EE stated that he was kneeled down adding hydraulic oil to the shuttle car and when he attemted to stand up, his right knee would not straighten. EE saw the Dr. who applied a motion limiting brace to the knee and he returned to work on the next scheduled shift.

Employee was using the MRS Remote when he stepped on a crib block and strained his left knee. He continued to work until 7-13-09 when he missed work in preparation for surgery on 7-14-09.
EE was installing cable roof bolts, when a small piece of rock appx 6" in dia. X 1/8 " thick fell and struck him on the nose, resulting in 12 stitches to the 1" long laceration.

Employee was assisting in removing the drums from the continuous miner when the bar they were using in the railroad jack slipped causing injured to fall backward. He was transported to the hospital with a broken knee cap and miniscus. Employee had surgery on 11-26-08 and is a lost time injury.
An unintentional roof fall occurred in the #2 entry 60 feet outby spad #717. The fall was approx. 16 feet wide x 18 feet long x 70 inches thick. No persons, equipment nor ventilation was affected. The fall has been inspected by MSHA and roof trusses are being installed as roof support.
EE was operating the shuttle car when he suffered a heart attack.
This is a report a continuation of a rock fall reported at spad 511 on 5-6-08. The rock fall has extended toward the south west mains to spad 601. The fall has not affected persons, equipment nor ventilation.
A rock fall was discovered in the #1 entry 20 feet outby spad 534 in the 2nd sw mains. The fall was aprox 20' long x1 8' wide x 64 inches thick. The fall did not affect persons, equipment nor ventilation.
A roof fall was discovered 70' outby spad 521 in the #2 entry of the snd sw mains. The fall was apx. 20'long x 18'wide x 64 " thick. The fall did not affect persons, equipment nor ventilation.
A roof fall was discovered in the 2nd west mains in the #3 entry at spad 509. The fall is approximately 20'long x 20' wide x 70"high. The fall did not affect persons, equipment nor ventilation.
A roof fall was discovered in the #2 entry 20 feet outby spad 511 in the 2nd west mains. The fall is appx. 18'wide x 20' long x 64 "high. No persons, equipment nor ventilation was affected.
The #1 MRS was 150 inby spad #635 when it became disabled and would not tram. After attempting to correct the problem the roof conditions deteriorated and fell in on the MRS. The fall area was approximately 20'x20'x 10 feet thick. No persons were affected by the fall. MSHA was contacted and monitored the recovery process.
Employee had drilled a hole and stuck the roof bolt in the hole and was preparing to push the bolt up. The bolt didn't align with the wrench so he was going to bump the rotation and caught his glove around the roof bolt. He received 22 stitches to his left hand. He returned to work underground on his next scheduled shift.
EE stated that he was changing a fitting on a shuttle car and went to remove a crib block, when he caught the index finger on his left hand, crushing the tip. Approximately 1 inch of the finger tip was removed surgically. EE returned to work on the next scheduled shift.

A ROCK FALL WAS DISCOVERED IN THE #2 ENTRY APPROXIMATELY 70 FEET OUTBY SPAD #533 DURING THE PRESHIFT EXAMINATION AT 8:30 PM. THE FALL WAS APPROX. 20' LONG X 18' WIDE AND 10 FEET IN THICKNESS. THE ESCAPEWAY WAS REROUTED, THE ROCK FALL CLEANED UP AND THE ROOF RESUPPORTED WITH ARCHES. IT DID NOT AFFECT PERSONS, VENTILATION NOR EQUIPMENT.
A roof fall above anchorage occurred in the belt entry one crosscut from the outside. The fall was approximately 25'x18'x7'high. The fall was reported and investigated by msha. The area is being cleaned up and arches installed in the area.
Employee was backing the miner out of a cut when a small piece of rock fell from the roof, struck him on the back and caused him to fall on the remote box for the continuous miner. He was treated for bruising to the abdominal area and returned to light duty work on the next shift.
EE WAS WATCHING THE MINER CABLE WHEN A PIECE OF ROCK APPX. 12 INCHES X 12 INCHES UP TO 1 INCH THICK FELL FROM THE ROOF AND STRUCK HIS RIGHT RING FINGER. HE RECEIVED 9 STITCHES AT THE HOSPITAL AND RETURNED TO WORK ON THE FOLLOWING SHIFT.
EE WAS CUTTING BELT WITH A UTILITY KNIFE, WHEN HE CUT HIS LEFT THIGH. HE WAS TRANSPORTED TO THE HOSPITAL WHERE RECIEVED 5 STITCHES TO HIS LEG. HE CONTINUED TO WORK AND IS NOT A LOST TIME ACCIDENT.
EE stated that he was moving a continuous miner cable over to the rib and had turned, the rib rolled off and struck him on the right leg in the calf and knee area. EE's knee was pinned against a parked shuttle car by the material from the rib. EE was transported to the hospital where it was determined that he had a fracture to the tibia near the knee.
Employee was standing on a step ladder, attempting to hang a cable from the roof when he fell off the ladder striking the ladder as he fell. He was transported to MARH where it was determined that he had fractured 3 ribs. He returned to work on the next regular scheduled shift on light duty without loss of time.

Employee was using a utility knife to cut the conveyor belt when the knife slipped and cut his right leg. He consulted the doctor and received 2 stitches. He did not lose any time from work.
As EE was trying to center bolt wrench in chuck, the wrench started to spin twisting right hand around wrench causing two fingers to be lacerated.
