable force to a pipe wrench to turn a hitch attachment when he felt pain in his lower stomach.

Slipped on steps of a screen plant while pre-checking equipment, grabbed a handrail while falling and injured left shoulder.

EE was working with a pump at RNS Services Dilltown site. Upon completion of this job He felt slight pain and discomfort in his back. The next morning He was not able to report to work due to severe pain in his back. HE did go to Miners Hospital for treatment on 5/29/08.

Illness - high blood pressure.

The illness victim was operating a tracked bull dozer (Model D8R) from the beginning of his shift and the bulldozer was noted not to be moving, by several truck drivers and fellow employees.

At RNS Barnes/Watkins site on 2/21/07 employee was operating a rock truck when he hit a hole in the haul road. His upper body was twisted in an odd way causing pain to his upper back. He did not know the severity of the injury at the time and did not seek medical attention until 3/01/07.

Employee was using a hammer to loosen a corroded bolt. He struck the tip of his left little finger with the hammer. He did not realize the severity of the injury at this time. On 10-30-06 went to Miners Hospital for treatment of said finger, and was fitted with a soft cast for a fractured fingrer tip. He returned to work on 10-31-06.

EMPLOYEE WAS ENTERING AN EXCAVATOR, HIS FOOT SLIPPED ON SOME ICE THAT WAS ON THE STEP. EE'S LEG WENT FORWARD & HIS LEFT SHIN WENT INTO THE STEP BELOW HIM. EE WAS TAKEN TO THE HOSPITAL FOR MEDICAL ATTN. EE RECEIVED 5 STITCHES TO HIS LEFT SHIN & RETURNED TO WORK THAT SAME DAY.

Previous to this report date, employee was lifting a steel plate onto a truck. after placing this plate on the truck, he felt a slight strain in his lower abdomen. After work he noticed a bulge in his lower left side. On July 8 it was determined that it was a hernia. He is not to report to work until cleared by his physician.

EE WAS DRIVING TRACTOR TRAILER. THE ROADWAY TO THE REFUSE SITE HAD BEEN WATERED TO SUPPRESS DUST. THE TEMPERATURE HAD DROPPED AND THE ROADWAY FROZE PARTIALLY. EE WAS BLINDED BY BRIGHT SUNLIGHT AND HIS SPEED WAS EXCEEDING THE 10 M.P.H. LIMIT. THE TRUCK FAILED TO NEGOTIATE A TURN AND DROVE OFF THE ROAD, STRIKING A CEMENT WALL.

Installing hyd. oil into tank off dozer, while exiting machine, slipped off of the push arm, onto the ground twisting his left ankle.

From inside cab of 234C Cat excavator, employee stood up and twisted his body to the left to close the door. While twisting he heard a pop sound from his right knee.

WHILE DISMOUNTING FROM DOZER (TRACK WAS MUDDY & WET) FOOT SLIPPED CAUSING OPERATOR TO FALL TO GROUND & HE LANDED ON LEFT WRIST & FRACTURED IT

MECHANIC HAD JUST FINISHED REATTACHING THE TAILGATE ON THE INTERNATIONAL DUMP TRUCK. MECHANIC REACHED UP AND SHUT GRAIN DOOR WITH RIGHT HAND W/D REMOVING LEFT HAND FROM PATH OF GRAIN DOOR. THE LEFT RING FINGER WAS PINCHED AND APPROX. 1/2 IN OF THE FINGER WAS AMPUTATED ABOVE THE KNUCKLE.

FELL THROUGH OPENING IN FLOOR OF AIR STRUCTURE TABLES TO FLOOR BELOW WHICH IS APPROXIMATELY 20 FT SUSTAINING BROKEN ANKLE FRACTURED ARM(RIGHT) CHIPPED TEETH.

INJURED MAN WAS PULLING SHACK ON THE STACKER BELT MAN SLIPPED TWISTING HISRIGHT KNEE CAUSING A SPRAIN OF THE RIGHT KNEE.

A METAL SPIKE WAS PERTRUDING THROUGH A WOOD PLANK. AS THE INJURED MAN WAS HANDLING THE PLANK THE SPIKE PUNCTURED HIS LEFT THUMB REQUIRING STIT CHES.

INJURED MAN WAS POUNDING ICE OFF BELT ROLLERS WITH A HAMMER THE HAMMER AND HIS LEFT HAND WENT BETWEEN THE BELT AND THE BELT ROLLER CAUSING A FRACTURE OF THE LEFT HAND

INJ MAN WAS REPAIRING THE DE MATERIAL SCREEN LOCATED AT THE WET PLANT THE WRENCH HE WAS USING SLIPPED STRUCK MAN ON FACE CAUSING A LACERATION REQUIRING STITCHES

STANDING ON A 6" X 6" WOOD BLOCK WELDING, SLIPPED AND STRAINED CHEST MUSCLE.

INJURED MAN CAUGHT FOOT ON A RUBBER MAT LAYING ON THE FLOOR OUTSIDE OF THE MAIN CONTROL OF THE CLEANING PLANT FELL ON THE CONCRETE FLOOR CAUSING A FRACTURE OF THE LEFT LOWER ARM
