Mt. Cydonia Sand Plant I Metal/Non-Metal

EE was cleaning catwalks and washing the wash plant. when EE was done they started walking down the steps but forgot something and walked back up the steps. EE then went to walk back down but tripped over the hose that they had just used. EE slid down the steps landing on their buttocks bruising lower back

EE claims was opening the gate leading to the plant. The latch was hard to open so EE kicked the latch while holding the gate. The latch struck EE's hand causing a laceration. EE received stitches to close the laceration.

EE was washing the windows of 986k loader. When EE stood up EE suddenly blacked out. When the EE came to, was lying on back on the loader platform. EE did not fall off the loader. EE felt pain in lower back and rib area. EE was taken to the local hospital for evaluation.

An EE was trying to change out the bucket on the skidsteer. The bucket would not easily release. While "shaking" the bucket, another EE walked up to see why the bucket wouldn't release. As they approached, the bucket suddenly released, falling to the ground and striking the EE on top of left foot causing a contusion.

EE was operating CAT 988K, was driving on a haul road in the south pit. Drove through a wash out that was cause by heavy rain the night before. Felt a sudden pain in lower back which was diagnosed as a lower back muscle strain.

Small stone on landing surface caused right ankle to twist, was working at top of primary crusher

EE was climbing into a Dozer when EE went to grab the door handle, EE slipped and fell 7 feet onto EE's back.

Employee was attempting to clean hydraulic oil off the windshield of the loader, while stretching for the windshield, they lost their footing and fell onto the steering cylinder.

Employee strained lower back lifting a turbo onto an engine manifold. Note: Employee received initial medical treatment on 12/07/2017.

OPERATING CONTROL BOOTH FOR PLANT DIRT GOT IN EYE.

Driver was getting out of truck and slipped.

EMP WAS REMOVING A ROLL OF BELTING FROM A PICK UP TRUCK, STANDING ON TAILGATE, WHEN CABLE BROKE AND EMP WENT TO THE GROUND. EMP HURT HIS LEFT LEG, FOOT AND ANKLE.

EMP WAS USING A SLEDGE HAMMER ON STONE BIN AND FELT PAIN IN NECK AND ARM. EMP WENT TO HIS FAMILY DOCTOR ON 8/5/13 BUT DID NOT REPORT INJURY TO H&S DIRECTOR UNTIL 8/16/13. EMP WAS GIVEN RESTRICTIONS ON 8/26/13 AND MAY NEED SURGERY

Left Ankle/Foot - Loading a walk behind saw onto it's trailer, it reared up hitting ankle/foot causing ankle fracture

emp was lifting sheaves and guard into place and hurt his left shoulder. emp is off work

Employee hurt his lower back while lifting and twisting as he tried to change a tire.

Emp was chipping slag from weld and got debris in his right eye. Emp did not report incident until 03/11/2011.

Emp was using a utility knife, it slipped and cut his left arm. Stitches required.

Employee hurt his right ankle and shoulder while trying to climb into his loader and slipped.

EE tripped on rock and tore ligament.

EMPLOYEE WAS WORKING ON A TRUCK WHEN HIS KNEE "POPPED" OUT. INJURY NOT REPORTED UNTIL 2/28/01.

DUE TO RAIN, THE CLAY/SILT SURFACE BECAME SLIPPERY WHICH CAUSED THE EUCLID TO SLIP OVER A SMALL EMBANKMENT. NO SETABELTS WERE WORN BY THE DRIVE OR THE PASSENGER/TRAINEE.

WHILE USING A SLEDGE HAMMER, THE EMPLOYEE BEGAN FEELING PAINS IN HIS THUMB ON HIS RIGHT HAND.

CUSTOMER WAS TARPING TRUCK WHEN OFFICE PERSONNEL HEARD SOMEONE SCREAMING FOR HELP. AN AMBULANCE WAS CALLED AND CUSTOMER WAS TRANSPORTED TO HOSPITAL.

EE WAS INSTALLING A SCREEN ON THE SHAKER AND CUT HIS FINGER ON THE EDGE OF THE SCREEN.

EE WAS PREPARING THE MANLIFT FOR USE. HE LIFTED THE SIDE ENGINE DOOR TO START THE MANLIFT AND LET THE DOOR DROP ON HIS HAND LEFT. HE RECEIVED A CUT BETWEEN HIS LEFT THUMB AND FOREFINGER WHICH REQUIRED STITCHES.

INJURED WAS IN HIS PICKUP STOPPED ON BLACK GAP ROAD WAITING TO MAKE A LEFT HAND TURN INTO MT. CYDONIA PLANT #2 WHEN A WOMAN DID NOT SEE INJURED STOP AND FAILED TO STOP HER CAR BEFORE STRICKING THE BACK OF INJURED'S PICKUP CAUSING A SUDDEN JOLT AND INJURING INJURED'S BACK.

FILLING BIN TRUCK STANDING ON LEDGE AT TOP OF STEPS, SLIPPED OFF AND FELL STRIKING AND BREAKING RIGHT HEEL.

THE EMPLOYEE WA8 INSTALLING A SPRING ON A TRUCK, THE SPRING WEIGHT WAS OVER 200 POUNDS AND THERE WAS NO ROOM TO USE A JACK.

HELPING REPAIR 7RACK ON SHOVEL EMPLOYEE WAS HOLDING BAR TO PUSH OUT PIN ON TRACK ROLLER HAMMER GLANCED OFF PIN & STRUCK EMPLOYEE ON FACE UNDER LEFT EYE
