EE was cleaning catwalks and washing the wash plant. when EE was done they started walking down the steps but forgot something and walked back up the steps. EE then went to walk back down but tripped over the hose that they had just used. EE slid down the steps landing on their buttocks bruising lower back
EE claims was opening the gate leading to the plant. The latch was hard to open so EE kicked the latch while holding the gate. The latch struck EE's hand causing a laceration. EE received stitches to close the laceration.
EE was washing the windows of 986k loader. When EE stood up EE suddenly blacked out. When the EE came to, was lying on back on the loader platform. EE did not fall off the loader. EE felt pain in lower back and rib area. EE was taken to the local hospital for evaluation.
An EE was trying to change out the bucket on the skidsteer. The bucket would not easily release. While "shaking" the bucket, another EE walked up to see why the bucket wouldn't release. As they approached, the bucket suddenly released, falling to the ground and striking the EE on top of left foot causing a contusion.
EE was operating CAT 988K, was driving on a haul road in the south pit. Drove through a wash out that was cause by heavy rain the night before. Felt a sudden pain in lower back which was diagnosed as a lower back muscle strain.
