EE was pulling a large jack stand out from under a haul truck when EE felt pain in lower abdomen. EE did not feel EE needed to see a doctor at this point. EE said it was more of a discomfort than pain. On 9/16/22 EE notified supervisor that the injury was getting more uncomfortable. EE was sent to the doctor at this point.
While working under a haul truck a 3/4" ratchet slipped out of the employee's hands, striking the employee in the side of the head as it fell.

The miner hit knee on a piece of machinery while walking past it.
The miner hit the end of the left index finger while driving in a screen tension wedge. This resulted in a laceration on the tip of the finger requiring three sutures.
The miner suffered from a back strain while bent over replacing belt clips on a conveyor belt. The miner has a history of back issues. EE's back has two vertebras fused from an injury several years ago.

The miner was working on a piece of equipment while in a kneeling position. An air conditioner hose blew near the miner. When this happened the miner jumped up while turning to run from the potential hazard of the hose failing. The sudden twisting of miner's back and running caused a lower back strain.

The miner received arc flash burn to his eyes. The miner was working with a welder in our welding shop. They were working back to back. The miner received the arc flash from the reflection off the inside of the welding helmet he was wearing.

While unloading some loader cutting edges out of a loader bucket the employee smashed his right index finger between a cutting edge and a wood block he was setting them on. This resulted in a laceration to his finger requiring five sutures.

The employee of the contractor was adjusting a return roller on a conveyor on there portable stacking equipment. The wrench slipped off of a bolt while pushing on it causing his arm to make contact with the moving belt and the frame work resulting in a laceration and two broken bones in his right arm. Note: This job has been completed and the contractor will not return.

Employee was stripping insulation off of an electrical connection with a utility knife. The knife slipped and cut the the top of his left thumb requiring 6 sutures.

Employee was walking down a cat walk when he stumbled on a step. He landed on his his left hand and thumb fracturing a bone in his thumb.
The equipment operator was installing a radiator fan guard after repairs were made to the engine. The operator used poor judgement and attempted installing this guard while the engine was running and made contact with the engine cooling fan. This resulted in a laceration requiring sutures to his right ring finger.
The operator was carrying items in both hands while climbing into the cab of a scraper. He set his right knee down on one of the steps to keep his blance and then pushed off with that knee to climb in the rest of the way. He felt a sharp pain in his knee when he pushed off. He said the pain quickly went away and he worked the rest of the day.
While the employee was dismounting a Cat 621 scraper/waterpull he strained his back. Employee stated he was using the proper three point dismounting procedure when the strain occurred.
Employee walked down a ramp along side of a screen, at the bottom of the ramp is one step down to a platform. When the employee stepped onto the platform his left knee gave out on him. Employee stated there was no material on the platform and he was looking at the platform when he stepped onto it. The doctor diagnosed him with a sprained left knee.

Strained shoulder while lifting a 6" pump discharge hose.
EE HAD CUT BOTH ENDS OF A 4" X 4" X 3/8" ANGLE IRON CROSSMEMBER APPX. 6'6" LONG WITH A CUTTING TORCH. WHEN THE CROSSMEMBER DID NOT FALL FREE, EE HELD ONE END WITH HIS LEFT HAND & HIT THE OTHER END WITH A HAMMER. THE END HE WAS HOLDING UNEXPECTEDLY FELL FIRST APPROX. 2' TO ANOTHER CROSSMEMBER, SMASHING 2 FINGERS BETWEEN THEM. BROKEN BONES AT THE TIPS OF 2 FINGERS; 10 SUTURES.

WHILE CLEANING PARTS TO A CONE CRUSHER EMPLOYEE SLID HIS FINGER ALONG A SHARP THREAD AND SLICED IT. IT WAS A SMALL CUT NOT REQUIRING SUTURES. HE GAVE HIMSELF FIRST AID (CLEANED & APPLIED BAND AID) AND WENT BACK TO WORK.

TWO WORKERS WERE CHANGING CUTTING EDGES ON A CAT 633 SCRAPER. INJURED WAS LAYING ON A CREEPER REMOVING BOLTS FROM THE CENTER EDGE. CO-WORKER WAS MOVING A SIDE EDGE OUT OF THE WAY. WHEN HE HEAR D THE EDGE HIT THE GROUND, HE TURNED AND FOUND INJURED'S RIGHT ARM PINNED UNDER THE CUTTING EDGE. HE REMOVED THE EDGE.
THE MINER CAME TO THE PLANT MGR. INDICATING THAYT HE PULLED SOMETHING IN HIS STOMACH AREA WHILE REMOVING STICKS FROM A CONVEYOR BELT. THE INJURY OCCURRED IN THE SAME AREA AS A RECENT GALLBLADD ER OPERATION. HE WAS SENT TOTHE DOCTOR TOHAVE IT CHECKED OUT.

WHILE HAMMERING ON A CONVEYOR RETURN ROLL TO REMOVE BUILD UP MATERIAL ON THE ROLL THE TOP OF HIS RT WRIST MADE CONTACT WITH THE CONVEYOR BELT CAUSING AN ABRAISON. THE ABRASION BECAME INFECTED THREE DAYS LATER AND THE EE WAS SENT TO THE DR FOR TREATMENT.
WHILE SHOVLEING MATERIAL OFF OF A PLATFORM HE HIT THE BAK OF HIS HAND AGAINST A HAND RAIL. THIS IRRITATED AN UNKNOWN CYST AND CAUSED IT TO SWELL. THE INJURY WAS NOT REPORTED TO HIS SUPERVISOR FOR APPR ONE WEEK AFTER THE INCIDENT. THE EE WASTO THE DR UPON MOTIFICATION.

EMPLOYEE HAD BEEN ASSISTING IN SMALL LOGGING/CHOKE SETTING OPERATION. SEVERAL LOGS HAD BEEN STACKED ONTO ONE ANOTHER, AND EMPLOYEE WAS STEPPING DOWN OFF OF LOG STACK. AS EMPLOYEE STEPPED ONTO BOTTOM-MOST LOG, HE STEPPED ONTO A SECTION OF THE LOG WITH NO BARK. THE LOGS WERE ALSO SLIPPERY DUE TO RAIN WEATHER. EMPLOYEE SLIPPED AND LANDED WITH RIGHT LEG BENEATH HIM INJURING RIGHT KNEE.

RIGHT HAND SLIPPED OFF HOSE WHILE PULLING CAUSING HAND TO HIT ENGINE FAN. EE WAS IN VILATION OF SAFE WORKING PRACTICES AND INJURY ILLNESS PREV. PROGRAM. EE WAS ALSO COMPLACENT AND DID NOT
WHEN EE AND ANOTHER EE PULLED A PALLET OUT FROM UNDER SOME BUCKET CUTTING EDGES, THE EDGES FELL DOWN ON EE'S RT FOOT, SMASHING HIS BIG TOE NEXT TO IT.
