While replacing hydraulic line on JLG man lift, miner was reaching above EE's head into difficult to reach area felt sharp pain in shoulder. The miner took Ibuprofen & went to EE's chiropractor that night. 9/14 while lifting ordinary grease gun pain increased & manager sent Miner to ER- Diagnosed with sprained shoulder. Prescribed Muscle relaxer and rest-to return to work 9/20

Employee, while traveling along haul road on a pit bike the front tire was kicked out by a rock. Employee lost balance and fell down on a rock which lacerated their forearm that required stitches.

Miner was assisting in pouring cement, miner was wearing knee high rubber boots which were duck taped to pants at some point cement came in contact with pants making them wet. Approx. 1 hour of work and miner noticed skin irritation, first aid did not nullify irritation. Miner was sent to hospital for chemical burns of mid leg region

Another miner applying alcohol to truck airline as EE walked around the front of the truck, airline inadvertently disconnected & sprayed EE with alcohol. Some alcohol got into EE's eyes, eyes were promptly rinsed with eye wash. Miner was informed if irritation returned to go to emergency room. ER/Dr prescribed eye drops and advised against work for 3 days.

Plant operator starting up the plant, noticed the main crusher vibrator not working. Locked out and proceeded to check if drive belt missing. Belt was still present, so EE gave the pulley a tug, felt a sharp pain in left shoulder for aprox 20 min. pain faded and worked rest of day, notifying management at end of the day.

While descending a flight of steps that were covered in approx 1 in of wet snow. EE lost footing on the 3rd to last step and fell to the ground below onto EE's back. Employee worked for approx 2hrs was later sent home after which EE went to hospital, diagnosed with deep bruising of the back. 2 days of rest prescribed.

Employee was walking on flat surface at control booth - There were no obstacles in his path.

Employee climbed up on Volvo highlift to check hydraulic oil. Raised up very quickly & bumped his left nostril into CB antenna.

When moving a stone off plant - Stone pinched little finger against post

Employee was working the rock truck. He dropped the emergency power steering motor out of his own hands onto his head.

EE was working on main belt, 2 other EE's were working in above crusher. A 2x8x3 board was being used to prop up a hydraulic cyclinder on backhoe, cyclinder was moved and board fell thru crusher striking EE's hard hat and head.

Ee was lifting and pulling a small trailer a short distance on cement in the garage. He then put it down on a stand when he felt pain. He went to a chiropractor. Several hours later he was taping paper on a truck cab. Then he lifted a one half gallon paint can at which time he felt excruciating pain.

Spillage rock - moving a rock that was too big to handle by himself.

Rock jammed in return roller on main belt; he shut off main belt, removed rock then 2nd rock hit his finger.

Employee was running drill press when cuttings caught his finger requiring several stitches. No lost time occurred.

Walking on catwalk. Tripped and fell, falling on his hand. There was no debris or obstruction on catwalk.

EE was driving wedge to lock cone crusher down with a sledge. when a piece of unknown debris bounced off safety glasses and was deflected into eye. when ee was seen at er-nothing was in his eye-it was washed out at plant. he had a small laceration.

EE WAS PRYING ANGLE IRON OFF FORD TRK & JAMMED HIS THUMB. AT THE TIME HE THOUGHT IT WAS JUST SPRAINED. IT CONTD TO BOTHER HIM AT WHICH TIME HE SAW A DR RECENTLY

EE STATES TOOK ORDER TO FILL CUSTOMERS MAGAZINE WITH AUST 15. HAD TO STACK THE BAGS TO THE VERY TOP OF MAGAZINE TO MAKE ALL OF THEM FIT. LIFTED ONE TO THE TOP WHEN HE FELT A SHARP BURNING PAIN DEEP IN MY SHOULDER.

USING A SLEDGE HAMMER TO REMOVE A ROCK FROM FEEDER.

EE AND COWORKER GOT INTO A FIGHT. COWORKER PUNCHED EE. FIGHT HAD NOTHING TO DO WITH BUSINESS. COWORKER WAS FIRED ON THE SPOT.

MOMENTS BEFORE EMPLOYEE WAS WORKING ON PLANT THEN HIGHLIFT WAS LOADING TRUCK WITH ROCK DUST EMPLOYEE STUCK HIS HEAD OT OF TRUCK WINDOW TO SEE WHAT WAS GOING ON & GOT DUST IN HIS EYE HE TRIED T O CLEAR IT BY LIFTING EYELID UP & OVER TO CLEAR DUST OUT THAT IS WHEN RUST WAS PICKED UP OFF LOWER LID & GOT IN EYE HE DID NOT KNOW THERE WAS RUST ON HIS FACE

EE WAS USING PRY BAR TO GET A ROCK OUT OF SURGE BIN BELT. HE HIT HIS RIGHT ELBOW ON A PIN. EE THOUGHT IT WAS JUST BRUISED BUT ON 4-23-94 IT STARTED TO SWELL. HE WENT TO THE DOCTOR ON 4-24-94 I T STARTED TO SWELL HE WENT TO THE DR ON 4-24-94

EMPLOYEE WAS HELPING MOVE OUT OF SERVICE CONVEYOR FRAMES. HE WAS WALKING AROUND. E STEPPED ON A OLD BOARD AND HIS KNEE WNET THROUGH BOARD. AT THE TIME THE EMPLOYEE SIDE IT WAS JUST BRUISED, NO THING FURTHER. IT DID NOT BOTHER HIM RIGHT AWAY. RECENTLY, IT STARTED TO BOTHER HIM SO HE WENT TO THE DOCTOR. EMPLOYEE HAS MISSED NO WORK.

WORKING ON 988A CAT HIGHLIFT- HANDLING THE HYDRAULIC CYLINDER. CYLINDER FELL ON SMALL FINGER OF RIGHT HAND.

EMPL WAS CHANGING OIL IN 988 CAT HIGHLIFT. MACHINE WAS WET & MUDDY. EMPLS FEET WERE ON COUNTERWEIGHT AT REAR OF MACHINE & HAND GOT CAUGHT NEAR REAR LIGHTS CAUSING EMPL TO FALL.
