The employee experienced some discomfort in the right shoulder after over-exerting to collect finished product grab samples from the top center palletized 55 lb. bag, from the HRP auto-line. The employee had not reported the injury until 1/15/2024. Operator was diagnosed with a strain.

The employee was manually loading a wooden pallet into the robot pallet feeder, the employee felt pain in their left shoulder. This task occurred numerous times previously in the shift. The employee initially took Ibuprofen for the pain. The following day, the employee went to urgent care and was removed from work. X-rays showed no fractures.

The operator was drilling a hole through steel to affix a grease line. The drill bit bound up while drilling and spun the drill body striking the operator on the left side of the face causing a bruise. The physician checked for concussion symptoms, none found, and removed the operator from work to prevent a concussion while the operator was in a vulnerable state from this impact.

Employee was using a 6" grinder with a zip cutting wheel to cut a 6" channel iron. The grinder handle was removed to allow getting the grinder into a tight space. As employee was cutting the channel iron, the grinder kicked back and the wheel struck the employee's left hand cutting through work glove and causing laceration to left ring finger.

Employee was placing a 55lb bag of finished product on to a pallet for further packaging. The employee stated EE felt a "pop" when EE twisted and placed the material at approximate waist height onto the pallet. The employee was diagnosed with a strained muscle behind EE's left shoulder blade.

Employee cut left palm requiring three stitches while using a knife to trim excess urethane from an apex mold.

Employee reported a right shoulder strain on 11/12/2010 due to bagging 55 pound bags of product. Employee did not receive treatment that would classify the injury as reportable. Since that time the injury diagnosis has changed to right wrist carpal tunnel syndrome and the employee needs physical therapy.

Employee reported on 7/27/10 that he had sustained a back injury from bagging product on 7/19/10.

Employee was brushing screens, he set down the brush and started to walk down the stairs. Employee stated he lost his balance and caught the railing with his right hand to prevent himself from falling. Employee reported pain in his right shoulder.

Employee stood on scalper bin lid and the lid gave way. Employee fell into bin. He caught himself on the side of bin causing a contusion to the side of his left chest.

Employee was closing claim shell to feed belt and got a piece of metal stuck in his finger.

Employee reported experiencing back pain after hooking chain to screen.

Employee reported on 8/12/2008 he strained his back on 8/11/2008 while trying to move a spool of wire. He did not need medical treatment at that time. He missed work 8/20/2008 thru 8/24/2008. He presented a slip on 8/25/2008 stating he needed to be off work for this time frame, but could return to work without restrictions on 8/25/2008.

Operator was coiling a section of steel reinforced rubber hose. The hose slipped from his grip and the exposed wire cut his left thumb.

Employee was using a 4 inch hand held grinder, with a cutting disc, to cut a piece of angle iron. The cutting disc became lodged in the cut and broke apart. Two broken pieces contacted the employee's le, causing cuts.

Product being bagged for shipment became airborn and was caught in the employee's eye causing an infection.

Repetitive Motion has caused employee to develop Carpal Tunnel disease.

Piece of steel being cut fell on the operator's foot causing bruising to the foot.

Operator slipped while coming down off work platform and landed on the hand railing. Railing struck and bruised the employee's ribs.

WHILE ATTEMPTING TO INSTALL (HANG) A HOPPER, FINGER BECAME PINCHED BETWEEN VALVE OF HOPPER AND THE HOPPER FEEDER.

Transfering (Handling) 100lb bags from one pallet to another, Pulled muscle in lower back.

Pulled Muscle in arm while scraping excess material from a bulk bag during the bagging process. Pain in right shoulder & arm.

EE SPRAINED HIS LEFT WRIST FROM A FALL ON A CATWALK. USING HIS HADN TO BREAK THE FALL, SPRAINED HIS WRIST.

EE STRAINED HIS RT. WRIST, WHILE BAGGING 55 LB. BAGS OF GARNET.

EE'S LEFT HAND WAS STRUCK BY THE END OF A HYDRAULIC LINE. HYDRAULIC SWIVEL FITTING BROKE WHIPPING THE HOSE AT THE EE.

WHILE THREADING PLASTIC WRAP INTO A WRAPPING MACHINE, EE'S LEFT FORE FINGER & MIDDLE FINGER WERE PULLED INTO THE FEED ROLLS. EE SUFFERED SEVERE LACERATIONS TO THE 2 FINGERS.

