Employee was slicing a rubber liner and the knife cut through the rubber and sliced into EE's leg. Employee needed 5 stitches to close the wound.

Employee was dismounting from haul truck when the employee missed a step and fell to the ground. Employee landed hard on left foot and fractured the heel bone and damaged the Achilles tendon.

Employee had hand on hydraulic hose that punctured the skin.

While digging out a crusher, some rocks fell and struck the employee's hand and injured the left thumb.

Employee strained their back while moving electric motor on a pallet on shelving in warehouse.

Employee was moving a large rock from haul road and felt a strain to left arm while moving rock.

Employee was working on conveyor conduit in a crouched position for an extended period of time. When EE straightened up at completion of task, EE felt minor ache in lower back. Pain increased as day and evening went on. EE saw medical attention the next morning.

employee was installing a new hose at quarry dewatering pump. A front end loader with forks was being used to position the hose and when the loader back away from the area, his front tires dropped off the work platform and the forks came down on the employees foot. employee had fractured bone in his right foot.

Employee was bitten on right bicep area and the bicep muscle became swollen and red and developed mild pain.

Employee slipped off railroad coupler while manually releasing the hand brake on a railroad car. The employee fell against the coupler landing on the left side of his chest against the coupler. The employee was diagnosed with a fractured rib.

Employee was cleaning at tail section of conveyor belt k and his left glove got caught on a trough idler and pulled his left arm in through the idler to his forearm. the left arm was squeezed between the idler and the conveyor skirt frame. He received abrasions to the left forearm.

Stained shoulder while bending rebar. Felt "pop" in shoulder. Next day shoulder was tender and hurt on rotation or lifting. Went to WorkFirst for evaluation and treatment. Did not miss any time or days of work.

EE was drilling holes in angle on a drill press to make brackets. He had turned the press off and while removing the angle, the drill bit was still rotating some and there was a shaving stuck to it. This shaving caught his glove and twisted his hand around. He was able to pull his hand free of the glove. The twisting resulted in a broken bone in his hand.

Employee was manually turning crusher sheave, when sheave slid off crusher shaft and pinched 2 fingers between shaft and sheave upper flange. 1 finger broken and 1 finger dislocated on right hand of employee

A cable holding a chain come-a-long failed and the chain hit an employee in the head. The employee received a laceration to the right side of his head that required 5 stitches to close the wound.

Employee slid down a conveyor belt that was laying on the ground and over an embankment. There was a 7 foot vertical face at the bottom of the embankment, and when the employee slid over this area, he fell vertically to the ground and landed in a seated position.

Employee was using a punch and hammer to create a bolt hole in a rubber skirt liner. The punch in his left hand tilted and he was not able to stop swinging the sledge hammer in his right hand and he smashed his left hand. The left index finger was bruised and the left middle finger had the skin split open for the length of two knuckles.

Walking to equipment room when slipped on ice and fell down.

EE was removing wear shoe from Eagle Screw. EE was bent over screw to catch bolt as it was hammered out of shoe and shoe broke during hammering. Sliver from shoe punctured EE's t-shirt and skin.

EE tripped over blast hole causing while walking to haul truck. Injured left wrist when he fell to the ground.

Contractor was grinding slag from welding when a small piece of metal got embedded in his right eye. He was wearing a welding shield and safety glasses.

Employee was climbing out of a tri-axle dump truck cab, when he lost his footing on the tank step and fell backwards onto the ground, striking his head directly on the ground. Employee sustained a lacaration to the head.

Crane operator was picking up crusher part and attempting to place in loader bucker. When injured employee tried to re-adjust the chain strap, the crusher part shifted and pinched his left hand between the part and the loader bucket.

Employee was removing 54" roller (approx 200 lb) from belt structure. When it let loose, employee had trouble controlling the roller and it pinched his finger against structure.

Employee was handling 12"x 48" x1" thick steel plate trying to position it to weld in when his finger became caught on edge.

Employee was driving locomotive with loaded rail cars attached, going toward other cars to hook together when brakes failed. Locomotive traveled 1/4 mile on a 3 slope when it hit other cars.

Employee was doing a pre-inspection on a 1988 loader. He was up on the rear of the loader platform and the battery box lid had a broken hinge and the battery lid tipped and employee fell off the loader striking his head on the stel frame.

