Contractor EE was repairing a ripper tooth from a D10 Dozer. EE hit the ripper tooth with a hammer a couple of times to get it in place for the retaining pin. A piece of metal sheared off of the ripper and embedded into the EE's left arm. EE eventually went to the ER for treatment. It was removed and EE was prescribed antibiotics.

EE was loading 18" culverts onto a trailer when one became unstable. EE tried to stop and reload it back onto the trailer. EE felt a pop on the inside of right elbow. EE suffered a torn distal biceps tendon.

EE was cleaning out a scraper bowl when they slipped and fell backwards into the bowl. EE was waiting on a doctor's appointment for follow up. EE is now on injury leave awaiting further tests and treatment.

EE was removing a pin from the broken bail of a scraper. EE slipped and fell appx eight feet to the ground.
EE was washing out a loader engine deck. EE descended ladder, slipped on the cab step that was covered in wet bentonite and oil, and fell on their right hip/side. EE injured their right hip area and neck.
Employee was trying to dislodge a rock from between discs on tractor and used a hammer and a screwdriver. Employee hit the top to their left hand with the hammer when the rock broke free.

Employee was ripping rock with a dozer, when the dozer tracked over a rock and dropped. The employee's back started hurting, and they called their supervisor and went home for the day to go see a doctor. Employee has seen a doctor but states they have not gotten their paperwork from the doctor yet 07/25/2022

Employee was exiting 627G Scraper when they missed a step while descending. Employee fell backwards and landed on their lower back area and hips.
Employee was operating a scraper, transferring bentonite and ran over a large rock on the edge of the haul road. The resulting impact from rolling over the rock jarred and strained the employees lower back.

EE lifted a cutting edge to install on a 627G scraper and twisted lower back. He felt pain then and for the next few days before going to a Chiropractor on 06/03/2013 who told him bone spurs prevented proper alignment of the back. He is going to another doctor today, 06/10/2013 for further diagnostics. He is currently off work.

EE was cutting metal with an abrasive saw when a piece of metal passed by his face shield and glasses and embedded in his eye. He thought it might extract itself but he eventually went into emergency room to have it removed the next day. It was a mild embedding and was quickly extracted by the doctor. There will be no lost time from this accident.

The employee was involved in push-pull scraper operation. He was in 2nd scraper and hooked to 1st scraper with the bale. The 1st scraper moved forward causing the 2nd scraper to unhook which caused it to drop down hard, causing a trauma of compression to the center of lower back.

At 4:45 a.m. the driver entered loader put on seat belt and proceeded to load his trailer. He noticed that he was close to the embankment. That's when the edge gave way and the loader slid down the edge and laid over on it's left side. The driver was bounced around in the cab but needed no medical treatment at that time. Later EE went to the Doctor and was checked over.
EE was barring track onto dozer, felt discomfort in lower stomach. He thought it was pulled muscle and waited a few days to see if it got better - it did not. He went to doctor on 02/27/2011 to confirm it was a hernia which will require surgery. No lost time as yet.

EE was cutting hinge off right loader door. He was cleaning weld off with grinder when it kicked back and deeply cut left arm several inches above the wrist. He went to emergency room to have it cleaned and stitched up.
EE ran over a large rock while operating a scraper which caused him to pull groin muscle in right leg. He expects to return to work on 02/23/3009.

EE hit a bump while running scraper and felt pain in upper back near his shoulder. There was a stinging sensation when he would look to his right.
Driving down road at speed during plowing operations, hit a bump, causing his face to strike steering wheel. He sustained bruises - missed 2 days work.
While stepping down out of trailer van, the wind caught van door and slammed it around against employee, knocking him to the ground. He sustained trauma to the right knee and leg by way of bruising and abrasions. He went to emergency room at hospital and had x-rays, which do not reveal any fracturing. Leg was wrapped in a soft-brace, therapy and several days lost time.

Employee slipped off step while in process of starting scraper engine and herniated a disk in lower back when landing.

He went to doctor the following afternoon due to some blood in his urine. He was instructed to take 5 days off to ensure that he would be ok.

EE WAS HELPING MECHANIC MOVE AN ENGINE BLOCK FROM ONE TRUCK TO ANOTHER WHEN HE PINCHED THE TIP OF HIS RIGHT PINKY FINGER BETWEEN THE TRUCK BED AND ENGINE BLOCK, CAUSING IT TO BE SEVERED ABOUT HALF WAY THROUGH THE FINGER NAIL. HE WAS TAKEN TO THE EMERGENCY ROOM TO HAVE IT SEWN UP.
THE DRIVER WAS GOING DOWN THE HILL ON HAUL ROAD, THE TRUCK BEGAN TO GAIN SPEED. THE BRAKES WERE APPLIED ALONG W/ENGINE BRAKE. THE TRUCK WASN'T SLOWING DOWN FOR THE CORNER AT BOTTOM OF HILL. TH E AIR PRESSURE WAS LOW & THE DRIVER ENTERED THE CORNER TOO FAST. TRUCK LAID OVER ON LEFT SIDE. DRIVER WAS PINNED BETWEEN SEAT & DOOR, RECEIVED CUT ON LEFT CALF & HEAD.

HE HAS HAD PREVIOUS BACK TROUBLE AND SURGERY IN YEARS PAST THE SAFETY OFFICE RECEIVED THE INJURY REPORT ON JAN 18 2002.
