EE was running a loader, loading a truck, when there was a sudden issue with the steering system, and it began jerking to the left and the right. EE was diagnosed with a neck strain. The loader was taken out of service until it gets fixed.
EE was attempting to remove a bolt and nut. The nut was welded to a bracket. While using an impact gun, the wrench slipped off the nut, causing the bracket to spin. This caused a cut to the left pinky finger. EE tried to get bracket off by using a wrench so the bracket wouldn't spin, but it did, when the wrench slipped the bracket spun. This required 5 stitches to pinky finger.

While maneuvering to grease along the bottom of a front-end loader, the employee bumped the back of his head and then reacted by suddenly leaning forward, striking the front of his head against a corner on the loader.
Employee was carrying one end of a long rebar and started to lift the end above his head. As he did so, he took a step forward and slipped on the rebar mat he was standing on and pulled a muscle in his calf. The rebar mat was approximately 5 inches off of the ground.

As the employee pulled open the tailgate on his bin truck to dislodge a rock that was wedged, the tailgate suddenly closed shut and crushed his finger against the truck bed.
The injured employee was assisting two others in manually removing and lowering a 250-pound primary crusher impact bar from waist level to a set of wooden blocks on the catwalk. When the injured employee?s wet gloves slipped from the bar, the uncontrolled bar crushed his finger against another bar that was already positioned on top of the wooden blocks.

WHILE GRINDING ON THE ALUMINUM DUMP BED OF A BIN TRUCK, A METAL PARTICLE GOT INTO THE EMPLOYEE'S RIGHT EYE. HE WAS WEARING ALL REQUIRED PERSONAL PROTECTIVE EQUIPMENT, INCLUDING SAFETY GLASSES AND A FACE SHIELD AND THE HAND GRINDER WAS GUARDED.
WHILE CLIMBING THE SIDE OF #5 CONVEYOR INSIDE #1 SCREEN TOWER TO LOOK INTO THE COLLECTOR HOPPER FOR A WEAR PLATE THAT CAME OFF OF THE CRUSHER, THE EE LOST HIS GRIP, FELL, AND TWISTED HIS KNEE.

WHILE ATTEMPTING TO CLEAN THE FRONT WINDSHIELD OF HIS LOADER, THE EMPLOYEE SLIPPED OFF OF THE RIGHT FRONT FENDER AND FELL APPROXIMATELY 8 FEET TO THE QUARRY FLOOR BELOW.
THE EE LOADED HIS BIN DUMP TRUCK WITHOUT KNOWING THAT THE TAILGATE WAS UNLATCHED. AFTER NOTICING THAT THE LOAD OF STONE WAS PREVENTING THE TAILGATE FROM COMPLETELY CLOSING HE REMOVED SOME BUIL DUP. WHEN THE STONES FELL OUT, THE TAILGATE SUDDENLY CLOSED AND CRUSHED 3 OF HIS LEFT FINGERS BETWEEN THE TAILGATE AND DUMP BED.
THE EE DEVELOPED CUMULATIVE SHOULDER PAIN AS A RESULT OF CONTINUOUS OPERATION OF A LOADER. THISOCCURRED AROUND 3/1/02, NO SPECIFIC TIME GIVEN.

EE FELL HALFWAY DOWN AN UNUSUALLY STEEP STAIRWAY THAT IS ADJACENT TO THE METAL DETECTOR AT THE PRIMARY PLANT AND SUSTAINED MULTIPLE INJURIES.
EMPLOYEE DEVELOPED DELAYED BACK PAIN AFTER SHOVELING STONE OFF #2 SCREEN.
DEBRIS ENTERED THE EMPLOYEE'S LEFT EYE WHILE EMPTYING A SNAD BIN. THE DEBRIS WAS GENERATED AS AN EMPLOYEE WORKING ABOVE WAS BANGING ON THE SIDE OF THE BIN TO LOOSEN CLOGGED MATERIAL.
EE STRAINED HIS BACK WHILE TRYING TO SLIDE A PIECE OF PLYWOOD BETWEEN A CONVEYOR BELT UNDER TENSION AND THE CONVEYOR TROUGH IDLERS IN ORDER TO LACE THE BELT TOGETHER.

THE EMPLOYEE WAS OPERATING A TOWMOTOR AT THE TIME. WHILE OPENING THE TOWMOTOR DOOR TO GET OUT OF THE TOWMOTOR, THE EMPLOYEE PUSHED AGAINST A CRACKED WINDOW ON THE TOWMOTOR DOOR. HE SUFFERED A SMALL CUT ON HIS LEFT HAND WHICH HE PUT A BANDAID ON. HE CONTINUED TO FEEL DISCOMFORT AND WENT TO THE DOCTOR 10-9-89. THE DOCTOR USED A SMALL KNIFE TO REMOVE A SLIVER OF GLASS.

WHILE LOWERING THE DUMP BODY EMPLOYEE REACHED OUT OF THE CAB AND ATTEMPTED TO PULL THE LEVER UP TO CLOSE THE DUMP BODY TAILGATE. AS HE HELD THE LEVER, AND THEDUMP BODY CONTINUED TO LOWER, HIS ARM WAS PULLED DOWN AGAINST THE GRAB HANDLE ATTACHED TO THE SIDE OF THE TRUCK CAB.
