Employee was cutting a rubber replacement piece when the knife slipped and cut EE's right thigh open.
Person was reloading staple gun. When returning to operations they pressed the trigger and put a staple into hand.

EE was working on the bulldozer when EE lost EE's footing and bumped EE's leg on a metal object. EE started having swelling & redness in that area of EE's shin a couple weeks later. EE seeked medical attention & it is an infection and doctor prescribed an antibiotic & at least 3 days off work.

Employee was cleaning up some scrap metal and he was lifting a metal cabinet when the cabinet fell and it fell on his finger. When the cabinet fell it put a deep cut in his finger.
Reassembling V-screen driveshaft. While adjusting tension on a lift plate using overhead crane, lift plate unexpectedly shifted and pinched tip of thumb under a large nut used to hold lifting plate.
Employee was measuring the depths of the holes for blasting. The ground was wet & muddy, so when the loader backed up a stone flew out from under the tire and hit Employee in the left leg(shin). He fell to the ground scraping up his right leg & knee.

Employee was driving the loader when he drifted off the road when he was hanging up his microphone for the CB and when he drifted off the road he hit a rut in the road and hit his head on the rops cage in the loader.
Employee was removing boards with hammer/pry bar when the board broke loose and he slipped and fell into the wall onto his shoulder. Had an MRI done a few weeks later and found out he has a detached rotator cuff which he has a surgery scheduled for May 20th. Employee and employer did not find out extent of injury till 4/22/13.
