During the process of replacing clips (brackets) in the Filter Press, EE was accessing the area behind the door to install on clips, when they suddenly twisted their right ankle.

EE was working on replacing screens on the scrubber screen. The top deck oversize grizzly panel was removed and set on the inspection bench. While pulling the screen panel out, the screen caught the grizzly panel which then fell on the mechanics left foot.

EE using pneumatic torque gun. Positioned tool on lug in the 6 o'clock position. EE is right hand dominant & was holding grip with right hand & used left hand to set socket on lug. Then placed left hand under barrel of gun. Squeezed trigger with right hand before moving left hand off of barrel. Resulted in distal end of left index finger amputation.
IW was moving a pallet from a stack to the floor, dropping it on the ground. Pallet tipped forward striking the employee's wrist causing injury
Employee was at the Scrubber Screen working on/inspecting grease lines before start-up. EE was leaving the upper work platform and stepped on an uneven piece of floor grating. EE's right foot landed on a piece that had a raised edge. EE's right foot rolled to the right side.
EE was working on preparing a media batch. Incident occurred when coworker was placing media barrel off the dolly. The barrel slipped while it was getting unloaded. EE at the time was guiding the barrel of dolly and the slipped barrel crushed EE's finger between another barrel.

EE reports that EE was driving the forklift in the Lower Plant Loading area when EE hit a pothole in the ground. Because of the jolt of the forklift EE felt pain in EE's lower back. At that time, employee chose no medical treatment. On 1/13/2023 the employee visited the doctor where EE was prescribed 6 chiropractic treatments.
Quarry Haul Truck driver missed last step and fell to ground. Right leg was caught in last step on the ladder and hurt right ankle. EE was taken to the doctor and they were diagnose with a strain of right Achilles tendon and Contusion of right foot. EE was given an immobilizing device.
A mechanic was replacing a conveyor belt in the lime plant. Lime fell from the upper level onto the mechanic's goggles. When the mechanic adjusted the goggles, lime fell into eye. After flushing the eyes on-site, the mechanic was taken to a doctor, prescribed 7 days of a prescription eye medication, and released to full duty for next scheduled shift.
Mechanic was kneeling down on one knee and bent forward to turn a 10" gate valve. When EE stood up from one knee, EE felt pain in lower back. EE reported this to the supervisor immediately and received first aid but did not seek medical attention until the following day when pain had increased. A doctor gave EE restrictions that prevent EE from doing the full job.

Employee was tramming JLG when they engaged a pothole full of water. As the tire dipped the energy transferred to the bucket and caused employee to bounce up and when they came down they twisted their right ankle.
When completing a vehicle inspection, the heavy equipment operator lost EE's balance and hit EE's elbow on a metal railing. The miner was given moderate duty work restrictions.

After installing screens at HMS Wet Screen in the morning & DSF Midwestern Screens in the afternoon, at the end of the day employee felt muscle spasms on the right side of neck and mentioned it to the leadman. Employee was still feeling pain the next day and requested to see a doctor.
Employee was talking to the employees working at Sacker #1 at the Pool Warehouse. As the lift table lowered, the employee's foot was caught below the motor housing, crushing the employee's foot. Employee was diagnosed with a broken bone in right foot.
Employee was carrying wire spools up the stairs at the tertiary crusher. On the second or third step, EE lost EE's footing and fell backwards striking EE's head on the concrete floor. Employee sustained a laceration requiring stitches.
Employee felt pain in EE's left shoulder while working at the rock picking station. Employee reported on 1/28/16 but did not request medical treatment. The employee requested medical evaluation on 3/22/16 and was placed on restricted duty.
Employee was working at the sacker #1. EE took a step toward the pallet with a full 50 lbs. sack in hand. Placing a bag requires stepping toward the sacker rotating EE's body approximately 90 degrees clockwise. Employee states that as EE planted in right foot EE got a sharp pain in EE's lower back and it ran to EE's thigh (the glute and hamstring).

Employee was carrying empty bags into the warehouse and tripped over a pallet, causing him to fall.
Employee was walking down a 3-step stair and missed the second step causing him to fall. Employee is feeling pain in his left leg and was release to work with restrictions
Employee was working to repair a screw conveyor. Due to the configuration of the area, employee had to climb in and out of the area and kneel during the repairs. Employee was wearing knee pads, but they were worn and did not offer much protection. Employee's right knee was sore and swollen at the end of the day.

Employee was working with a wheel barrow moving rock. His right foot slipped causing him to fall to his knees. Wheel barrow handle struck him under his right arm pit causing right shoulder strain.
Employee felt pain in the groin area while sacking on 9/24/12. Employee reported internally, but did not request medical treatment. On 7/17/14, employee reported worsening of pain and requested medical evaluation. Employee was diagnosed with a hernia and had surgery 9/2/14.
Miner was dumping a wheel barrow of rock, when completed and started to pull the wheel barrow back to him he felt a pain in his left shoulder.
Employee indicated pain and swelling in right knee at the end of the day after kneeling and twisting working on installing spring assembly on dump truck.

After installing toe boards along catwalk employee felt swelling on right knee. This is being reported at this time as a result of employee reporting pain in right knee on 1/30/12 with doctor's visit and first report on 1/30/12 indicating restrictions of no repetitive squatting or kneeling.
Employee was shoveling some wet, heavy sand during housekeeping and felt pain and bulge in left side. Doctor's note indicates employee having surgery to correct left hernia.
While sacking 50 lb. sacks of sand, employee indicated left shoulder started to hurt
Employee was shoveling and got a sharp pain in his back.

