Employee was cleaning using a Bobcat MT55 (walk behind loader). Employee became pinned between Bobcat machine and belt frame. Torso and upper legs were pinned.

Employee was operating a 972M Loader. Employee felt something wet on arm, and saw was bleeding from back of right arm, just above the elbow. Employee discovered a small puncture, but has no idea what caused it. Couldn't get bleeding to stop, so went to clinic. A stitch was used to close wound.

Clearing brush and trees from hillside, came into contact with Poison Oak. Employee is allergic apparently. Poison Oak has spread to various parts of his body.
Grinding chisel for use on F250, got metal grindings in right eye, past safety glasses and face shield.
Worker was putting panels on the racking system and hit his head on a cross rail. EE was wearing hard hat when the accident happened.
Jammed pinky finger while installing a v-belt resulting in a dislocated finger.
Worker was adjusting frames, he tripped lost his balance and fell on a frame.

EE ASSURED HIS SUPERVISOR THAT HE WAS NOT INJURED AND RETURNED TO HIS SHIFT. THE NEXT DAY EE COMPLAINED OF BACK PAIN, AND WAS SENT FOR MEDICAL EXAM.

EE WAS GRINDING A STEEL HARD LINE [HOSE] WHEN HE CUT HIS LEFT HAND ON GRINDER. HE CLEANED & BANDAGED THIS MINOR LACERATION. EE DEEMED NO OTHER TREATMENT WAS NECESSARY. CUT BECAME INFECTED TWO DAYS LATER & EE SOUGHT MEDICAL ATTENTION. SURGERY WAS PERFORMED TO CLEAN & DISINFECT WOUND & TO CHECK FOR CARTLIDGE DAMAGE-NEGATIVE.

EMPLOYEE WAS WORKING CLOSE TO BUCKET AND WAS NOT EXPECTING MACHINE TO ENCOUNTER UNEVEN TERRAIN. EMPLOYEE WAS STRUCK WITH THE LOADER BUCKET WHEN MACHINE MOVED TO AN UNSTABLE SLOPED AREA.

EMPLOYEE WAS ATTEMPTING WITH PRY BAR TO DISLODGE A LARGE ROCK THAT WAS BLOCKING THE THROAT OF THE CRUSHER. THE PRY BAR CAME INTO CONTACT WITH THE CRUSHER AND SHOOK VIOLENTLY. EMPLOYEE ATTEMP TED TO GRAB BAR AND WAS CUT IN THE RIGHT HAND.
EE WAS ATTEMPTING TO REMOVE A ROCK FROM THE UNDER CRUSHER CONVEYOR BELT. EE S GLOVE AND HAND WERE CAUGHT IN A RETURN IDLER AND HE WAS UNABLE TO FREE HIS HAND AS IT WAS PINCHED BETWEEN THE BELT AND IDLER. THE IDLER AND CONVEYOR BELT CAUSED ABRASION TO RIGHT FOREARM, HAND AND FACE.
