IP was transferring process solution (mix of sulfuric acid, dipotassium phosphate, and ammonium sulfate) from one tank to another using a pump and hoses. During transfer a solution splash contacted IP's left eye. First aid eye flushing was administered and IP was transported to medical facility for additional flushing. No permanent damage or impairment.

Employee orally ingested approximately 3 ounces of electrolyte solution causing throat irritation. Employee was hospitalized for 24 hours for observation.

The employee was moving a copper cathode from scale when the cathode slipped off bundle and struck employee in the top center of left foot. Initial examination determined that the employees foot sustained a contusion. He was released back to work with limited restrictions.

EE was removing wear plate bolt, the wear plate fell and a sharp edge cut his right middle finger. EE needed stitches. Crew EE works with didn't return to work until 4/24/10.

Was pulling water pipe and felt a pain in his back
Was coming down from the leachfield heap when he twisted his ankle.
Was putting a new screen in the screening plant when the screen fell on his finger smashing it.
Was cleaning tail pulley on BC 14 when roller came back and pinned his arm.
When he was shoveling he felt a pull in his right forearm.
It had been raining all day and when he stepped out of skid steer, he slipped and kicked the bucket.
Was using jack hammer to clean aglomerator when it slipped and fell on his foot.
He was on the leach field pulling 4" drisco, when he pulled the drisco his shoulder dislocated. He then put it back in place on his own.
Employee was removing a stuck cathode from stripping machine when foot got pinched between cathode blank and frame. The employee's right foot received a puncture/laceration.

Employee was installing tires on grasshopper frame and strained back.
Installing back panel of rock box; panel being set when gust of wind pushed panel and struck employee. Employee had Contusion and abrasions to chest and shoulders. Placed on temporary modified-work status.
Installing back panel of rock box; panel being set when gust of wind pushed panel and struck employee on left foot. Employee?s foot/toes were crushed. Placed on no-work status, hospitalization, pending amputation.
Employee was placing jumper bar on cell line and smashed right index finger between jumper bar and hanger bar on cell line. There was little damage to finger and no impairment.
Piece of round stock rolled off table and employee attemped to catch it. His left ring finger then got smashed between the floor and the metal. Require 4 stiches. immediate return to duty

While loading steel onto the bucket of a skid steer, a piece of metal fell off, striking employee on the left foot.
Tying rebar on 8-16-07, he hit his arm on it which caused a small scratch. He did not report the incident until his arm swelled up which was on 8-23-07. The doctor gave him antibiotics and said if got worse to go to the ER. The next day it was worse so he went to ER and there it was found that he had a staph infection. He was released to return to work.
Cut 4th finger on right hand while removing rock from tailgate of 10 wheeler dump. Received stitches @ Benson Hospital. Was released to return to work
