Occupational Illness diagnosed as being caused by prolonged noise exposure.
Employees shift ended at 7:00 p.m. and at 7:10 p.m. while employee was leaving the facility on personal motorcycle, employee made contact with an ecology block causing an accident. Employee received minor abrasions and a torn calf muscle in employee's left lower leg.
Employee was using a guide rod to align the bolt hole on a liner prior to final placement. Employee had placed their right hand on the liner to assist with guiding the liner. When the liner was going into place between the other liner, the employee hand remained on the liner and was caught between two liners causing the right proximal phalange to break near the upper knuckle.

THE ELECTRICAIN WAS USING A HACKSAW TO CUT AN ALUMINUM LUG THAT WAS HELD BY A VISE. WHEN THE SAW BLADE CUT THROUGH, THE EMPLOYEE;S FINGER STRUCK THE EDGE OF THE LUG. IT TOOK 2 SUTURES TO CLOSE THE WOUND.
EE TOOK LEAVE ON 2/8/02 TO HAVE AN OLD BACK CONDTION TREATED. TODAY 2/22/02 WE WERE INFORMED THAT THE DR CONSIDERS THE CONDITION OCCUPATIONALLY RELATED. THE EE HAS A BULGED DISK IN AN AREA THAT HE HAD SURGERY ON IN 1995 AND THAT WAS NOT DETERMINED TO BE OCCUPATIONALLY RELATED AT THAT TIME.HE HAS BEEN DEALING WITH INFLAMATION, PAIN & MUSCLE SPASMS SINCE 1995 BUT DID NOT HAVE AN
EMPLOYEE SCRAPED HIS RIGHT ARM IN SEVERAL SPOTS WHILE CLEANING A CYCLONE SCREEN FULL OF CRUSHED ROCK. THE SCRAPES WERE MINOR, HOWEVER A COUPLE OF SPOTS BECAME INFECTED. HE DID NOT REPORT THE INCIDENT AND DID NOT VISIT THE DOCTOR UNTIL 1-10-02. HE IS BEING TREATED FOR INFECTION WITH A COURSE OF ANTIBIOTICS.

EMPLOYEE WAS WORKING IN THE MILL TORQUING BOLTS ON A DOUBLE DECK SCREEN WITH A 600 LB TORQUE WRENCH. EMPLOYEE WAS STANDING BETWEEN TWO SUPPORT BEAMS HOLDING ONE SUPPORT BEAM WITH ONE HAND WHIL E PULLING ON THE WRENCH WITH THE OTHER WHILE PULLING ON THE WRENCH HE FELT PAIN BETWEEN HIS SHOULDER BLADES.
