Employee was shoveling stone off conveyor belt when slipped and hit nose against I-beam.

Employee welded a nut to the head of a bolt that was worn down to be able to remove it. When they went to actually put pressure on the hand tool and welded nut, the nut broke off and caused their hand to slip and hit part of a crusher table.

While working on a conveyor employee picked up a wrench to hand to another employee. When the employee took a step and turned EE felt a pop in EE's right knee.

Miner was on top of a crusher waiting to unload a wear plate EE was going to install in the hopper above EE. While the plate was being set on top of the crusher for EE with the tele-handler, it shifted and the miner tried to grab it to keep it from falling. In doing this EE strained a muscle in EE's lower back.

A valve for the lid lift sys. on a ISC 77 crusher stuck in the open position letting the pressure off the hydraulics. This allowed the lid to descend. Employee didn't install the safety pin in the lift system to prevent this before doing maintenance. As they moved the lid over for inspection, it descended unnoticed and pinched EE's finger between the lid and a hold down loop.

Employee was stepping down two steps onto a landing that is built with expanded metal for the platform. There is a slight bend in the catwalk where it's welded to an I beam for support. As the employee stepped on the cat walk, EE stepped into the bent area, causing EE to roll ankle which was later determined to be a sprain.

Employee had a STS in EE's yearly audio metric test.

Miner was helping remove a bucket from a Cat 232D. As they hit the locking pins loose, from the dirt and mud build up, the bucket broke loose and swung forwards and backwards striking left foot. The arms of the skid steer were up and the bucket rolled forward putting the cutting edge on the ground. the operator was in the cab at the time.

988K Loader Operator did not see service truck or ground laborer near truck as they were backing up and struck the laborer in the lower back with the rear bumper of loader. The loader operator pulled forward dumped a bucket of material then backed up and struck the service truck.

Miner was using a 4 1/2" grinder to cut a piece of screen cloth. The grinder had had the guard removed. As EE was cutting the screen it cut through the wire and hit the next one causing it to kick back and hit EE's left hand. Cutting EE between the middle finger and index finger, parallel to EE's fingers.

EE was putting a piece of scrap metal from the crusher (45-037) into the steel scrap hopper. While throwing it into the hopper EE felt something in the left elbow area give and a burning sensation started on the inside of EE's arm.

WHILE EE WAS DOING A PRE-OP INSPECTION ON TRUCK, WHILE DISMOUNTING THE TRUCK EE FELL TO THE GROUND STRIKING HEAD.

WHILE EMPLOYEE WAS REPLACING A WIRE CLAMP ON THE PIONEER STACKER CONVEYOR, THE PHILLIPS HEAD SCREW DRIVER SLIPPED OFF THE SCREW PUNCTURING HIS LEFT HAND REQUIRING TWO STITCHES.

WHILE EE WAS DOING A PRE-OPT INSPECTION OF THE POND AREA HE SLIPPED ON THE FROZEN MUDDY GROUND TWISTING HIS RIGHT ANKLE.

WHILE LIFTING A STEDMAN WEAR PLATE INTO PLACE, EE LOST HIS GRIP. THE PLATE FELL 2 FEET ON HIS LEFT PINKY FINGER.

After cutting a 4x4 angle iron 20' long it fell to the ground and flipped up and landed on employee's right foot and ankle.

WHILE PLACING A 50 TON RAM UNDER THE CENTER PIN OF THE LOADER, EE FELT A STRAIN IN HIS GROIN.

WHILE SHOVELING AROUND THE CRUSHER, EE EXPERIENCED CHEST PAIN.

WHILE LIFTING A BUCKET PIN, EE FELT A STRAIN IN HIS LOWER BACK.

WHILE EE WAS DISMOUNTING THE DOZER, HIS FOOT SLIPPED OFF THE RAIL CAUSING HIM TO FALL TO THE GROUND

WHILE OPERATING 30-B CRANE, EE PASSED OUT AND FELL OFF THE CRANE APPROX 5 FEET TO THE GROUND STRIKING HIS FOREHEAD AND RIGHT SHOULDER ON THE TRACK.

WHILE WALKING UNDER A CONVEYOR, EE STEPPED ON A ROCK CAUSING HIM TO LOOSE HIS BALANCE. WHEN HE FELL TO THE GROUND, HE JAMMED HIS LEFT FOOT FRACTURING HIS LEFT HEEL.

WHILE REPLACING A BREAKER BAR IN THE CRUSHER, IT JARRED LOOSE FALLING ON EE'S LEFT THUMB.