Employee was using cutting torch to cut off a bolt head when a piece of slag flew behind goggles into left eye.

While pushing and pulling lever to raise and lower truck body, feeling pain thru left arm and elbow.

Employee Welding, (hard surface) 30 x 70 crusher plates when he received flash burns to left forearm.

EMPLOYEE WAS CUTTING OUT WEAR LINERS WITH TORCH AT THE 30 X 70 CRUSHER AND BREATHED IN SMOKE WHICH CAUSED BURNING IN HIS CHEST.

EMPLOYEE WAS USING A STEEL BAR TRYING TO DISLODGE A LARGE ROCK THAT WAS JAMMED IN A CHUTE

EMPLOYEE WAS WELDING OVERHEAD AND A PIECE OF METAL OR SLAG FELL INTO HIS EAR.

Employee was setting angle iron in place while welding a guard on catwalk at 2A conveyer.

While operating 980 G loader, a particle of dust or dirt entered his right eye.

Employee was standing near other ee's making a belt splice and a object flew in behind his glasses into his left eye.

WHILE SEPARATING RAILROAD CARS FOR LOADING, AN AIR HOSE BLEW APART UNDER PRESSURE & STRUCK EE ON LEFT KNEE & LOWER BACK.

EMPLOYEE WAS HANDLING A 2"X 12" PIECE OF WOOD & A SPLINTER WENT THRU HIS INDEX FINGER.

WHILE STEERING & SHIFTING LOADER, EMPLOYEE EXPERIENCED SHARP PAIN IN LEFT SHOULDER.

Employee was climbing up ladder to enter haul truck and felt severe sharp pain in lower back, radiating down both legs.

EMPLOYEE WAS MOVING A 55 GAL DRUM AND CUT FINGER ON SHARP EDGE.

EMPLOYEE WAS GRINDING A PIECE OF METAL AND A SMALL PIECE FLEW IN UNDER HIS GLASSES AND LODGED IN HIS LEFT EYE.

EMPLOYEE REPORTED TO MANAGEMENT ON 8/26/02 THAT HE HAD HURT HIS BACK ON 8/23 & THAT HE WAS GOING TO SEE A DOCTOR. MANAGEMENT SENT HIM TO THE COMPANY DOCTOR AT READING CONVENIENT CARE. HE SAID THAT HE HIT A BUMP IN THE ROAD WHILE DRIVING THE TRUCK & HURT HIS LOWER BACK.

EE WAS STANDING OUTSIDE CAB ON LOADER, DROPPED HIS LIGHTER, BENT DOWN TO PICK IT UP AND WHILE STRAIGHTENING UP STRUCK HIS HEAD ON CANOPY OF LOADER WHICH EXTENDS 8" TO 10" FROM CAB.

EE WAS PLACING SCREEN CLOTH IN SCALPER SCREEN. SCREEN FELL ON RIGHT HAND.

EE WAS LOOSENING NUT ON 2 1/2" BOLT. SLEDGE HAMMER DEFLECTED OFF WRENCH AND EE STRUCK ANOTHER BOLT WITH FINGER OF LEFT HAND.

EE WAS CHECKING FAN BELT SLIPPING AND MECHANIC WENT TO START LOADER. SOUNDED WARNING BUZZER, BUT INJURED EE DID NOT REACT QUICK ENOUGH AND GOT FINGER CAUGHT IN BLADES OF FAN BELT.

EE WAS PERFORMING MAINTENANCE WHEN A WRENCH SLIPPED STRIKING EE'S CHEST

EE SLIPPED/FELL ON ICE WHILE WALKING BEHIND THE STORAGE TRAILER.

EE REPORTS THAT HE IS SUFFERING FROM CARPAL TUNNEL IN BOTH HANDS FROM REPEATED USE OF HAND TOOLS.

EMPLOYEE HIT HIS RIGHT INDEX FINGER WHILE SWINGING A HAMMER.

EE WAS LIFTING INGERSOL RAND JACK HAMMER WHILE HOLDING HOLES IN BOLTERS. WHILE DOING SO HE FELTBACK PAIN

EMPLOYEE COULD NOT RECALL DETAILS OF THE INCIDENT. THE LAST THING SHE RECALLS IS PREPARING A SAMPLE FOR GRADATION IN THE LAB. INJURY IS CONSISTENT OF A FALL.

