As employee approached a ladder to descend from a dragline, he lost his footing and fell onto the dragline "shoe"; he fell on the same level at which he was walking. He was given treatment by EMTs and taken to the emergency room. Employee has sore back muscles and ribs.

Pulling table bushing out of a drill and aggravated his hernia from July, 2011. Went to see a doctor on September 27, 2011.
Pulling on heavy dragline chain to get it in place. Employee did not seek medical advice until approximately a week later and then was diagnosed with a hernia. After a follow up consultation with another doctor on 7/1/2011, employee in the near future is looking to have hernia surgery.

While carrying lunch pail in one hand, was holding on to climb down rail on front end loader with other hand. Missed bottom step with foot, was still holding onto handrail and stretched. Employer was informed by employee on 5/7/2010 that this incident caused a tear in rotator cuff.

THOMAS DID STRAIN HIS BACK W HILE MOVING AN OIL BARREL WHICH HE SHOULD NOT HAVE BEEN DOING. THIS AGGRAVATED THE PROBLEM WHICH HE HAD IN THE PAST.