WHILE INJURED WAS CLEANING UNDER THE CONVEYOR A ROCK FELL OFF STRIKING HIM ON THE RIGHT SIDE OF HIS NOSE.

WHILE EE WAS REPLACING THE BATTERY BOX COVER ON THE R35 EUCLID DUMP TRUCK HE LOST HIS BALANCE AND FELL BACKWARDS TO THE GROUND LANDING ON HIS FEETING AND A 2INCH ROCK BREAKING HIS LEFT HEEL.

WHILE INJURED WAS OENING THE OVERHEAD SHOP DOOR THE WIND BLEW A PARTICLE OF STEEL INTO HIS LEFT EYE.

WHILE HE WAS TIGHTENING A NUT TO MOUNT A MOTOR TO A STAND, THE MOTOR AND STAND TIPPED OVER PINNING HIS LEFT ANKLE UNDER IT AND BRUISING HIS RIGHT KNEE.

WHILE OPENING CAP HOUSE DOOR, HIS THUMB CAUGHT BETWEEN THE DOOR AND THE LOCK HASP, CUTTING THE END OF HIS THUMB.

HE WAS CHANGING THE CUTTING EDGE ON A LOADER BUCKET. HE HAD REMOVED THE OLD ONE AND WAS REFITTING THE CENTER SECTION. HE USED A PUNCH TO LINE UP THE BOLT HOLES AND WAS PLACING A BOLT INTO THE FIRST BOLT HOLE. THE STEEL SLIPPED AND FELL CATCHING THE LEFT HAND BETWEEN THE BOLT HIS WAS HOLDING AND THE 200# CUTTING EDGE. THIS CRACKED ONE KNUCKLE AND CHIPED ANOTHER.

I.NAME STEPPING OFF STAIRWAY TO PEP SCREEN,FOOT HIT UPON BUMP OF OLD FOOTPRINT.FOOT SLIPPED SPRAINING LEFT ANKLE,CAUSING FALL INTO BOTTOM OF HANDRAIL WHICH BRUISED AND CUT UPPER RIGHT ARM,SHOU LDER.WORKED THE REST OF THE DAY THEN REPORTED IT NEXT AM -FOOT WAS TOO SORE TO WORK TRUCK PEDDLES.

THE INJURED WAS REACHING FOR A CHAIN ON A FLAT BED OF A TRUCK.HE WAS JUST A LITTLE TOO SHORT TO REACH IT,SO HE JUMPED UP WHERE HIS STOMACH WAS RESTING ON THE BED,HIS FEET WERE ABOUT A FOOT IN THE AIR AND HE COULD REACH THE CHAIN.AS HE SLID DOWN FROM THE TRUCK HIS LEFT FOOT LANDED ON A FROZEN TIRE RUT AND HE TWISTED HIS ANKLE.

WAS CLEANING UP3UNDER DISCHARGE CONVEYOR STANDING UNDER CATWALK TO CONTROL SHACK A ROCK RICOCHETED & HIT HIM IN THE HARD HAD KNOCKING IT OFF THEN EITHER HE WAS STRUCK BYU ANOTHER ROCK OR MOR E LIKELY FELL O3TO A ROCK ON THE GROUND LACERATING THE RT.REAR OF HIS SKULL THIS REQUIRED 5 STITCHES HE WAS GIVEN TGETANUS SHOT & ANTIBOTIC TO PREVENT INFECTION

I.NAME WAS CUTTING OFF BOLTS FROM A BASE TIE-DOWN. HE STOPPED FOR A MINUTE AND THOUGHT THE FLAME HAD EXTINGUISHED. HE WIPED OFF THE TORCH TIP WITH HIS LEFT HAND, AT WHICH TIME THE TORCH FLARED UP AND BURNED HIS HAND

WORKING UP UNDER SWIVEL FOR THE BOOM WHEN TAKING OFF A HYDRAULIC HOSE THAT WAS TIGHT THE COUPLING BROKE FREE & I.NAME RIGHT THUMB HIT A SHARP METAL EDGE. THIS LACERATED THE THUMB & REQUIRED 10 STITCHES. THUMB IS SORE BUT MOBILE.

WORKING WITH 1/8" PLATE TO MAKE GUARD SOMETIME DURING THE 28TH OR 29TH A PIECE OF RUST GOT AROUND BERSHIRES GLASSES & LODGED INTO HIS RIGHT EYE. ABOUT 2:00 PM ON THE 29TH TH EYE BEGAN TO WATE R & STING. IT WAS LOOKED AT BY A CO-WORKER & THEN BERSHIRE WENT TO THE EYE DOCTOR WHERE THE RUST WAS REMOVED FROM THE CORNEA.