EE WAS CLIMBING INT0 WATER TRUCK WHEN HE LOST HIS BALANCE AND FELL.

EE WAS CUTTING HOLE IN A STEEL PLATE WITH A TORCH. A CAR OF SPRAY PAINT IN CLOSE PRAXIMETY TO THE WORK AREA, IGNITED AND BURNED THE EE.

WHILE EE WAS CUTTING BEARING CAPS HE STEPPED ON A NAIL.

EE WAS WORKING ON THE DIFFERENTIAL OF A MICHIGAN LOADER WHEN HE STRUCK HIS LEFT HAND LITTLE FINGER WITH A HAMMER.

EMPLOYEE WAS GETTING MOTOR OUT OF WAREHOUSE AND WAS PULLING MOTOR ACROSS FLOOR. MOTOT HIT CRACK IN FLOOR AND CAUSED EMP TO JERK BACK AND FORTH STRAINING BACK.

EE WAS ATTEPTING TO REMOVE A LARGE ROCK WHICH WAS LODGED IN CRUSHER OPENING. FOLLOWING PAST PRACTICE, A HOOK WAS LOWERED BY CRANE INTO CRUSHER WITH IDEA OF HOOKING ROCK AND BY LIFTING, WOULD R OLL ROCK INTO POSITION THAT IT WOULD FALL INTO CRUSHER AND BE CRUSHED. HOOK SLIPPED FROM ROCK WHILE BEING LIFTED BY CRANE, CAUSING A FRACTURE OF HIS RIGHT ARM ABOVE THE WRIST.

EMPLOYEE WAS LIFTING A ROCK TO PLACE UNDER THE TIRE OF THE END LOADER, WHICH WAS STUCK IN THE MUD, AND HE FELT SOMETHING PULL IN HIS LEFT ARM.

WHILE INSTALLING BEARING ON JACKSHAFT OF PRIMARY CRUSHER, THE BEARING HOUSING SLIPPED CATCHING EMPLOYEE'S MIDDLE FINGER BETWEEN 1 1/2" STUD BOLT AND THE JACKSHAFT. EMPLOYEE WAS WEARING GLOVES AT THE TIME OF ACCIDENT.

WHILE SHOVELING COAL INTO FURANCE LOACTED IN GRAGE, FLAMES FLARED OUT OF FURNACE CAUSING BURNS ON FACE AND TOP OF LEFT HAND.

AT APPROXIMATELY 10:15 PM ON JULY 14,1989.INJURED (COMPANY FOREMAN) WAS STRUCK BY A CAR.THE VEHICLE WAS TRAVELING ON ROCK HOLLOW ROAD,BIRSDBORO,PA.

WHILE EMPLOYEE WAS CLEANING LEFT SHAKER SCREEN, HE BUMPED HSI RIGHT ELBOW ON THE SIDE OF THE SHAKER.

EMPLOYEE WAS DIGGING OUT THE SCALPING SCRFEEN CHUTE WHICH HAD BLOCKED WITH MUD & DIRT WHEN HE TWITCHED THE LOWER PART OF HIS BACK

EMPLOYEE CUT TOP OF RIGHT HAND AT KNUCKLE WHILE CLEANING SLIPPAGE MATERIAL FROM UNDER "H" BELT.

WASHING MATERIA5 WITH HOSE UNDER #30 CRUSHER,MOVED LADDER AND TWISTED HIS BACK.

EMPL COMPLAINED3BOTTOM OF RIGHT FOOT HURTS WHEN HE APPLIES PRESSURE TO ACCELERATOR.

HE WAS GUIDING A POWER CABLE OVER ROCKS WHEN ON END OF CABLE FELL OFF OF A ROCK AND CAUSED A JERK ON THE CABLE WHICH MADE EMPLOYEE LOSE HIS FOOTING. THIS CAUSED HIM A JAMMED RIGHT SHOULDER.

EMPLOYEE WAS PUNCHING HOLES WITH A PUNCH WHEN HE STRUCK HIS LEFT THUMB WITH A HAMMER. A CRUSHING INJURY WITH LACERATION REQUIRING STITCHES WAS THE RESULT.
