The employee was working to remove the impeller from a pump. When the impeller broke loose, something struck his hand, or his hand struck something inside the pump casing (he is unsure). The employee suffered a broken pinky finger and a laceration to the same finger.
The employee was performing work on a clogged shoot to a sump. The breaker bar he was using fell and pinched his band between the bar and the grating that covers the sump, splitting his finger open. He was wearing gloves. As a result of the incident, several stitches were needed to close up the wound.
Four employees were working to repair a blown pipe. While holding the fitting up on both sides, one of the employees hands slipped, causing it to swing into the groin of another employee holding it in place. The employee felt pain, but did not think he was injured. On 10/29/14, the employee said the pain was worse and sought medical attention.
An employee was rolling over a piece of Novaflex hose when the inner support wire caught his hand and punctured the skin. A physician prescribed an antibiotic to prevent infection. No sutures were needed.

The employee slipped on a sidewalk, striking his elbow. An initial medical examination showed no injuries and he was only given work restrictions for the remainder of the day. Months later, pain STARTED in his shoulder. An MRI in July 2014 showed AC degeneration and tendonitis in the right shoulder.

Employee was working on cleaning shot/adjusting screens on the rev separator matrix. Employee reached down into rev matrix to grab a screen panel that was misaligned and cut his middle finger on the sidewall of the matrix. The employee was wearing gloves prior to grabbing the screen, but removed his glove so he could grab the screen easier. The cut required several stitches.
EE picked up a carry bag containing a chainfall and felt a pain in abdomen. EE checked out by a doctor and cleared to return immediately back to work. The doctor stated that he had a hernia and noted limitations, none of which would prevent him from doing his assigned job tasks. EE came to work on 10/15/12 with a new form from a new dr stating the limitations/no work w/hernia.
While walking on flat ground, the employee felt a sharp pain in his foot. After a day, the pain didn't go away so he sought medical attention. The nurse practitioner's best guess is that his left foot was sprained or that there may be a stress fracture. The x-ray did not show any damage to the bone.
The employee was stepping off of a Gradeall onto a rocky surface and suffered a sprain to her lower leg.