Strain/lower left back while trying to push full pallet with sacks down the pallet rolls.
Employee was climbing up ladder to change a light bulb when he slipped on the ladder rung and hit his knee on the railing.
Employee was cleaning build-up on D Conveyor catwalk, as he was shoveling he hit his finger on the handrail.

Using hand crank to move BW-9 belt conveyor at the DSF plant to new location felt pain in right elbow and wrist.
Switching gas valves at kiln, struck his left knee below knee cap on portable steps and caused laceration.
Removing cap off of pipe with bar and hammer, bar moved and hit left thumb with hammer.
Employee was removing a wire brush from a hand grinder when wires from the brush punctured the glove and the palm of his right hand.

Hit his left ring finger with the handle of grease gun while greasing journal on #1 mill.
EE was using hydraulic bearing puller to pull a bearing off of a shaft, when puller fingers slipped and striking his left pinky finger, resulting in a fracture to the bone at the knuckle on his left hand.

Employee was loading a bulk truck. He thought the tank may be empty so he reached outside the handrails and stuck his right hand in a 3"x4" opening to check for material flow. The air-operated cylinder closed the clam shell gate catching his hand between the cylinder rod clevis and the opening.

Worker was on his way to an area at the middle of his shift and drove over a drainage swale across a main road causing the loader to bounce and swerve. The operator hit his head on the window and frame on the right side of the cab. He was wearing his seat belt and aware of the drainage swale.

WHILE WALKING ON THE WEST SIDE OF THE TRUCK SHOP, THE EE CLAIMS HE STEPPED ON A ROCK AND TWISTED HIS ANKLE. THE INJURY WOULD NOT HEAL, HAD SURGERY ON 3-9-99
EE WAS LOADING CLCINE IN 2500LB SUPERSACKS. HE WAS MOVING A PALLET ONTO THE PLATFORM FOR THE NEXT SACK WHEN HE FELT A PAIN IN HIS LOW ABDOMINAL AREA AND RIGHT LEG. WE SENT HIM TO THE DR AND UP ON EXAMINATION THE MEDICAL FINDING WERE CALCIUM DEPOSITS IN HIS HIP AND TENDINITIS. BY LIFTTING THE PALLETS HE AGGRAVATED THE PROBLEM.
EE WAS CUTTING LIMBS AWAY FROM THE TELEPHONE LINES AT THE RAISCH TANKS WITH A CHAINSAW. THE SAW STALLED HE ATTEMPTED TO RESTART THE SAW, IT BOUNCED BACK LACERATING HIS LEFT HAND MIDDLE FINGER AREA THAT REQUIRED STITCHES.
EE WAS ATTEMPTING TO BREAK UP HARD MATERIAL IN THE BOTTOM OF THE DENSIFIER SCREW CONVEYOR WITH A JACKHAMMER WHEN THE JACKHAMMER SLIPPED AND PINCHED EE'S FINGER CAUSING A LACERATION.
EMPLOYEE WAS DELIVERING A LOAD OF SMALL ROCK TO THE SOUTH END OF QUARRY. EE WAS ATTEMPTING TO UNPLUG PC-1 HOPPER WITH A PRY BAR. THE HOPPER WAS PLUGGED WITH LARGE ROCKS.AFTER DISLODGING THE ROCKS, EE NOTICED BOTH RT AND LEFT WRIST VERY TENDER. ON 8-10-95, HE HAD AN ORTHOPEDIC EVALUATION THAT RECOMMEDED NOT TO WORK.

HE ALSO TOLD AN EMPLOYEE ONE WAY OR THE OTHER I'MGOING TO HAVE SAAT AND SUN OFF.

TH US LEAVE NO RAMP ON WHICH THE DOZER COULD TRAVEL ALSO HE WAS NOT PERPENDICULAR TO THE BANK AND THE DOZER ROLLED ON ITS RT SIDE HE WAS WEARING A SEAT BELT NO INJURY NO EQUIPMENT.

HE HAD MOVED BEHIDN THE FO RKLIFT WALKING TO THE OFFICE AND OUT OF THE DRIVER'S VIEW. THE RDRIVER NOTICED INJURED WALKING BY AND THOUGHT HE WAS OUT OF THE WAY. AS THE DRIVER WAS BACKING UP. THE CO-WORKER CALL TO HIM.
EMPLOYEE WAS WORKING ON A 50TON HAUL TRUCK STEERING RAM, HE & A COWORKER WERE TRYING TO REMOVE SOME VERY LARGE NUTS & BOLTS WITH A VERY LARGE WRENCH & CHEATER BAR. WHEN HE JERKED HIS BACK. HE REPORTED THE INJURY BUT TRIED TO IGNORE THE PROBLEM FOR 6 MONTHS OR MORE. HAD SURGERY 6-13-94 FOR SAME PROBLEM.
INJURED WAS SACKIGN 65 AQ PRODUCTS WHEN HE FELT A LOW BACK STAIN ON 4-26. HE REPORTED TO FIRST AID ON 5-3 AND WE SENT HIM TO THE DR AND HE WAS TREATED FOR THE LOW BACK STRAIN OFF AND ON TO TH IS DATE 12-13-93. HE IS EXPERIENCING MORE BACK PAIN AND HAS BEEN ADVISED NOT WORK UNTIL THE BACK PROBLEM HAS BEEN CORRECTED.