EMPLOYEE STEPPED OFF A LADDER & TWISTED HIS BACK

A BIN DISCHARGE HOSE BROKE & STRUCK EE KNOCKING HIM DOWN ON TOP OF TRUCK. HE STRUCK HIS LEFT ARMON TRUCK WHEN HE FELL.

WHILE UNPLUGGING A POWDER BAGGING MACHINE, EMPLOYEE STRUCK FINGER AGAINST THE FEED AUGER; LACERATION REQUIRED APPROX. 5 SUTURES.

WHILE DUMPING BARRELS INTO TRUCK. THE EE FELL IN THE HOLE/OPENING IN TOP OF TRUCK HITTING HIS RIGHT SIDE. HE FRACTURED TWO RIBS.

EMPLOYEE WAS LIFTING A SCREW FEEDER AND PULLED A MUSCLE IN HIS BACK.

EMPLOYEE REPORTED A BACK STRAIN AS A RESULT OF DUPING BARRELS OF PRODUCT INTO A HOPPER.

EMPLOYEE REPORTED A RIGHT SHOULDER STRAIN AS A BRESULT OF BAGGING AND PALLETIZING 50LB BAGS OF PRODUCT.

EE STRAINED HIS BACK WHILE PUMPING BARRELS OF SCRAP STEEL OFF THE BACK OF A TRUCK.

EE STRAINED LOWER BACK WHILE LIFTING AND PALLETIZING 100 LB BAGS OF GARNET.

EE WAS RAISING A SCREEN COVER USING A PULL CABLE & SLIPPED ON THE FLOOR GRATING CAUSING THE SCREEN COVER TO FALL & THE PULL CABLE PULLED THE EE'S RIGHT SHOULDER WHILE HE HELD ONTO THE CABLE HA NDLE.

EE STRAINED HIS BACK WHILE PUMPING BAGS OF RERUN MATERIAL.

EE REPORTED PAIN IN RIGHT WRIST AS A RESULT OF BAGGING & STACKING PRODUCT.

EE FINGER WAS PINCHED BETWEEN TWO STEEL BLOCKS WHILE USING A PRESS TO STRAIGHTEN A LENGHTH OF PIPE.

EE STRUCK HIS ELBOW AGAINST A STORAGE HOPPER WHILE SWEPING THE FLOOR.

EMPLOYEE STRAINED BACK MUSCLES WHILE WORKING INSIDE A BALL MILL.

EE WAS DRILLING URETHANE PART WHEN A PIECE OF MELTED URETHAN CAME OFF THE DRILL BIT AND STUCK ON THE EE'S RIGHT HAND

SLIPPED ON CARDBOARD & STRUCK KNEE AGAINST A BARREL.

EE BENT OVER TO PLACE A STACK OF EMPTY PRODUCT BAGS ON THE FLOOR AND FELT A PULLING SENSATION PAIN IN HIS BACK.

POSSIBLY TORE MUSCLE OR LIGAMENT IN RIGHT ARM WHILE PULLING ON CABLE AND PULLEY SYSTEM TO RAISE THE COVER ON A DERRICK SCREEN.

PINCHED FINGER IN CONNECTION WHILE INSTALLING EXHUAT PIPING.

EE FELT A PULLING SENSATION IN HIS RT GROIN AREA WHILE LIFTING 100 LB BAGS OF GARNET.

WHILE HANDLING MATERIAL WEARING GLOVES METAL SLIVER ENTERED LEFT HAND RING FINGER.

THE EMPLOYEE WAS CLIMBING DOWN A PERMAENAT LADDER WHEN HE LOST HIS FOOTING AND FELL AGAINST THE SAFTEY CAGE SURROUNDING THE LADDER.

EMPLOYEE WAS PU8HING A PORTABLE WALK-WAY INTO POSTION FELT A PULLING SENSATION IN RIGHT LEG.

EMPLOYEE DOES N2T KNOW WETHER INJURY IS WORK RELATED. INJURY POSSIBLY FROM LIFTING 100 POUND BAGS WHILE AT WORK.

EE WAS TURNING 2VER A AP MACHINE WHEEL ON BENCH WHEN INJURY OCCURRED. WHEEL WT APPROX 150#.
